The Role:
As a NursePractitioner at Greater Good Health, you'll be at the heart of our mission to provide exceptional care to our I-SNP population. Our High Risk program is designed for patients with significant health needs and complex conditions to prevent acute care readmission and over-utilization of inpatient care. A GGH High Risk NursePractitioner will provide in-facility, in-home, and virtual clinical support for these patients as well as patients returning home from an acute inpatient admission. The goal is to prevent readmission, but also to provide support for clinical and psychosocial factors that contribute to the cyclical utilization of healthcare.
We want our clinicians to take the time needed to address the patient's needs and concerns. Your responsibilities will include:
* Provides full scope of primary care services including well acute, and chronic condition management
* Conduct patient visits in our client's facilities, in the comfort of the patients' own homes, and through virtual visits
* Manages patient panels and "quarterbacks" patient care with patient's team to deliver high-touch, high-quality care
* Orders diagnostic tests and interprets results
* Performs Annual Wellness Visits and Health Risk Assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific clinical coding and documentation
* Takes medical histories, examines patients, makes diagnoses, and prescribes medication to the extent allowable by state guidelines
* Performs appropriate screenings, tests, and in-office procedures

* Coordinates care with the PCPs and other referral sources

We pay up to $150 per hour; you will receive compensation for each evaluation. We remit payment within 45 days of encounter documentation in our user-friendly EMR.
Typical Appointments
In-person appointments consist of general medical evaluations in your practice. Evaluations are performed once per veteran and typically take 30 to 90 minutes. There are no ongoing treatment needs for these veterans; it is a one-time encounter.
We Schedule Around Your Availability.
Make your own schedule! Our current need is 3 days per week. We efficiently schedule a series of evaluation appointments that last no more than a total of eight hours on each of those days.
No Administrative Tasks Required
Our back-office support team handles all administrative work, including contracting with the Veterans Administration, liaising with contractors, and payment. This approach allows you to spend your time where it should be: with your patients.
Training and Experience
These evaluations make a difference in the lives of veterans and directly impact their access to the ongoing care they need. Our clinical onboarding team of advanced level practitioners is available to train you on working with the veteran population.
