Develop and direct the implementation of treatment plan throughout the hospitalization. Document information relevant to the care of patients in a professional and timely manner. Develop competency in virtual care environments, including, but not limited to, telephone encounters, video encounters, and use of peripheral devices and software used in the care of remote patients. Conduct in-home assessments within the Hospital-at-Home catchment area as required to advance medical and social care needs of patients. Work with multidisciplinary team to plan safe discharge and transition to post-acute services. Counsel and educate patients and their social support system regarding health promotion, appropriate management of acute conditions, and self-management of chronic conditions. Coordinate care with health care professionals at Parkland Health or within the community including specialty care and allied team members. Interpersonal and Communication Skills Utilize interpersonal and communication skills to maintain a therapeutic and ethical relationship with the patients and their social support system. Demonstrate respect, compassion, integrity, and responsiveness to the needs of the patient. Utilize interpersonal and communication skills to work effectively with colleagues within the care team and the health system.
