Summary of Responsibilities
A Behavioral Health Practitioner (BHP) works under the direction and supervision of a Clinical Manager, providing community-based behavioral health services to individuals with psychiatric illness, substance use issues and/or co-occurring disorders. BHPs provide individual, family and group counseling, crisis intervention, conflict resolution, initial assessments to determine level of care, treatment planning, case presentations, presenting psychoeducational information to individuals regarding diagnosis, symptoms, medications, relapse prevention and safety planning. The BHP provides services to both children and adults. The position also includes participating in multidisciplinary case conferences and supervision, treatment plan reviews, developing aftercare treatment and all required documentation utilizing Collaborative Documentation.
Essential Duties and Responsibilities
•Primary focus is on providing comprehensive clinical services and therapy which includes biopsychosocial assessments, screening and placement for level of care, diagnosis, individual psychotherapy, group therapy, family therapy, brief interventions utilizing evidence-based care and tools and educational counseling services to a caseload of person's served consisting of adults and children.

Responsibilities
Social Services Assistant Essential Functions
Assisting the Social Services Director in the operation of the Social Services Department. This includes:
Assisting with the interviewing of resident /families to obtain social history;
Assisting in arranging transportation to other facilities when necessary;
Assisting in the monitoring of resident mood/behaviors;
Assisting in developing and implementing behavior management programs;
Assisting in the maintenance and acquisition of eyewear, hearing aids and dentures as well as obtaining personal items and specialty needs for the residents;
Assisting in the oversight of psychotherapeutic medications;
Assisting in room change-plans, documentation and follow-up;
Assisting and maintaining transfer documentation;
Assisting in the collection and execution of CQI and MDS data;
Assisting in periodic progress notes;
Assisting in the scheduling and documentation of care plan meetings;
Assisting with the review of nurses notes to determine if the care plan is being followed;
Maintaining the confidentiality of all residents and their care to ensure resident rights, privacy and property are protected at all times; &
Reporting and documenting resident and/or visitor incidents, accidents, complaints or allegations according to the facility's policy and procedure using the designated occurrence reporting system.
Complying with safety requirements, infection control measures, and maintaining a clean work environment in accordance with facility policy as indicated by:
Ensuring that work/assignment areas are clean and records, files, etc., are properly stored before leaving such areas on breaks, end of work day, etc.;
Abiding by all emergency protocols for crash carts, resident codes, fire safety, missing residents, tornado preparation, violent residents, Wander guard alarms, door alarms, and other facility policies as assigned;
Reporting any forms of abuse witnessed and aware of and taking appropriate action according to policy;
Following infection control and universal precautions procedures;
Following periodic cleaning schedules for equipment and returning equipment to proper area after use; &
Ensuring the facility is maintained in a clean and sanitary manner at all times.
Performing other duties as assigned by the Social Services Director.

What you will do:
This is work responsible for providing residential care, assisting with enrichment activities and supporting the treatment, psychiatric rehabilitation, stabilization and recovery of people with a mental illness. As a Human Services Worker II you will treat people in a respectful manner that preserves their dignity and rights and maintain a professional caregiver relationship with people served.
How you will make an impact:
The incumbent is responsible for preserving the rights, safety and security of people served by observing them:
* At all times to learn what type of environment and interactions make them feel safe, secure and calm and to identify distressful stimuli or triggers:
When providing ward coverage.
When escorting to and from activities and appointments to ensure that they arrive safely.
During meals to monitor intake and ensure that they don't choke or trade food.
When a person served is placed on special observation status to address dangerous or high risk behaviors.
When a restrictive intervention such as seclusion or restraints is used to assess for release criteria and immediately identify health needs that may arise.
Communicating with them:
* At all times to develop a trusting, caregiver relationship and identify signs of distress or health concerns.
When a person served is placed on special observation status, in the seclusion room or in restraints to assure the person that he or she is safe, help them develop trust in you as a caregiver and identify signs of distress or health concerns.
Intervening:
* To separate them from distressful stimuli and assist in resolving conflict.
* To ensure that people served are not neglected, abused or exploited; or to ensure that it is reported to the abuse hotline if they are.
* To ensure safety during medical or disaster emergencies.
* To ensure safety when a person served is exhibiting a behavior dangerous to self or others, which may involve the use of physical holds and placing people served in a seclusion room or mechanical restraints.
Qualifications:
* Must be 18 years of age at time of hire.
* Six months of experience working in a medical, psychiatric, nursing, or childcare setting, customer service experience or in working with developmentally disabled persons; or in working in a correctional facility OR college or vocational/technical training from an accredited institution at the rate of 15 semester, 22 quarter or 360 classroom hours; OR do you possess a current State of Florida Certified Nursing Assistant (C.N.A.) license.
* Ability to attend a 1 - 2-week mandatory training course that will include completion of a CPR certification. This training could be scheduled during day, weekend, and evening hours.
Special Notes:
Pay: $33,579.00 annually/$1,291.50 bi-weekly. 5% Additive dependent on unit assigned
Physical agility requirements for Direct Care Staff:
* Must be able to ambulate up and down a stairwell.
* Must be able to assist with evacuation of persons served during an emergency situation.

