Join a committed team of professionals! FCCP works with families who have a child with behavior issues or diagnosis, where the family is not sure how to get help. Other families are eligible if they are at-risk for involvement with the Department. All families are in need of assistance in navigating services or community resources that may be available to help their family and utilizing family and natural supports to overcome barriers. The goal of FCCP is to strengthen families in becoming more self-sufficient.

The Care Coordinator is a navigator with the family, working with families in their home and in the community, collaborating with natural supports and other service providers to assist the family in identifying their needs and goals. Must be able to address crisis issues & help to stabilize families, as well as safety-planning when needed, as well as ongoing monitoring for child safety and risk factors. The FCCP is strength based and family driven, with the Care Coordinator conducting a strengths-based, culturally competent family needs assessment. The ongoing work is to help the family to meet their goals while increasing their protective capacity with their children, so the children can remain with their parents.

The Care Coordinator will also meet the requirements of documentation for client records and conduct other assessment tools with the family. The Care Coordinator will participate in Wraparound and other required trainings and become wrap certified within one year of employment.

Education/Licensing Requirements : A Bachelor’s Degree in Human Services field required. Bilingual candidates encouraged! Home visiting experience preferred.

Qualifications: The Family Networks Service Coordinator requires a Bachelor’s Degree in related human service/clinical field and one (1) year of case management or direct care experience. Summary
Provides group and individual counseling, community support, case management and crisis intervention services to promote personal growth in the form of emotional, cognitive, behavioral and/or psychological changes in clients with a primary mental health diagnosis. Includes performing continuing assessment of the client's needs and functioning, maintenance of appropriate documentation and development and implementation of treatment plans consistent with the client's therapeutic needs.
