We are looking for a part time 21+ hours per week Van Driver to transport our clients for our Adult Day Health program in our Mashpee location. The hourly rate is $20 and the position is Monday, Wednesday, and Friday split shift, 6:30 - 9:30am and 1:00 - 5:00pm.
The Adult Day Health Program is a medically-based program that helps the participant remain independent while promoting good health and well being.
Job Responsibilities:
Transport program participants according to established safety rules and road regulations, set by D.O.T.
Secure and maintain all required safety equipment in the vehicle.
Follow the Vehicle Maintenance Protocol and keep the Transportation Coordinator informed of all service needs.
Report any problems or maintenance concerns to the Transportation Coordinator.
Keep exterior and interior of vans clean and free of potentially hazardous materials.
Assist at sites as needed.
Maintain CPR and First Aid certification.
Ensure that all passengers have secured seat belts at all times. Ensure wheelchairs are secure with tie downs at all times according to DOT regulations.
Maintain a daily transportation log.
Complete a Daily Pre and Post Trip Inspection Form.
Will drive para van as needed to ensure transportation needs are meet.
Ensure that the Daily Passenger Log is complete, signed and turned into the Site
Education and Experience:
Experience with either elders or the disabled (ADH).
Maintain a valid drivers license with good driving record/proven record of vehicle safety.
Possess the ability to safely push, pull, load, and unload participants requiring wheelchairs.
Possess the ability to work with others as part of a team and communicate effectively.
Successfully pass a preemployment drug test in addition to being enrolled in a drug consortium for random testing.
Complete and maintain CPR and First Aid certified.
Experience driving commercial vehicles preferred.
**Must be able to successfully complete a MA State CORI check, CPS check and company paid, pre-employment physical, drug testing and be enrolled in our drug and alcohol testing consortium.**

We are currently searching for a Full time (40 hours per week) School Liaison for our Family Resource Center (FRC). The hours are Monday through Friday 8:30am - 5pm with one to two evenings per week until 7, and Saturday for events as needed. The hours are flexible to accommodate the nights and Saturday needs. The School Liaison works closely with local school districts to help Family Members navigate through educational and school-related concerns and needs, as well as works with school districts to identify Family Members that may be in need of family support services.
The FRC is a community-based, culturally competent program that provides evidence-based parent education groups, information and referral, mentoring, educational support and other opportunities for children and families. The FRC also provides services specific to Children Requiring Assistance who are having serious problems at home and at school, including runaways, truants, and sexually exploited children.
Responsibilities include:
Serve as a resource to families, schools and the community in improving overall student attendance and graduation rates, reducing truancy rates, identifying systemic issues, barriers, trends and needs, networking and disseminating information, designing solutions and expanding system capacity.
Focuses on understanding and meeting family needs.
Ensure that all Family Members experiencing CRA-related issues are connected to services that meet their individual needs and shall ensure that there is a process established to provide information and referral and direct assistance where possible for individual Family Members
Responsible for negotiating and implementing special events and other inter-agency activities that involve schools.
Provide support for families with school age children in functional areas including meetings with teachers, assistance with obtaining an Individual Education Plan, and meetings with other school personnel, as necessary.
Facilitate FRC Parenting, Family and Youth groups.
Education, Experience, and Requirements:
High school diploma or GED plus 2-4 years of related experience required.
Associates degree preferred.
Effective oral and written communication skills as well as excellent interpersonal skills.
Required to drive a personal vehicle on FRC business.
Must have acceptable MVR, and appropriate insurance coverage.

Must be at least 21 yeas of age.
Bachelor's Degree in Child Development, Early Childhood Education, Elementary Education, Child Guidance, Human Services, Nursing, Psychology, Physical Education, Recreation, Child Psychology, the Arts, Work, Sociology or child Care.
EEC Director Certified required
Must have six months experience working with school age children.
Must have six months of administrative experience or evidence of satisfactory completion of at least nine credits in management or administration subject areas from an accredited institution of high education.

