Become the Case Manager you always wanted to be Work with interdisciplinary team, guiding treatment plans based on patient needs and preferences. Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans Participate in planning for and the execution of patient discharge experience. Monitor patient experience: quality/timeliness/service appropriateness/payors/expectations. Facilitate team conferences weekly and coordinate all treatment plan modifications. Complete case management addendums and all required documentation. Maintain knowledge of regulations/standards, company policies/procedures, and department operations. Review/analyze case management reports, including Key Care Indicators, and plan appropriate actions. Understand commercial contract levels, exclusions, payor requirements, and recertification needs. Attend Acute Care Transfer (ACT) meetings to identify trends and collaboratively reduce ACTs. Meet with patient/family per Patient Arrival and Initial Visit Standard within 24 hrs. of admission. Perform assessment of goals and complete case management addendum within 48 hours of admission. Educate patient/family on rehabilitation and Case Manager role; establish communication plan. Schedule and facilitate family conferences as needed. Assist patient with timely procuring/planning of resources to avoid discharge delays or issues. Monitor compliance with regulations for orthotics and prosthetics ordering and payment. Make appropriate/timely referrals, including documentation to post discharge providers/physicians. Ensure accuracy of discharge and payor-related information in the patient record. Participate in utilization review process: data collection, trend review, and resolution actions. Participate in case management on-call schedule as needed. Qualifications - License or Certification: Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling). If licensure is required for one's discipline within the state, individual must hold an active license. Must meet eligibility requirements for CCM or ACM™ certification upon entry into this position OR within two years of entry into the position. CCM or ACM™ certification required OR must be obtained within two years of being placed in the Case Manager II position. - Minimum Qualifications: For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirement for licensure or certification is a diploma rather than an associate degree. For all other eligible licensed or certified health care professionals, must possess a minimum of a bachelor's degree and graduate degree is preferred. 2 years of rehabilitation experience preferred. Key Responsibilities WebEOC Administration Administer and maintain Juvare WebEOC V10 for daily operations and system stability. Support planning, testing, and implementation efforts tied to future migration to Juvare WebEOC NexUs. Manage users, positions, roles, permissions, and overall access provisioning. Configure and maintain incidents, system processes, notifications, plug-ins, and general system settings. Develop, update, and troubleshoot boards, including code adjustments and design enhancements. Move boards and system configurations between environments through imports/exports. Maintain clear documentation of configuration changes, system updates, and operating procedures. Integration & Technical Support Configure and support GIS integrations for both cloud and on-premises WebEOC environments. Utilize the ArcGIS API to integrate mapping services, geospatial layers, and situational awareness tools. Responsible for the implementation and maintenance of an effective alcohol and other drug abuse program with emphasis placed on awareness, prevention, staff training and networking with community resources in compliance with American Correctional Association (ACA) standards, Management & Training Corporation (MTC), Florida Department of Corrections (FDOC) and Florida Department of Management Services (DMS) directives. Essential Functions: * Develop and implement a system to promote the coordination and integration of the substance abuse prevention components with other facility programs. * Ensure the documentation of all inmates' assessments and plans are in inmates' medical records regarding substance abuse problems. * Provide ongoing comprehensive training for all staff members in the substance abuse area. * Assist in providing substance abuse related materials and information in the health education program and development of health services standing orders and facility operating procedures regarding substance abuse. * Conduct substance abuse and cognitive behavioral classes, group educational, peer support and other prevention sessions for inmates/staff. * Encourage awareness of substance abuse issues throughout the facility. * Develop linkages with community resources for substance abuse services. * Promote the development of positive social skills through modeling appropriate behaviors and intervening when inappropriate behaviors are observed. * Maintain accountability of inmates and property; adhere to safety practices. It is expected that the incumbent shall perform other duties as assigned within his/her capabilities as determined by management. Education and Experience Requirements: Bachelor's degree in social work, counseling, psychology, criminology or related field and where applicable, must possess appropriate certification. Two (2) years of substance abuse treatment experience preferred. Directly related experience may be considered in lieu of formal education requirements if approved by management. Valid driver's license in the state of Florida with an acceptable driving record required, unless waived by management.
