*What You Will Do:*

* Develops and implements an individualized Plan of Care that incorporates realistic, and attainable goals that is also outcome-focused so that the services provided will improve the patient's quality of life and promote dignity.
* Makes appropriate referrals for evaluation/care to other disciplines and services, and coordinates care with others to ensure effective and efficient care is provided.
* Utilizes interview, observation, and evaluation in assessing clients and applies nursing judgment, practice standards, in formulating nursing interventions and making recommendations to the physician, client/family and IDT/IDG.
* Reports changes in client condition as appropriate and in a timely manner, to the client's physician and/or Case Manager/designee and obtains orders for changes in the plan of treatment to respond to the client's condition.
* Re-evaluates and updates patient's plan of care based on patient goals and progress towards outcomes.
* Assess patient and family learning styles and needs for teaching regarding disease process, self-care, end of life care, and dealing with ethical concerns as well as patient goals as part of plan of care.

* Accurately documents observations, interventions and evaluations pertaining to client care management and services in accordance with agency policy and procedure utilizing the point of care information system. This will include assessments, visit reports, medication information, treatments, coordination of services, client progress or lack of progress toward goals, and contacts with physicians and agency and community providers.
* Takes appropriate steps to assure timely submission of client data to the office for inclusion in the client record (by timely electronic transmission of data, or when appropriate by fax or mail).
* Documentation reflects progress towards patient/family goals.
* Documents all collaborative contact with physicians and other members of IDT/IDG team.
* Assures that documentation and orders are up to date and completed in a timely manner.
*Minimum Qualifications:*
* Graduate of an approved Nursing education program
* Current Registered Nurse licensure in the state of practice.
* Must have a minimum of one (1) year experience as a professional acute care nurse.
* Home healthcare/hospice experience preferred.
* Exhibits strong communication and interpersonal skills, with a special emphasis on grief/loss management.
* Must have current Driver's license and reliable transportation to and from work site.

Job Summary:
Supervises residents and ensures them a safe and secure environment. Using developmental screenings and assessments work collaboratively with parents and develop a comprehensive Individual Educational Service Plan (IESP) for each child. The IESP should be designed to promote age appropriate development for each child and reviewed at least quarterly.
Develop and utilize teaching methods and materials for use in the home.
Assist in the planning and facilitating of group meetings, which provide education and information on current Early Head Start policy and curriculum development.
Maintain accurate written records including assessments, IESP documentation, developmental screening instruments, home visit reports, and other required forms.
Plan monthly socialization activities for all families enrolled in the program.

Coordinate recruitment, registration, screenings, enrollment and selection activities.
Conduct and track children's screening status and follow up accordingly.
Assist with attendance reporting.
Intervene and contact parents when there are problems with attendance.
Conduct home visits to provide support and/or meet families identified needs.
Work in a collaborative effort with agency/community resources to provide wrap around and follow up services to families in order to strengthen and enable them to function independently, to improve their capacity to function as a healthy families, or if needed, to find appropriate alternatives for the care of the children.
Keep abreast of local, state and national requirements and changes that affect component area.
Facilitate parent meetings and/or support groups.
Conduct parent, children, and staff training sessions.
Responsible for keeping client files in order and up to date.
Identify, follow up and monitor weekly health, nutrition, mental health, social service, special education and dental needs for each child.
Cooperate in any investigation related to personnel, licensing, accreditation, or other circumstances.
Assist Fiscal Manager with volunteer and in-kind donations, record keeping and contacts families to encourage volunteer participation.
Assist Site Leader in special tasks as it relates to program operations.

Provide one to one assistance for families requesting or in need of applying for or negotiating educational services.

Job Qualifications:
Prior experience working with adoptive parents and the adoption process preferred.
Experience providing in-home services and/or working directly with children and their families in various settings, including child welfare, schools, juvenile justice, and mental health preferred.
Understanding of basic needs, social, cultural and religious differences in families' required.
Provide services in Genesee, Huron, Lapeer, Sanilac, St. Clair, Tuscola county
Ability to provide culturally competent services that shows sensitivity to the service population's cultural and socioeconomic characteristics