We have an excellent full time opportunity for a Children's Clinician to provide outpatient treatment to individual/families of the Behavioral Health Center in an office and/or outreach basis. The starting salary range is $59,000 - $61,500.
Duties and responsibilities include:
See children and their parent/guardian as assigned for assessments, for evaluations and for ongoing treatment in an office and or outreach basis.
Attend case conferences and multi-disciplinary meetings. Maintain caseload as assigned to perform assessment, treatment, planning and ongoing psychotherapy.
The Clinician should be knowledgeable and experienced providing individual therapy to children and in the use of several of the following treatment modalities; individual, family, and group. Make referrals to other resources, agencies, clinics, and hospitals as needed.

Duties and Responsibilities:

Coordinate member and caregiver family counseling as needed.
Monitor assigned member health status.
Review member personal care services, at least every other month
Complete semi-annual health status reports and multidisciplinary care plans every 6 months.
Provide case management services.
Write progress notes and maintain member/caregiver records in a timely manner.
Plan, implement, and evaluate caregiver-training sessions.
Assist in outreach efforts through public speaking engagements and meetings with area professionals in an effort to increase community awareness of program services.
Attend supervisory meetings both on a team and on an individual basis as required by the program director.

Initiate consultation with program RN's as client/caregiver concerns are identified.
Education and Experience:
Must have graduated from an accredited LPN Program and be currently licensed in the Commonwealth of Massachusetts Board of Registration in good standing.

The Long Term Services and Supports (LTSS) Community Partners (CP) program has an exciting opportunity for an Outreach Care Coordinator (OCC). This is a Full Time (40 hours per week) with a Monday through Friday, 8:00 a.m. - 5:00 p.m. schedule. The position is budgeted to start between $20.50 - $25 The OCC is responsible for making initial contact with hard to reach members and promoting coordination in the delivery and receipt of services to enrollees assigned by Accountable Care Organizations (ACO) and Managed Care Organizations (MCO), or Mass Health.
Duties and Responsibilities:
Respect the confidential nature of all program services and communications therewith.
Conduct outreach to assigned enrollees to inform about the functions of a CP Program, role of the Care Coordinator, the optional nature of CP enrollment, choice in CP providers, the benefits of enrollment and the process for enrolling. Provide written documentation to members as warranted.
Document all outreach attempts which typically include a minimum of 3 phone call attempts; and additional strategies including face-to-face attempts if warranted, and other correspondence.
Make contact with members during outreach to complete Follow Up after Discharge summary as needed.
Schedule Intake appointments for Care Coordinators as appropriate.
Complete Risk Assessment as required.
Inform potential member of Participation Agreement and facilitate sending it to the enrollee.
Facilitate warm hand-off for completion of Comprehensive Assessment with ACO / MCO partners.
Responsible for marketing program to ACO / MCO Health Centers, hospitals, PCP offices and other entities including maintaining collaborative relationships with same.
Responsible to delegate assignments to part-time Outreach Worker as applicable
Determination of caseload assignments per geographic area on a monthly basis.
Complete No Contact, Disengagement, request for information, Intake Packet and other letters as required to send to members during absence of Intake Specialist.
Completion of disenrollment forms for members on assigned caseload.
Assignment as Care Coordinator to assist in gaps in coverage as needed, or in growing area of need in a geographical region, including but not limited to the following duties and responsibilities.
Obtain the Comprehensive Assessment from the referring ACO/MCO, integrate findings into person-centered Care Plan and provide update as needed for any changes in functional status, ADL, or IADL needs.
Ensure LTSS Care Plan is approved by the Enrollee and the Primary Care Provider within required time frames. Ensure Care Plan is shared with all members of the care team.
Facilitate enrollee's access to needed services, including but not limited to, Flexible Services, ACO/MCO chronic disease management programs, LTSS and other Community Resources.
Maintain regular contact with the enrollee to monitor and promote progress toward Care Plan goals; to assist the enrollee to navigate and access needed services: to provide ongoing monitoring of the implementation of the LTSS Care Plan; to monitor for changes in status including health, housing, natural supports and caregivers and to provide health and wellness coaching with a minimum of one face-to-face contact quarterly.
Maintain all documentation required for the delivery of LTSS Community Partners services.
Education and Experience:
Must have strong written and oral communication skills and demonstrated computer skills.
Bi-lingual capacity in Spanish, Cape Verdean Creole, Haitian Creole, Portuguese strongly preferred.
Must have a minimum of High School Degree (or equivalent) and at least two years of experience, or an Associate's Degree and at least one year of experience in the field; or a Bachelor's Degree in social work, human services, nursing, psychology, sociology, or a related field.

