Program Overview:
Intercept , developed by Youth Villages, is an evidence-based intensive, in-home program that focuses on keeping families together by preventing out-of-home placements or facilitating reunification after such placements, including foster care, residential treatment facilities, hospitalization, or juvenile detention centers. It is one of the first programs to receive the well-supported designation from Title IV-E Prevention Services Clearinghouse developed in accordance with the Family First Prevention Services Act (FFPSA) having been shown to reduce out-of-home placements and accelerate permanency for children.
This program serves youth from birth to age 18 who are facing serious emotional or behavioral challenges or have experienced trauma, such as abuse or neglect. Services are provided by highly trained Family Intervention Specialists directly in the family's home and community-Intercept is not an office-based program.
Intercept provides intensive, strength-based support tailored to each family's unique needs, ultimately helping them achieve lasting change through evidence-based mental health interventions, along with the development of new parenting and communication skills.
Essential Duties and Responsibilities:
The Family Intervention Specialist:
Carries a caseload of 4 to 6 families
Holds individual and family sessions with each family 3 times a week, scheduled at the convenience of the families
Conducts on-going assessment of youth to determine their needs from a strength-focused, solution-based perspective
Attends 3 weekly meetings (individual, team, and clinical consultation) for professional development to help enhance clinical skills
Drives up to 60 to 80 miles to meet with families in their homes and communities (additional travel may be required)
Collaborates with other providers, case workers, and courts to formulate a collaborative treatment plan
Provides on-call crisis support to the youth and family (schedules vary by location)
Completes accurate and timely documentation in an electronic medical record system (EMR)
Performs other duties as assigned
Additional Information:
Schedule is flexible and non-traditional as it is based around the availability of youth and families served.
Applicants must possess a current, valid driver's license, an automobile for work purposes, and proof of auto insurance.
Community-based staff will be reimbursed for applicable mileage.
Salary:
$50,000 - $57,000 per year based on education and clinical license
Qualifications:
Master's degree in a social services discipline (preferred)
Bachelor's degree in a social services discipline (required)

The Family Centered Treatment Specialist provides quality psychotherapeutic services to children, adolescents, and families utilizing the Family Centered Treatment model.
Essential Duties & Responsibilities:
Provide Family Centered Treatment (FCT) Services as per the service definition and model fidelity requirements.
FCT Certification is required and must be completed within one year of employment in order to continue to provide FCT services and maintain employment with Health Connect America (HCA).
Complete FCT Checkoffs and other internal trainings within designated deadlines. Trainings are assigned and monitored through an e-learning system.
Deliver FCT services to children, adolescents and their families, primarily in their living environments, with a family focus.
Provide 24/7 on-call coverage to a caseload of families and children. Service is regularly provided in the home and community, and will require service provision at the convenience of the child and family requiring a flexible work schedule.
Complete all required Clinical Records and FCT documents with the child and family and ensure the documents are entered/uploaded timely in the Electronic Health Record, according to company standards.
Develop Service Plan in coordination with Treatment Team Process and maintain the plan per designated authorization processes in the clinician's area of service. Work collaboratively on behalf of the child and family with the identified Treatment Team members to ensure effective communication and coordinated service provision.
Conduct ongoing revisions of Service Plan and is responsible for implementation of Service Plan.
Provide psychoeducation as indicated in the Service Plan.
Conduct interventions with the family aligned with the FCT model to include facilitating the movement of the child and family through the four phases of treatment and completing all fidelity documents.
Assist the families and children to establish weekly goals and help them to identify the steps that need to be taken to reach the established goals.
Make referrals to services and resources on behalf of the families and children.
Develop and coordinate discharge plan based on treatment, indicating all relevant aftercare needs and plans. Access community resources that would be beneficial to client aftercare needs.
Coordinate and oversee the initial and ongoing assessment activities.
Convene the Treatment Team for Service Plan.
Consult with identified medical (e.g., primary care and psychiatric) and non-medical (e.g., Dept. of Child Services, school, Dept. of Juvenile Justice) providers; engage community and natural supports, and include their input in the person-centered planning process.
Ensure linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations.
Provide and coordinate behavioral health services and other interventions for the youth or other family members with other behavioral health professionals and Treatment Team members.
Monitor and document the status of the consumer's progress.
Participate in Quality Assurance/Quality Improvement activities to include audits, corrections and designated improvement plans and projects.
Qualifications
Education & Experience:
Minimum of a Bachelor's Degree in human services or related field is required.
Two years' experience working with youth or families.
Current Family Centered Treatment Certification is preferred.

