Position Summary The Caseworker is a full-time position at WAW's Virignia Community Center (VACC) and reports directly to the VACC Case Manager. The Caseworker is responsible for working on various client's cases in a timely manner, supporting the Staff Attorney alongside the case management team. The Caseworker will provide services as needed, always using a strength-based approach. The caseworker will work with external organizations and agencies to maximize support for WAW's clients. Responsibilities: ● Assist in implementing service plans determined by goals of clients, including reviewing service plans, identifying progress, and revising the plan as needed. ● Conduct safety assessment and safety planning with domestic violence survivors and assist them in obtaining safe shelter while their case is being processed. ● Accompany or arrange for clients who need support to other agencies, facilities, and appointments. ● Seek opportunities to partner and collaborate with organizations in the VA area to maximize the resources of the VACC. ● Manage in developing cases for clients required to appear at fair hearings. ● Refer clients to other organizations for additional services, (e.g. family legal services shelters). ● Advise clients about the services they are entitled to. ● Translate on behalf of clients. ● Assist the legal team filling USCIS forms for clients. ● Ensure all client information is entered into WAW's database in a timely manner. ● Ensure the hard copy of clients' case files are updated and securely stored. ● Draft profiles of WAW clients. Organizational Responsibilities ● Serve as a spokesperson on behalf of WAW at various programs, meetings, and functions when approved by the US country director and/or executive director. ● Translate for participants at monthly Women Circle sessions and other programs as needed, and must be organized and have great attention to detail. ● Help with the community outreach and other events when needed, and take part on other administrative tasks, special projects and assignments, as needed. ● Stay informed and updated about WAW's programs in Afghanistan. Our Director Team is also comprised entirely of parents, so we understand the need for balance. The BCBA works with the Director Team and a team of BCBAs and RBTs to provide a range of ABA assessment and clinical services for children with Autism Spectrum Disorders. The BCBA completes initial and ongoing assessments, develops treatment plans, provides training to caregivers, and supervises RBTs on site and via remote supervision. The BCBA is responsible for coordinating, scheduling, and communicating with RBTS as well as caregivers. This role will focus on coordinating the discharge process for patients transitioning from health and rehabilitation settings to home or other care environments. The ideal candidate will assist in planning, organizing, implementing, evaluating, and directing the overall operation of the facility s Social Services Department, ensuring compliance with current federal, state, and local standards, guidelines, and regulations, as well as the facility s established policies and procedures. Key responsibilities include assessing patients' social, emotional, and logistical needs, collaborating with medical staff, family members, and community resources, and developing individualized discharge plans to ensure a smooth transition. The role requires excellent communication skills, a strong knowledge of healthcare systems, and the ability to provide ongoing support and guidance to patients and their families. Any facility with more than 120 beds must employ a qualified social worker on a full-time basis. A qualified social worker has: A minimum of a bachelor s degree in social work or another human services field to include, but not limited to, sociology, gerontology, special education, rehabilitation counseling, or psychology. One year of supervised social work experience in a health care setting working directly with individuals. Major Duties and Responsibilities The Social Services and Discharge Planning Assistant will assist in overseeing the development, implementation, supervision, and ongoing evaluation of the Social Services Department designed to meet and assist residents in attaining or maintaining their highest practicable well-being. This includes identifying the need for medically related social services and ensuring that these services are provided in accordance with State and Federal regulations. The Social Services and Discharge Planning Assistant will facilitate residents safe transition back into the community based on the resident/representative s wishes, safety, and clinical recommendations. The Social Services and Discharge Planning Assistant will assist with transitions back to the resident s prior level of living (i.e. home, assisted living) or help identify and arrange a new discharge location based on the residents assets and needs. The Social Services and Discharge Planning Assistant may assist in lateral transition to another SNF or transition to a long-term care nursing home when appropriate. The Social Services and Discharge Planning Assistant will collaborate with the interdisciplinary team to identify clinical discharge planning needs and arrangement of community-based services and follow-up care within the community. Additional Tasks Adheres to all facility policies and procedures. Treats all residents and staff with dignity and respect. Promote and protect all residents rights. Reports to Director of Nursing or Administrator any allegations of potential abuse or neglect, or misappropriation of resident property and participates in the investigative process as needed. Complies with standards of business conduct, and state/federal regulations and guidelines. Always follows appropriate safety and hygiene measures to protect residents and themselves. Reports noncompliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel. Report any retaliation or discrimination to HR or the compliance officer. Reports any occupational exposures to blood, body fluids, infectious materials, and/or hazardous chemicals/equipment in accordance with facility policy. Always follows appropriate safety and hygiene measures. Reports work-related injuries and illnesses immediately to supervisor. Follows established infection control policies and procedures. Maintains confidentiality of protected health information, including verbal, written and electronic communications. As a condition of employment, completes all assigned training. Participates in all life safety and emergency drills and trainings. Fulfill responsibilities as assigned during implementation or activation of the facility s emergency plan.
