This role involves collaborating closely with multidisciplinary teams, interpreting neurodiagnostic studies, and guiding treatment plans for patients with complex neurologic presentations.
Key Responsibilities
Conduct comprehensive neurologic evaluations for patients with suspected or confirmed TBI and post-concussive symptoms.
Interpret neurodiagnostic tests, including EEG, qEEG, VNG, balance assessments, imaging, and other concussion-focused modalities.
Develop individualized treatment plans addressing cognitive, vestibular, psychological, and functional symptoms related to head injury.
Collaborate with rehabilitation specialists, primary care providers, neuropsychologists, physical therapists, and case managers to coordinate continuity of care.
Provide medical oversight and clinical guidance to technicians and support staff performing diagnostic procedures.
Maintain accurate and timely documentation in accordance with clinical guidelines and regulatory requirements.

Primary Care Plus is looking for well qualified Internal Medicine and Family Practice physicians to join our growing practice in NEW ORLEANS, Louisiana. PCPlus clinics are part of a clinically integrated network designed to provide enhanced care to Medicare Advantage, Quality plan, and ACO members by delivering high quality, coordinated healthcare that produces superior outcomes in a cost effective manner. Our clinical team includes physicians, APRNs, RN Care Managers, clinic support staff, and a Quality Improvement team who work together to coordinate care for patients in both the outpatient and inpatient settings. A robust transitional care and high risk outreach program provides physicians with additional information to support their patients outside of the clinic.
PCPlus also uses a sophisticated electronic medical record system and robust analytical data analysis to identify patients who are high risk for poor outcomes in order to improve their quality of life. Consistent feedback is provided to the clinical team on all business objectives including quality measures, diagnostic accuracy, and hospitalization rates. Physicians are not measures based on productivity or RVUs, but rather on outcomes of their patient panels. Most physicians will care for panels in the 600-800 range and appointments are scheduled for 30-60 minutes per patient.
The Primary Care Physician (PCP) is a Licensed/Board Certified/Board Eligible trained professional in internal or family medicine who plays a key role as part of the clinical operations team providing direct patient care and providing assessments primarily in the ambulatory health care center setting or elsewhere as deemed necessary. It also includes the participation in clinical rounds and conferences plus in-depth documentation through written progress notes and summaries. The PCP will be required to demonstrate the ability to function both independently and in collaboration with other health care professionals. The PCP will consult with the applicable managers and medical directors to ensure compliance with guidelines along with participating in risk and quality management programs, clinical meetings and other meetings as required. The PCP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.

The salary range for this role is $226,000 to $366,000 annually based on full-time employment. Salary Range is defined as total cash compensation at target. The PCP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures. ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.
The PCP will take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient.
It is expected that the PCP will engage with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not).
The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.
The PCP leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.
For patients that are unable to come to the office-in hospital, SNF, LTC or homebound, PCP will engage with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources that may be available depending on the market.
PCP will have an active role in the management of their center and will help cover for other providers who may be out for various reasons. It is also expected that each PCP will take an active role as needed in recruiting patients for the center and additional providers for the company.
Supervises, collaborates with, participates with, or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remains accountable for the actions of the APP while employed with the company.
Performs other duties as assigned and modified at manager's discretion.

EDUCATION AND EXPERIENCE CRITERIA:
MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
Must be able to obtain a State Medical License or already have a current, active State Medical License for the state(s) in which he/she will be working
Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred. Board Eligibility is required
Once Board certified, PCP will maintain board certification in their specialty by doing necessary MOC, CME and/or retaking board exams as required
Must have a current DEA number for schedule II-V controlled substances

