We provide vital support during life's most challenging moments, helping individuals heal, grow, and thrive. We're Hiring! We are seeking a Family Resource Specialist to join our team. This casual part-time position is based out of Door County and is responsible for assessing the strengths and needs of pregnant women and families with infants, determining service needs and providing information, connections, and linkages to a continuum of community services based on strengths and need. Hours for this position will be less than 20 hours per week, Monday through Friday, with a primarily daytime schedule that will vary based on family, clinic, and hospital needs. Key Responsibilities * Visit hospital(s) and assemble list of parents. Conduct visits in the assigned hospital or home and contact all parents who have delivered to explain the benefits of participating in the program. * Complete assessment on each parent accepting the interview in an organized and uniform manner. * Assess family strengths and needs, determine program eligibility, and make appropriate referrals when needed. * Provide information on community resources. * Build and maintain strong, professional working relationship with the hospital staff and other service providers. Qualifications Education: * Required: Associate's degree in child development, communications, marketing or related field or equivalent experience * Preferred: Bachelor's degree in child development, communications, marketing or related field or equivalent experience Experience: * Required: 1 year experience working with families, child abuse and neglect prevention, maternal/child health, or child development. * Preferred: 2+ years' experience working with families, child abuse and neglect prevention, maternal/child health, child development and/or home visitation. Skills and Competencies: * Advanced interpersonal skills to build rapport quickly with others. * Acceptance of individual differences. * Strong organizational and computer skills. * Ability to manage a flexible schedule to meet client needs, which may include evenings. * Valid Wisconsin Driver's License with reliable personal vehicle. Jefferson Center for Mental Health is seeking a self-motivated, independent, Intensive Family Skills Coach & Case Manager who can work effectively on a multi-disciplinary team. Excellent rapport-building skills, strong boundaries and self-awareness, high-level organization and time management, a positive outlook, and creative strengths-based approach are necessary for success. EDUCATION/CREDENTIALS: * Bachelor's degree in psychology, social work, or related field, or * Comparable degree, plus two or more years' related experience and/or training * Motor Vehicle Insurance Coverage requires applicant must be 25 years or older. Must have an acceptable Motor Vehicle Driving record as determine by our insurance carrier. An MVR will be pulled along with a background check. EXPERIENCE/SKILLS: * Trauma-informed training and approach. * Comfort managing intense emotional content. * Experience providing direct services to clients, including: * Case Management (benefits, housing, food, prosocials, etc.) * Motivational Interviewing and Implementation Coaching to assess and improve motivation for change (motivation identification, increasing change talk, goal setting, etc.) * Skills Coaching (parenting, coping, mindfulness, social and life skills, etc.) * Psychoeducation (trauma, attachment, parenting, coping, etc.) * Ability to evaluate and prioritize needs. * Ability to work collaboratively and creatively with a treatment team. * Ability to manage a high level of documentation requirements. * Knowledge of local resources preferred. DUTIES: Willingness and ability to learn and adhere to a specific treatment model. This position will provide TST skills coaching and case management services to adult caregivers and to clients aged 4-21 who are engaged in a high level of therapeutic care, via the Trauma Systems Therapy (TST) Program. The coach/case manager will support the TST treatment process and meet client needs that fall outside of the scope of therapy. They will collaborate closely with the TST therapist and the family to meet agreed upon goals and objectives. They will coordinate care with internal and external service providers, consistent with the current TST Phase of Treatment. Flexibility in work setting/location. Work with clients will regularly take place in the client's home or in the community, including Jefferson, Gilpin, and Clear Creek counties. Expect to serve mountain-located clients. Some duties may be conducted across a hybrid work setting, including telehealth, office-based, and work from home, as approved by supervisor(s). Reliable transportation required. You will drive your own vehicle across the Denver metro area and into the foothills/mountains. You will drive in all-weather conditions. Monthly mileage reimbursement is available. Evening availability required. Must be able to provide evening sessions at least three evenings per week as needed (last session time offered at 7pm). Working days are Monday through Friday. The Intensive Family Skills Coach & Case Manager, as part of the Trauma Systems Therapy (TST) Team, will be responsible for the following: Direct Services: * Understand how trauma impacts the lives of the people being served, so that every interaction is consistent with the recovery process and reduces the possibility of re-traumatization. * Demonstrate ability to work with families with diverse backgrounds including awareness of cultural differences. * Support therapeutic work by collaborating with TST therapist and assisting the family in treatment goals. This will include practicing parenting skills, life skills, coping skills, and supporting follow-through with safety and behavior plans. * Outreach and effectively plan for sessions, in conjunction with TST Therapist, to engage clients and maximize benefit from services. * Provide regular needs assessments and link clients to relevant services. * Collaboratively create case management goals and objectives with the TST therapist, including input from client and family. * Effectively assess and engage clients in ability to solve their own problems and follow through on action plans. This includes completing case management tasks WITH clients, modeling HOW to approach and complete tasks, and eventually assigning the clients homework to complete on their own. * Attend and effectively participate in meetings with client and family to advocate for client needs. * Assess and identify risk factors, including lethality. Report risk factors and concerns to therapist and supervisor. Support engagement in safety plans created with therapist and supervisor. * Maintain appropriate clinical and work-life boundaries within a family systems treatment approach. Indirect Service & Care Coordination: * Coordinate care with internal and external service providers. * Exhibit strong communication and collaboration with a multidisciplinary team to address barriers to treatment and advocate for client needs. * Maintain regular communication with community partners. This may include court hearings, social services staffings, school meetings, or other. Must have a strong understanding of professional boundaries in these settings. * In collaboration with therapist, identify need to link consumer with medication evaluation when necessary. * In collaboration with therapist, refer client to appropriate community providers and options for higher level of care, during and/or after TST episode of care is complete. Supervision & Development: * Attend team meetings, supervisions, and mandatory in-services on time and as scheduled. * Actively participate in planned individual and weekly group supervision by coming prepared with an agenda. Report high risk/problem cases and utilize a problem-solving approach. Contribute to team consultation process. * Openness and commitment to self-awareness, self-evaluation, and examination of instances of transference and countertransference. * Participate in staff development activities to enhance professional growth and follow individualized training plan. Be responsive and timely to directives from supervisor(s). * Volunteer to work on Center committees and/or task forces, as approved by supervisor(s). Administrative & Compliance: * Maintain appropriate service documentation, including billable progress notes, general notes, TST-related paperwork, and other duties as assigned. * Comply with all corporate compliance and HIPAA standards. * Submit paperwork on time and meet Jefferson Center's productivity standards (typically 90% compliance or higher). * Complete data collection, submission, and tracking required by state, Medicaid, grant funding, and other sources. * Complete required monthly trainings on time.