* This position will require night, weekend, and holiday work; including working on scheduled off duty days and various shifts. This position has also been designated as an essential position. 8) Assists Social Services Director with room changes; including notification and documentation of notification of residents, families and staff members. Prepares and supports resident throughout these changes.
9) Assists the Social Services Director with completing the social history, psychosocial assessment, and other required assessments (BIMS, PHQ9) within the required time frames. Participates in the MDS/Care Planning process (as needed) and documents accordingly. Documents patient progress and significant concerns/events in medical record.
10) Upon admission and throughout stay, coordinates information which will enable resident to be discharged at medically appropriate time.
11) Assists residents and family members in obtaining services needed to assure continuity of care after discharge. Issues transfer/discharge letters and Non-Coverage letters as required by Federal Regulations. Maintains a current community resource guide.
12) Educates and assists resident and family members with financial planning.
13) Be familiar with admissions process to be able to assist with tours, telephone calls, and admissions as needed.
14) Participates in the Utilization Review Process and issue denial/non-coverage letters accordingly, as needed.
15) Be aware of and follow all state, federal, company and center policies and procedures.
16) Attends facility meetings as required.
17) Keeps Social Services Director and Administrator informed and up to date on current status of social services and significant resident/family issues.

What will you do:
This is work responsible for providing residential care, assisting with enrichment activities and supporting the treatment, psychiatric rehabilitation, stabilization and recovery of people with a mental illness. As a Human Services Worker you will treat people in a respectful manner that preserves their dignity and rights and maintain a professional caregiver relationship with people served.
How you will make an impact:
The incumbent is responsible for preserving the rights, safety and security of people served by observing them:
* At all times to learn what type of environment and interactions make them feel safe, secure and calm and to identify distressful stimuli or triggers:
When providing ward coverage.
When escorting to and from activities and appointments to ensure that they arrive safely.
During meals to monitor intake and ensure that they don't choke or trade food.
When a person served is placed on special observation status to address dangerous or high risk behaviors.
When a restrictive intervention such as seclusion or restraints is used to assess for release criteria and immediately identify health needs that may arise.
Communicating with them:
* At all times to develop a trusting, caregiver relationship and identify signs of distress or health concerns.
When a person served is placed on special observation status, in the seclusion room or in restraints to assure the person that he or she is safe, help them develop trust in you as a caregiver and identify signs of distress or health concerns.
Intervening:
* To separate them from distressful stimuli and assist in resolving conflict.
* To ensure that people served are not neglected, abused or exploited; or to ensure that it is reported to the abuse hotline if they are.
* To ensure safety during medical or disaster emergencies.
* To ensure safety when a person served is exhibiting a behavior dangerous to self or others, which may involve the use of physical holds and placing people served in a seclusion room or mechanical restraints.
Qualifications:
* Must be 18 years of age at time of hire
* Ability to attend a 1 - 2-week mandatory training course that will include completion of a CPR certification. This training could be scheduled during day, weekend, and evening hours.
Physical agility requirements for Direct Care Staff:
* Must be able to ambulate up and down a stairwell.
* Must be able to assist with evacuation of persons served during an emergency situation.
* Must have normal vision and hearing, including use of corrective devices.
* Must be able to work with or around cleaning chemicals and supplies.
* Must be able to do repetitive movements with arms, hands, neck and head.
* Must be able to demonstrate competency, as appropriate, in all areas of training, some of which requires physical activity such as;
CPR and MANDT
Must be able to bend, twist, turn, kneel, stoop, squat, push, pull, lift and walk extended distances.

* This position will require night, weekend, and holiday work; including working on scheduled off duty days and various shifts. This position has also been designated as an essential position. Incumbents are expected to work during emergency situations or natural disasters and may be required to work before, during and beyond normal work hours/days.
* The incumbent in this position will work with residents who have been charged with felony offenses and may have psychiatric disorders.
* The incumbent in this position will assist residents in completing oral care, bathing, and showering as well as driving them to facilities and medical appointments etc.
