Key Responsibilities
Deliver comprehensive, patient-focused primary care services within a value-based care model.

Collaborate with a dedicated care team including a Medical Assistant, Scribe, and Social Worker.
Utilize on-site lab services with phlebotomy support and access to RubiconMD for virtual specialty consultations.
Maintain accurate documentation using Canopy and Greenway EMR systems.
Participate in community health initiatives such as wellness classes, exercise groups, and social engagement activities.

Schedule: Monday Friday, 8:00 AM 5:00 PM
Clinic Model: 3 5 providers per site (mix of MDs and NPs)
Transportation Support: Patient pickup service within 5 miles of the clinic
Community Engagement: Regular on-site patient wellness events (yoga, dance, spa days, and more)

Assist with training, orientation and clinical support for new providers, advance practice providers, and clinical support staff.
Fosters a professional, collaborative work environment with all team members and colleagues.
Maintain current New York Licensure and all licenses necessary to provide care in the medical practice.
Management of services in accordance with established organization policies and procedures and other medical programs and guidelines.
Participation in Quality Improvement (CQI) program and clinical projects for practice improvement when requested.
Provide case management with other health care team members.

Pediatric sub-specialists are available around the clock to support these medical services.
Our Emergency Department
The emergency department evaluates and treats patients presenting with all types of emergencies, 24 hours-a-day, seven-days-a-week. It is the only Verified Level II Trauma Center for both adult and pediatric patients on the south shore of Long Island. The emergency medicine clinical team works collaboratively with the in-hospital Trauma Team to provide immediate trauma care to all patients presenting with a traumatic injury. The emergency department also provides immediate access to a 24-7 cardiac percutaneous coronary intervention team and is a Comprehensive Stroke Center. The emergency department's staff includes physicians, nurses, and physician assistants who are trained and experienced at responding to emergent medical problems in patients of all ages. Approximately 90,000 injured or ill adult and pediatric patients are treated every year in the emergency department.
