Submit applications for prescription assistance; ensures refills and prescription renewals are correctly processed; maintains resource information, applications, and procedures to identify available prescription funding sources; completes applications and referrals for outside funding resources; and maintains all required documentation. Support patients in working toward the outcomes described in their Individualized Service Plans and Behavior Support Plans. Ensures medical appointments are scheduled and met.
