HOME HEALTH SERVICES WERE PROVIDED BY SKILLED NURSES, HOME HEALTH AIDES, AND PHYSICAL, OCCUPATIONAL AND SPEECH THERAPISTS. PALLIATIVE CARE SERVICES WERE PROVIDED BY A NURSE PRACTITIONER UNDER THE SUPERVISOR OF A LICENSED PHYSICIAN. IN 2019, THE ORGANIZATION PROVIDED 39,824 DAYS OF HOSPICE SERVICE TO 521 PATIENTS, AND MADE BEREAVEMENT CONTACTS WITH 964 INDIVIDUALS. A TOTAL OF 583 PATIENTS RECEIVED 18,049 HOME HEALTH CARE VISITS. A TOTAL OF 474 PALLIATIVE CARE PATIENTS RECEIVED APPROXIMATELY 591 VISITS.
