Since 2009, we've worked together in a state-wide collaborative to develop our practices into Patient-Centered Medical Homes (PCMH). This is a model of primary care initially adopted by the American Academy of Pediatrics about 20 years ago, that has also been adopted by the American Academy of Family Physicians, other primary care groups, and on a national level through the National Committee for Quality Assurance (NCQA). We are proud to say that since we first applied for recognition in 2008-2009, we have been recognized by NCQA as a Level 3 PCMH, which is the highest level, and have continued to achieve recognition with each yearly renewal.
In addition to collaborating with other local, regional and state Blueprint practices, we have also participated in collaboratives for office-based opiate treatment (OBOT), i.e., buprenorphine, sponsored by the addiction medicine department at Dartmouth-Hitchcock Medical Center and involving practices from all around the state.
We use state of the art monitoring equipment for our patients, such as, Holter Monitors, Computerized Blood Glucose monitoring, Continuous Glucose monitoring, and Ankle-Brachial Index measuring.
