What You Will Do
Applies mutually shared and lived experiences navigating the systems of care as a caregiver. Serves as a role model, advocate, and mentor for children and families
Assists with obtaining and maintaining clients' entitlements, including Medicaid, Public Assistance, SSI as well as assisting with problems related to housing or utilities
Interviews clients and significant others, when appropriate, in order to obtain personal histories and to understand the social and emotional factors related to the client's mental health problems
Participates in frequent active engagement with families to engage them in receiving services and assists in empowering them to take the steps necessary to maximize their quality of life
Develops inventory of resources that will meet the clients' needs as identified in the assessment process
Advocates for and assists clients in utilizing adjunctive community resources. Arranges transportation and accompanies clients to appropriate facilities/agencies, as necessary
Provides linkage, coordination with, referral to and follow-up with other service providers. Monitors appointments medical facilities, social service agencies or other providers of concrete services
Collaborates with interdisciplinary team members to discuss care needs and identify solutions to support clients/consumers/members
Attends planning meetings with service providers to coordinate service plans
Initiates written (including clinical and progress notes) and verbal communications, and ensures that all patient record keeping is in compliance with agency, state, city and federal requirements
Participates in agency and community programs and education, as requested
Participates in interdisciplinary team meetings, case conferences, staff meetings, quality assurance meetings, and supervision meetings as required
Qualifications
Licenses and Certifications:
Valid driver's license (i.e., license matching state of residence) required

Education:

Two or more years of equivalent and relevant experience in a social service agency OR an equivalent combination of the education and experience preferred
Work Experience:

The Family Advocate in the LYNC Program (Linking Youth to Natural Communities) will be the primary point of contact for bilingual youth and their families throughout the duration of the program. The Family Advocate will provide client services in Spanish and /or English and will assist clients with language barriers and translations in a non-clinical capacity to meet their needs. Engaging and navigating youth and their family is one of the primary functions of the Bilingual Family Advocate. As part of the LYNC team the Family Advocate will deliver culturally and gender responsive, trauma-informed, strength-based services to assigned youth and their families.
Essential Duties and Responsibilities:
Liaison to families, parents, guardians and community providers or referral source; Serves as point of contact for youth and families/guardians, service delivery in program, and connections to the community (in collaboration with Family Engagement Specialist); primary liaison to referral source and community linkages.
Interface with clients and conduct Family Advocate duties in Spanish and/or English and assist clients with language barriers and oral/written translation in a non-clinical capacity to meet their needs.

Works collaboratively with juvenile probation and provide enhanced family engagement services based on the JBCSSD Family Engagement and Partnership pilot:

POSITION SPECIFIC DUTIES AND RESPONSIBILITIES: ERSEA
Develop, maintain, and manage system to track the eligibility, recruitment, selection, enrollment, and attendance of children to comply with federal and state program regulations.
Annually create the criteria for the selection and enrollment of children assuring responsiveness to community assessment and reflecting data on dual language learners, homeless children, children in foster care, and children with disabilities.
Verify income and eligibility qualifications of children and families, and ensure applications are complete and data is accurately entered into the Child Plus database.
Create and maintain record keeping and reporting policies for waitlists, enrollment, attendance, timelines, schedules, and procedures in accordance with designated state and federal program requirements.

Serve as the program resource for consultation, information, and referral regarding community resources to staff and client families.
Serve as the program resource for other community organizations and resources and maintain collaborative agreements and effective community partnerships to support enrolled families in needed comprehensive services without duplication of services.
In collaboration with the EHS Director:
initiate agreements (MOU) with other public agencies, in particular public schools, for the sharing of child and program data while protecting the privacy of personally identifiable information.
Attend all Professional Development training required by the program.

Under the supervision of the school principal, provides counseling services for students manifesting social, emotional, behavioral and/or learning difficulties which interfere with the students' capacity to be successful in school. He/she works with the parents, teachers, other school personnel, outside agencies and the community to modify those aspects of the environment which interfere with positive school adjustments and healthy development.
Responsibilities
Identifies and assesses learning and behavior problems, including factors in the school environment contributing to student problems.
Identifies, assesses and when warranted and possible, modifies social-emotional, cultural, economics as well as other environmental factors impacting negatively on a child's success in school.
Incorporate students' parents as partners in the learning process. Keep parents apprised of student's progress, attitude and behavior. Keep scheduled appointments and be accessible to parents when they have questions
Chair academic conferences with parents i.e. promotion in doubt
Counsels students individually and in groups.
Provides consultation with school staff and parents concerning the growth and development of individual students.
Collaborates with school staff including other mental health providers assigned to respective schools.
Collaborates with special education teacher and serves on as a liaison with CSE.
Provides mandated workshops on child abuse and child neglect to all school staff.
Serves as an advocate for children and for parents around issues that are negatively impacting a child's success in school.
Liaisons with various agencies (DSS, DJJ, etc.) and institutions (churches, universities, etc.) in order to assist in effecting positive change for students and families that will translate into improved school adjustment and achievement for the child.
Monitors school climates and finds ways of positively impacting that environment for children.
Serves as a member of Crisis Intervention Teams.
Provides group counseling, educational workshops, and other services to parents on topics such as parenting, stress reduction
Pay Transparency: The salary range for this role is $ 62,500 - $98,000. Base pay offered may vary within the posted range based on several factors, including but not limited to education, job-related knowledge, skills, experience, and location.

NYS Certification in School Counseling required.
Masters Degree in School Counseling required

Patient visits will be made in both community and facility settings. As the Hospice Field Case Manager, you will develop and implement nursing plans of care and make recommendations for care based on patients' needs.
The schedule includes working one weekend per month and functioning in a backup On Call capacity 2-3 times per month. Car and escort service available for backup On Call shifts.
