The Long Term Services and Supports (LTSS) Community Partners (CP) program has an exciting opportunity for an Outreach Care Coordinator (OCC). Job Description * We have an excellent full time opportunity for a Clinician to provide outpatient treatment to individual/families of the Behavioral Health Center in an office and/or outreach basis. The starting salary range is $54,870 - $56,950 Duties and responsibilities: Clinicians will provide therapy services - individual, group, couples or family. Services may be office based, school based, community based or via telehealth as appropriate for the clinical needs of the client.
