Ensure compliance with hospital financial resolution policies, including conducting financial interviews with patients and families to assist with resolution of their account. Ensure productivity of various Workqueues to identify, verify, and assuring all accounts are ready for billing with adequate supporting documentation, within the established time frames.
Serves as a support to patients and their family members to assure patients have access to all available funding options, this involves working with private as well as governmental agencies, i.e., private insurance, HMO/PPO's, Medicaid, Medicare, and Third-Party Liability payers.

Job Summary:
The Delinquency Prevention Case Manager will provide neighborhood-based wraparound intervention services to at-hope youth and families to stop escalating involvement in the juvenile justice system. This prevention initiative will operate at 10 targeted Duval County public middle schools. Target population will be juveniles who have been arrested but not sent to a commitment program, youth who have received civil citations and “crossover” youth also in the dependency system. Referrals will come from JFCS Criminal Justice Reinvestment Grant staff at the Juvenile Assessment Center, the State Attorney's Office (civil citations), Child Welfare Family Services Counselors, Family Support Services of North Florida and/or the Department of Juvenile Justice for crossover youth.
The Case Manager will recruit and work with pre-identified at-risk children in 10 identified Middle Schools who meet the criteria listed above. The work will be done in collaboration with agency and community partners. The DPCM will develop a plan for success with the family and help to keep the family focused on the goals they set for themselves..
The goal of the family intervention and wrap around services will be to reduce delinquency recidivism, strengthen families, reduce truancy and improve academic performance and success at school.
Education:
Bachelors degree in Counseling, Social Work, or related field from an accredited college or university preferred.
Experience:
Minimum one year experience in case management or social work practice with children and families preferred
Primary Job Functions:
Recruit and serve a minimum of 200 middle school students who meet the identified criteria during the calendar year. Services will be provided for an average of three to six months.
Case Management services for youth, parents/caregivers enrolled in the program, including scheduled visits and phone calls with all parties monthly.
Assist parents with success plan tasks (including setting up appointments and transportation) and will staff cases with program partners and service providers.

What you will do:

The incumbent is responsible for preserving the rights, safety and security of people served by observing them:
* At all times to learn what type of environment and interactions make them feel safe, secure and calm and to identify distressful stimuli or triggers:

During meals to monitor intake and ensure that they don't choke or trade food.

* To ensure safety when a person served is exhibiting a behavior dangerous to self or others, which may involve the use of physical holds and placing people served in a seclusion room or mechanical restraints.
Qualifications:
* Must be 18 years of age at time of hire.
* Six months of experience working in a medical, psychiatric, nursing, or childcare setting, customer service experience or in working with developmentally disabled persons; or in working in a correctional facility OR college or vocational/technical training from an accredited institution at the rate of 15 semester, 22 quarter or 360 classroom hours; OR do you possess a current State of Florida Certified Nursing Assistant (C.N.A.) license.
* Ability to attend a 1 - 2-week mandatory training course that will include completion of a CPR certification. This training could be scheduled during day, weekend, and evening hours.