Part-time | Salary to be discussed with applicant Department: Health Department A WIC Breastfeeding Peer Counselor is a support person with personal breastfeeding experience who gives basic breastfeeding information, encouragement and support to WIC participants who are pregnant or breastfeeding.
The Women, Infants and Children (WIC) program is a supplemental nutrition program funded by the U.S. Dept. of Agriculture. Nutrition education, supplemental foods, breastfeeding promotion, education and support and referrals to community agencies are provided to low income pregnant, postpartum and breastfeeding women, infants and children up to the age of 5. The goal of the WIC program is to improve the health of families and individuals during critical times of growth and development.
The WIC Breastfeeding Peer Counselor is responsible for providing evidence based breastfeeding education and support for WIC participants through one-on-one clinic visits, phone calls, group classes or support groups. The WIC Breastfeeding Peer Counselor will participate in WIC community outreach and breastfeeding promotion activities and events.
The WIC Peer Counselor is expected to be an active participant in supporting the mission, vision and values of the WIC program and the Tennessee Department of Health. They will provide consistent, quality services to the local WIC participants within their scope of practice. They will demonstrate enthusiasm, initiative and flexibility in assignments and maintain positive working relationships and communication with their co-workers.
Minimum Qualifications:
* Has breastfed at least one baby
* Is enthusiastic about breastfeeding and wants to help other mothers enjoy a positive experience
* Has a high school diploma or GED
* Has reliable transportation, including traveling to other clinics for trainings and meetings (travel to other clinics is reimbursed)
* Can work 22.5 hrs. per week based in the Hendersonville Health Department beginning June 2026. The Sumner County Health Department WIC program operates 8:00 a.m. to 4:30 p.m. Monday through Friday.
* Willing to receive and make phone calls and check text messages at home, after clinic hours using their WIC provided cell phone
* Current or previous WIC participation preferred but not mandatory
Ideal candidates:
* Have experience with computers, including Excel and Outlook
* Bilingual (English & Spanish) preferred but not mandatory
* Has good verbal and written communication skills
* Has a willingness to learn and expand one's knowledge base
* Has good customer service skills and is able to be respectful in relating to persons of diverse ethnic and cultural backgrounds
* Self-starter, able to work independently
* Able to organize their work and keep accurate records of all contacts and documents contacts in a timely manner
* Demonstrates regular and punctual attendance
* Able to respect each client by keeping their information strictly confidential per HIPPA guidelines
* Has the ability to work well with others, as a part of the WIC and the TN Dept. of Health team
* Responds to public health emergencies or acts of terrorism in accordance with local, state and national response plans.
Training:
* Training and continuing education of Breastfeeding Peer Counselors is provided using approved WIC federal, state and county approved curriculum. Training includes assigned reading, online/video trainings, webinars, shadowing of other Peer Counselors, and attending in person and online staff meetings.

You can work as much or as little as you like as an On-Demand Team Member (TM) and your schedule may vary depending on shift availability and store needs. This role is ideal for those that are looking for a personalized work schedule.
As an On-Demand TM, you will not be included on the posted weekly schedule, but rather will have the opportunity to create your own schedule by picking up shifts posted by leaders or other team members (via our my Time mobile scheduling app) that work best with your schedule.
When you choose to pick up a shift, reliable and prompt attendance is necessary.
We also ask for your active engagement by picking up and working shifts once every 4 weeks (minimum of four hours).
