What you will do
Evaluates and treats patients with acute, chronic complaints and health maintenance concerns related to specialty.
Obtains complete histories and performs pertinent physical exams with assessment of normal and abnormal findings on new and return patients.
Performs or requests and evaluates diagnostic studies as indicated upon evaluation of the patient.
Administers medications according to the formulary protocol or upon patient-specific approval from the supervising physician, or transmits orally or in writing on a patient?s record, a prescription from the supervising physician to a person who may furnish such medication. All controlled substances in Schedules II-V inclusive require a patient-specific approval by a supervising physician in advance of transmittal of the order or administration of the medication [see Standardized Procedure: Administering, Ordering, Furnishing or Prescribing of Drugs; Formulary Protocol].
Orders and collects specimens for routine laboratory tests, screening procedures and therapeutic procedures, including blood and blood products.
Orders physical therapy, occupational therapy, respiratory therapy, radiology examinations and nursing services.
Performs designated procedures after demonstrated competency, according to written standardized procedures where applicable.
Obtains informed consent, as indicated.
Initiates arrangements for hospital admissions and discharges and completes appropriate paperwork.
As directed by the supervising physician, enrolls patients in investigational studies approved by the Investigational Review Board (IRB), and orders the necessary tests and medications. [see Standardized Procedure: Administering, Ordering, Furnishing or Prescribing of Drugs; Formulary Protocol] Medications that are not FDA-approved or are used for a non-FDA- approved indication (off-label use) require a patient-specific order in advance from the supervising physician.
After appropriate training, assists the supervising physician in the operating room (OR).
Acts as first or second assistant under the supervision of an approved supervising physician.
Performs surgical procedures in the personal presence of the supervising physician.
Recognizes and considers age-specific needs of patients.
Effectively communicates and interacts with patients, families, staff and members of the community from diverse backgrounds.
Recognizes situations which require the immediate attention of a physician, and initiates life-saving procedures when necessary.
Facilitates the coordination of inpatient and outpatient care and services as needed.
Facilitates collaboration between providers and coordination of community resources.
Ensures compliance with legal, regulatory and clinical policies and procedures.
Participates in quality improvement initiatives.
Provides and coordinates patient teaching and counseling.
Education Qualifications
1. Bachelor?s degree or above from an accredited college or university.

What you ll be doing

POSITION SUMMARY/SCOPE:
Family MedicinePhysicians at Santa Cruz Community Health (SCCH) provide comprehensive family medicine services, including preventive care, chronic disease management, pediatric care, reproductive health services, mental health and substance use treatment, in-office procedures, and same-day/acute care services, to a Santa Cruz County patient population rich in socioeconomic, gender, racial, and ethnic diversity. SCCH is committed to shaping a medical staff that reflects its patient population; bilingual English/Spanish is strongly preferred.
SCCH physicians work alongside other medical providers, including nurse practitioners, physician assistants, registered nurses, medical assistants, mental health providers, chiropractors, Chinese medicine providers, and case managers in an integrated Care Team model. Physicians are clinical leaders of their Care Teams and act as supervising physicians for advanced practice providers. Physicians meet regularly with their supervising Site Medical Director and the larger medical department to further their clinical knowledge and skill, discuss patient cases, review performance dashboards, and improve quality and clinical processes.
The new Morehouse School of Medicine Dominical Hospital Family Medicine Residency Program started in Santa Cruz in July 2024, and SCCH serves as the residents' continuity clinic, procedure clinic, and osteopathic manipulative treatment (OMT) clinic. SCCH family physicians serve in the role of clinical faculty for the residency program as they teach and precept residents in continuity clinic 1-4 shifts per week. As clinical faculty, they meet monthly with the precepting team to participate in shared learning and support, can seek a faculty appointment with Morehouse School of Medicine, may mentor resident physicians with longitudinal Quality Improvement Projects, and may teach didactics. The remainder of their time is spent seeing their own patients as a primary care provider.
SCCH family medicinephysicians may also teach students, and they make themselves available to respond to clinical questions from other care team providers including Advanced Care Providers (APCs) and Registered Nurses (RNs). As supervising physicians on their care teams, SCCH family physicians provide consultation visits to other care team providers' patients when requested and assume care of APCs' patients or transfer of care to APCs depending on patient complexity, provider scope of practice, level of education, and experience. They also champion efforts to improve clinical quality measures of the care team's panel, as directed by the Quality Improvement (QI team) and participate in other care team providers' clinical supervision and development including annual performance evaluation and quarterly peer review.
Most medical providers conduct some patient care remotely, usually from home, via telephone or video. 80% of an SCCH physician's time is spent providing direct patient care, while 20% of their time is allotted for clinical “administrative” time. All medical visits are 20 minutes in length, and physicians are expected to see nine patients per 4-hour shift unless precepting resident physicians. SCCH uses OCHIN Epic Electronic Health Record system for visit documentation, e-prescribing, and data collection for reporting on clinical quality.

Requirements:
Board-certified or board-eligible in Physiatry
Valid medical license in Santa Cruz, CA
Strong communication and interpersonal skills
Ability to work well in a team environment
Commitment to patient-centered care
Ability to manage a diverse caseload

Physical Requirements:
Prolonged periods of standing, walking, and sitting while working with patients as well as working on a computer.
Must be able to lift up to 50 pounds.
May be exposed to bloodborne pathogens, physical hazards (e.g., needle pricks, etc.) and chemical hazards.
Must wear the required PPE per company and/or facility requirement.
Must comply with facility vaccination requirements.
