Responsibilities:
As a Hospice Clinical Social Worker, you will:
Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care which fosters the personal worth, spiritual well-being, and dignity of each patient.
Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
Assess the social, spiritual, and emotional needs/factors in order to estimate the patient's and involved caregiver's capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness.
Prepare the patient to cope with the changes and the chosen family to support the patient including education on advance directives/advanced care planning
Utilize all available resources, such as chosen family, hospice, and community agencies, to assist the patient and chosen family to live better within the limitations of the illness
Support the bereavement program
Provide discharge planning related to change of level-of-care or community placement/location-of-care
Required Certifications and Licensures:
Licensed to practice as a clinical social worker in the state of agency operation if required
Must be a licensed driver who can travel to all business locations

The Residential Counselor at Adult Residential Recovery provides milieu management, psychoeducation groups and ensures safety of individuals in a substance use residential and inpatient program. This is an on call (as needed) position that does not include benefits.
Essential Duties:
* Provides milieu management, guidance, safety, stability, and direction to clients in early recovery from a trauma-informed lens.
* Maintains acceptable attendance and punctuality and is consistently at work on time.
* Interacts with client population to provide guidance, safety, stability, and direction including de-escalating potentially dangerous situations.
* Performs accurate and timely documentation at all times, including but not limited to admissions, shift notes, Per Diem notes, discharges, inventories and screening of clients' personal belongings, individual and group sessions, Medication Occurrences (MOR), and Adverse Incident Reports (AIR).
* Monitors and documents client's vital signs, physical condition and behavior, as needed. Must be able to administer blood pressure assessments, utilizing blood pressure cuffs, when Nurse is not available.
* Assists with the planning, conducting and supervision of patient activities, including community meetings, groups, external meetings on-site, family/friend visitation, recreational activities, and life skills.
* Monitors Breath Alcohol (BA) tests as needed and document appropriately.
* Administers medications to clients per QMAP standards.
* Assist in conducting Random Urine Drug Testing.
* Facilitate or co-facilitate psychoeducation groups.
* Assists team in the coordination of treatment referrals for clients motivated to seek on-going treatment following discharge.

The Family Stabilization Specialist provides crisis mental-health case management to referred cases in order to help stabilize the primary client and, when appropriate, their children and other household members. Services address the full spectrum of high-risk concerns, including domestic and intimate-partner violence, sexual assault, stalking, abuse or neglect of children, elders, and adults with intellectual or developmental disabilities, as well as substance misuse, suicidal ideation, complex family dynamics, co-occurring medical conditions, and severe mental-health disorders.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Serve as a professional advocate and resource for refugee clients and provide equal treatment to all client groups in accordance with the agency's commitment to the dignity of all persons.
Develop individualized Plans of Care to achieve relevant mental health or wellness goals for all FSS Participants through an evidence based, collaborative approach that includes refugee resettlement staff and clients.
Assist in identifying and screening referred client needs in order for appropriate services to be arranged.
Provide resources and assist with mainstream community services referrals to support with stabilization including mental health, housing, food, public benefits, disability, or employment needs.
Provide clients with informal, values-based psychoeducational support and crisis navigation as appropriate, including continued assessment and update of the client's Plan of Care.
Provide preventative family-centered, evidence-informed interventions that combine mental-health psychoeducation, mindfulness techniques, and trauma-informed strategies to create Plans of Care that promote best practices, respect cultural practices, and engage the client's own values.
Facilitate and provide access to translation and interpretation services as appropriate.
Empower and improve client confidence in accessing community resources through skill-building, coaching, and collaborative problem-solving.
Actively foster community partnership opportunities to increase client access to mainstream community services, including mental health services, social connections, and stability resources.
Work closely with agency staff to ensure crisis case management services are provided through a team approach in a sustainable manner.
Maintain professional client case noting of all contacts with the client and document all client services in a timely and accurate manner.

Savio is hiring family therapists! Savio will pay for training for therapists to provide Multisystemic Therapy.
Description
MST (Multisystemic Therapy) is a scientifically proven treatment model that targets youth 12-17 who are exhibiting delinquent-type behaviors such as running away, substance use, defiance, aggression, and truancy. Therapists work closely with parents and caregivers to leverage change by working in families' homes and in the community.
This position provides in-home services for the Denver metro area.

What does an MST therapist do?
Maintain small, intensive caseload of 5-6 families.
Conduct therapy in the community, in families' homes.
Use the following theories: CBT, Motivational Interviewing, Behavior Modification, Structural and Strategic Family Therapies.
Prevent out of home placement/incarceration.
Break cycle of system involvement
Empower families to create lasting change
Engage primary caregiver and other key participants in active change-oriented treatment.
Meet weekly for group supervision and expert consultation.

Starting with candidacy (LPCC, SWC, MFTC): $58,660
Starting Licensed (LCSW, LPC, LMFT): $61,660

Requirements
Minimum Qualifications:
Master's in counseling, social work, marriage and family therapy or related field
Bachelors with extensive experience considered on a case-by-case basis
DORA registration to provide therapy such as LPCC, MFTC, SWC
