Family Care Coordinator
Positions in both Licking and Knox Counties
Duties: In this role, you will provide care coordinationservices to youth clients [age birth to 22, if still enrolled in school] with mental health and substance abuse issues and their families. Implements monitoring system, determines client needs and ensures delivery of needed treatment and services. Screens and assesses clients for their appropriateness for agency services. Conducts interviews with family members, service providers, school personnel and others to obtain information for the assessment. Implements client monitoring systems, determines clients' needs and identifies clients' risk factors. Ensures delivery of needed treatment and services. Meets with individual clients and provides group facilitation. Works constructively with clients to reach agreed upon outcomes and coordinates care with internal and external providers. Shares on-call responsibility.

Qualifications: Candidates must possess strong written and oral communication skills and the ability to collaborate with other service providers. Associate's Degree in Human Services or related field with one to three years of case management experience for individuals with mental health or substance abuse issues required. Qualified Mental Health Specialist (QMHS) required. Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC) preferred. State of Ohio Driver's License; BLS/CPR certification required.

Responsibilities
Actively supports, represents, and extends the mission, vision, and values of the organization.
Provides in home supportive services, transportation, parenting education, counseling, community resource referral, advocacy, and other social service interventions to the youth and family, meeting or exceeding established outcomes thresholds on behalf of persons served.
Travels daily, to provide community based services to, and on behalf of, youth and families in compliance with organizational, contract, and regulatory requirements.
Completes all required documentation in a timely manner.
Completes all required trainings in a timely manner.
Coordinates and monitors services for the youth and family in the community, including but not limited to, mental health, medical, educational, psychological, vocational, and social services.
Regularly reviews and modifies youth based plans to assure consistent progress and success on written plan goals.
Establishes and maintains strong relationship with assigned foster parents and families of origin.
Assists in family reunification and/or stabilization efforts with the youth's primary family or other permanent living arrangement consistent with the youth's case plan.
Provides safety assessment, crisis response, and behavioral stabilization services as required to assure safety and stability for youth served.
Consistently achieves established productivity thresholds.
Participates in the Continuous Quality Improvement activities on a monthly/quarterly basis.
Actively participates in the weekly supervision process.
Minimum Qualifications
Bachelor's Degree in Social Work or comparable Human Services field from an accredited institution.
2 years of work experience working in direct service with youth and families strongly preferred.
Must have flexible availability as needed.
Must have a valid driver's license, reliable transportation, automotive insurance, and a good driving record.
Must have car insurance requirements of 100,000/300,000 bodily injury insurance.
A willingness to work flexible and non-traditional hours in the service of families of origin, and persons served.
Capacity to be flexible and responsive to youth served, foster caregivers, system partners, and internal customers.
Capacity to remain objective and professional in all areas of job function.
Demonstrates tolerance and respect for the ideas and actions of others.
Capacity to effectively work with and be respectful and sensitive to persons from various cultures, socioeconomic, ethnic, religious, and racial backgrounds.

* A bachelor's Degree or at least 120 semester hours with a major course of study (24 semester hours) with content as indicated above.

* Possess 24 months (2 years) working in a group childcare or youth program with completion of DoD approved competency training courses, evidenced by Service-issued documentation.
In addition to the above qualifications, candidates who possess the following qualifications will be considered "Highly Preferred" or "Best Qualified" and may be sent first for consideration:
1. Experience assisting with childcare or youth program national accreditation processes.
2. Experience participating in multi-disciplinary or higher-level child/youth program inspections.
3. Experience working with varying age groups (early childhood, school age, youth).
4. Experience leading or mentoring lower level childcare/youth program staff.

This position is responsible for coordinating patient activities on a nursing or procedural unit including service excellence, equipment/supply management, and tasks as assigned to promote throughput. This role may perform department support duties such as timekeeping, schedule entry, data collection and charge reconciliation.
**Responsibilities And Duties:**

Anticipates needs of patient/family and internal customers, proactively addressing needs - including, but not limited to wheelchair discharges, rounding (i.e., environmental, nutrition, safety and service) and cleaning unit work areas.
Assists public and professional staff as necessary including way finding.
Relays information to unit associates as appropriate.
Act as liaison between unit and ancillary departments.
20% - Department Support Duties
Provides support for assigned unit - including but not limited to Kronos/timekeeping, point of care scanning, copying and entering schedules into electronic format, and tracking/entering data related to unit staffing.
Orients new staff to unit workflow.
Assigns staff to nurse call system.
15% - Throughput
Communicates with patient care staff and other units to keep patient flow moving in a timely and efficient manner.
Assist nursing staff with admissions, discharges, and transfers.
15% - Equipment/supply management and charge reconciliation
Proactively orders/maintains equipment and stock supplies.
Coordinates identification, monitoring, and removal of all equipment alleviating potential clutter in clinical hallways.
Track orders for telemetry and enters patient data in telemetry system. Order supplies for nursing units.
Performs charge verification/ reconciliation.
Coordinates identification, inventory, and maintenance of IT equipment including Vocera hardware, iPad for MyChart Bedside, and/or pagers.

Monitor and manage scheduling of all Customer Engineer's (CE's) and SP service events within the district geographic service area, including assisting scheduling of preventative maintenance, emergency service (in coordination with the CRC), warranty, and start-up. Power District Operations Coordinators also assist scheduling of load banking (requirements, ordering, pickup & delivery) and test equipment.
Manage the site activation/cancellation activity on Service Partner Web (SPW) for new and pending contracts.
Provide physical tags to SP to attach to new equipment.
Review and modify PM schedule dates before releasing tickets to field.
Monitor and track preventative maintenance to ensure on time completion and contract compliance.
Maintain and communicate start-up scheduling.
Prepare and maintain weekend work calendar/duty tech schedule.
Provide PM/FCN due reports to CE to assist in scheduling PM's and FCN's.