DESCRIPTION OF DUTIES
The incumbent supports the delivery of medical care services for detainees held in the custody of ICE at a detention facility through their role as a primary care physician. The incumbent will provide direct patient care to detainees at their facility, who may include adult men and women and children, and will provide clinical oversight and consultation to other medical staff caring for detainees at the facility.
REQUIRED EDUCATION, LICENSURE, AND EXPERIENCE
Degree issuing institution and or program must be accredited by an entity recognized by the U.S. Department of Education
1. Qualification for a physician's degree: a. Have a Doctor of Medicine degree from a U.S. or Canadian program accredited by the Liaison Committee on Medical Education sponsored by the Association of American Medical Colleges and the American Medical Association (AMA); or
b. Have a Doctor of Osteopathic Medicine degree from a program accredited by the
Commission on Osteopathic College Accreditation of the American Osteopathic Association (AOA); or
c. Be certified by the Educational Commission for Foreign Medical Graduates if you are a graduate of a medical school that is not located within a U.S. State or territory or
other jurisdiction, or a Province or Territory of Canada 2. Current, full, and unrestricted license to practice medicine in the state in which the duty station is located. The license must be maintained in a permanent, full, and unrestricted status during the term of employment. Any changes to licensure must be reported to the employer immediately.
3. Must have and maintain an unrestricted license from the Drug Enforcement Administration (DEA) to prescribe controlled medications in the state in which the duty station is located.
4. Depending on the detention facility physician requirement, proof of completion of three years of medical specialty training in Family Medicine, Internal Medicine, Emergency Medicine, or Pediatrics leading to board eligibility in that field, with board certification preferred, and at least 1 year of directly related, professional experience performing professional or scientific work in a field of medicine providing direct patient-care services.
5. For physicians working at family residential centers: a. Board certification preferred, or board eligible, in Family Medicine or Pediatrics b. Family Medicine physicians working at family residential centers must be proficient to provide care to children and adults of all ages, to include the care of female health conditions, supervision of prenatal care, and care of chronic health conditions common to the primary care setting. 6. For physicians working at adult detention facilities: c. Board certification preferred, or board eligible, in Family Medicine, Internal Medicine, or Emergency Medicine.
d. Must be proficient to provide care to adults of all ages, to include the care of female health conditions, supervision of prenatal care, and care of chronic health conditions common to the primary care setting. 7. Must have and maintain one of the following certifications: Basic Life Support for health care providers through the American Heart Association , CPR for the professional rescuer through the American Red Cross or the American Safety and Health Institute.

MAJOR DUTIES AND RESPONSIBILITIES
1. Serves as on-site medical authority within the scope of services provided by IHSC, responsible for developing and coordinating all aspects of healthcare delivery at assigned facility.
2. Contributes to the production of the clinic with expectation to routinely see a patient at least every 30-45 minutes, depending on complexity of the patient encounter. This also includes signing of all assigned patient encounter notes daily. All chart reviews /co-signatures, referrals submissions/reviews, lab reviews, telephone encounters, actions, and message reviews are to be completed within policy guidelines.
3. Clinical supervisory authority over advanced practice providers (APPs), nursing staff, pharmacists, and medical assistants, including fulfillment of APP collaborative practice duties and requirements and delineation of scope of practice for medical assistants.
4. Provides direct care to patients throughout the lifespan within physician scope of practice and performs functions in accordance with currently accepted practice defined by the profession, licensure, certification, and granted privileges.
5. Conducts physical assessments, initial and follow-up assessments for acute and chronic medical condition, ensuring appropriate diagnostic and therapeutic tests and services are available, utilized and monitored.
6. Acts as initial reviewer and/or approver for all outside referrals/consults to specialty services not offered at the local facility.
7. Conducts daily rounds on patients housed in the Medical Housing Unit and/or consults with clinicians as necessary regarding their care, if applicable to the facility.
8. Responds to and/or coordinates response to medical emergencies.
9. Seeks guidance from and provides clinical report to the IHSC Medical Director, Deputy Medical Director, Regional Clinical Directors or other IHSC senior leadership as appropriate.
10. Supports facility performance improvement activities, and grievance and case review taskings.
11. Performs record keeping functions in accordance with program policies and position.
12. Maintains functional proficiency and ease of use utilizing electronic health records as required by, and appropriate to, position. 13. Completes all discipline specific testing, initially and during recertification periods. Medical practice privilege requirements must be maintained.
14. Completes all initial, annual and ad hoc training and health screening / prevention activities as required/assigned.
15. Maintains patient confidentiality, and confidentiality of medical records, in compliance with the Privacy Act and HIPAA regulations in all work activities.
16. Adheres to, and maintains awareness of, IHSC policies, procedures, directives, operational memoranda and accreditation standards as prescribed by ICE/IHSC.
17. Attends and participates in general/medical staff meetings.
DUTY HOURS / ON-CALL REQUIREMENTS
1. IHSC operates 24/7/365. This position is required to respond with physical presence during an emergency activation.
2. This full-time position is required the incumbent to provide rotational on-call coverage telephonically, and the incumbent may need to return to the facility during non-core hours to provide care.
SECURITY REQUIREMENTS
1. The Department of Homeland Security's Personnel Security Unit (PSU) must render a Preliminary Favorable Fitness Determination to be considered a qualified candidate.
2. A Complete Favorable Fitness Determination must be rendered by PSU upon completion of initial and subsequent background investigations to meet the requirements of this position.
PHYSICAL DEMANDS:
1. The work requires some physical exertion, such as long periods of standing; walking or jogging unaided over rough, uneven, or rocky surfaces; recurring bending, crouching, stooping, stretching, reaching, or similar activities; or recurring lifting of moderately heavy patients or equipment.
2. The work may require specific, but common, physical characteristics and abilities, such as agility and dexterity, visual, and hearing capabilities.
3. Must be able to respond to any medical emergency in the facility, via foot, within four minutes and perform CPR/emergency care standing or kneeling.
4. Must have the ability to assist sick, injured, or aging detainees or staff exiting the building during an emergency.