Providing intensive in-home family therapy to youth who meet criteria for a serious emotional disturbance as evidenced by CANS assessment.

Goals are designed to enhance problem solving, limit-setting, risk management/safety planning, communication, build skills to strengthen the family, advance therapeutic goals, or improve ineffective patterns of interaction, identify and utilize community resources, develop and maintain natural supports for the youth and parents/caregiver(s) in order to promote sustainability of treatment gains.

We are offering a $1,000 sign on bonus!
We are currently seeking a Full time (40 hours per week with 25 billable expectation) Therapeutic Training and Support (TT&S) staff member in our In Home Therapy (IHT) program. The starting pay range is $20-$22 and the hours are Monday through Friday, between 10:00 a.m. - 6:00 p.m. This schedule will change based on the clinical needs of assigned families.
The TT&S provides intensive family therapy services to a youth up to age 21 and his/her family in order to treat the young person's behavioral health needs, improving the family's ability to provide effective support to promote healthy functioning. IHT is a structured, strength-based therapeutic relationship between a Master's level clinician, a TT&S, the youth, and his/her family.
Duties and Responsibilities:
Have a willingness and ability to work with the SED, DMH and DCF populations.
In-Home Therapy is provided by a qualified clinician who may work in a team that may include a qualified TT&S staff member.
IHT Services may be provided in any setting where the youth is naturally located, in-person and via telehealth when clinically indicated.
Arrange for the collection of relevant information regarding the family's story, current situation, and family strengths and needs.
Implement a care/action plan based upon the child and families voice, choice, and needs under the guidance of an IHT Clinician and team recommendations,
Willingness and ability to work flexible hours, including evenings to meet the clinical needs of the child and family, including but not limited to, the home (including foster homes and therapeutic foster homes), schools, child care centers, respite settings, and other community settings.
Make referrals to other resources, agencies, clinics and hospitals as needed.
Maintain paperwork requirements and timeframes of agency, Mass Health Managed Care Entities and other commercial insurances.
Assess immediate risk for safety and coordinate resources for stabilizing that risk.
Guide the team members towards open, strength based, family friendly problem solving.
Maintain a healing and helpful relationship with youth, young adult and family.
Guide team to implement the interventions of the plan and update as necessary Monitor cases to ensure continued eligibility.
Understand and demonstrate commitment to agency mission, values, and philosophy in performing skills specific to role.
Maintain all ethical and legal requirements of the Agency.
Assume responsibility for meeting and maintaining all licensing, COA or agency requirements for personal health, training or other certifications.
The IHTTS must meet with the In Home Therapy Supervisor on a weekly basis for supervision in person.

Supervision notes must contain, at a minimum, information regarding frequency of supervision, format of supervision, supervisor's signature, and general content of supervision sessions.

Clinicians will provide therapy services - individual, group, couples or family. Services may be office based, school based, community based or via telehealth as appropriate for the clinical needs of the client.
Clinicians will attend case conferences and multi-disciplinary meetings. Clinicians will be expected to maintain caseload as assigned to perform assessment, treatment, planning and ongoing psychotherapy.
Clinicians will make referrals to other resources, agencies, clinics, and hospitals as needed.
Clinicians may be called upon to provide trainings and to participate in the clinic staff training program, as well as being available as a consultant on clinical work issues.
Ongoing supervision will be provided to meet requirement of the agency and licensing bodies.
Clinicians will provide on-call triage to clients of FSA's Behavioral Health Center on a rotating basis. Each on call shift is weekly, Tuesday to Tuesday from 5pm to 9am weekdays and 24 hours per day on weekends and holidays. Clinicians are reimbursed at a rate week per on-call rotation week.
Clinicians will be required to submit a timesheet biweekly.
Full-time Clinicians receive 3 weeks Paid Time off and 11 Paid Holidays. Paid Time off must be used by June 30th of each fiscal year unless the clinician starts in the final quarter of the fiscal year. Such person will have until October 1st of the next fiscal year to take accrued PTO. Full-time clinicians working 40 hours will be expected to bill 56 billable units per pay period.
Education and Experience:
Must have a Masters in Social Work, Clinical Psychology or related field.
Must be licensed or license eligible. - LCSW/LICSW, LMHC, LMFT.
