Develop strong transitional care plans and able to help keep track of paneled patients along the whole continuum of care including discharges from the hospital, ER, skilled nursing facilities/nursing homes, and home care.

COPC is the largest independent physician-owned primary care group in the nation. We are seeking an experienced full-time Family Medicine Physician to join Tri-County Family Physicians, 11925 Lithopolis Road, NW, Canal Winchester, OH 43110.
This is a large outpatient practice consisting of (6) family physicians and 3 APPs. There are several medical assistants and patient service reps to support the providers and patients. Job Description

Key Responsibilities
Clinical Care
• Provide comprehensive primary care to a managed patient panel

• Comfortable prescribing opioids when medically appropriate

• Minimum 5 years clinical experience post -training
- Experience from FQHC or community health settings may allow some flexibility
• Recent hands -on primary care experience within the last 2 to 3 years
• Comfort prescribing opioids when appropriate
• Full -time onsite availability M -F, 8am -5pm
• Professional maturity and reliability consistent with leadership -track expectations
