What you'll do in this role:
Manage Journey patient census and referrals for the assigned area from Journey candidate list, Journey Administrative Assistant, Customer Service, Clinicians, and other sources
Review all assigned patient charts to determine eligibility and appropriateness for the Hospice benefit and prepare for conversations with the patient, family, and/or caregivers
Make initial phone calls to all new Journey referrals to schedule home visits as well as follow-up calls to maintain contact with the patient and assess patient decline
Conduct patient visits for the purpose of building relationship with the patient and/or family and in-depth discussion of the hospice benefit when appropriate

Responsibilities
The Medical Social Worker (MSW) is a qualified professional who, in accordance with the plan of treatment, assists the physician and other team members in understanding the significant social and emotional factors related to the health problems.
Essential Duties and Responsibilities

Qualifications

RESPONSIBILITIES/ESSENTIAL FUNCTIONS:
The Foster Care Case Manager provides services to children placed in Foster Care and their families to ensure protection and stabilization for the children and to assist the families in developing and completing a successful plan for reunification. The Case Manager reports to the Foster Care Supervisor.
Assist in intake, placement and replacement of children who are referred to the Agency by the Michigan Department of Health and Human Services (MDHHS).
Develop, implement, document and review behavioral goals, methodology of service delivery and objectives as listed in a Parent Agency Agreement.

Document that all efforts have been made to work toward a speedy permanency plan for the child.
Prepare all necessary reports as required by MDHHS and the legal system according to prescribed guidelines.
Active and ongoing exploration of appropriate alternative placements with family members or kinship care to ensure the least restrictive environment and best interest placement for the child.
Facilitate aftercare services when a child has returned to family care or becomes a Permanent Ward to ensure that the placement is stable and timely.
Assist in support groups for foster parents, birth parents and kinship or sibling groups.
Represent the agency in a positive manner.
Participate in outreach and recruitment efforts.
Responsible for incorporating principles of diversity, equity and inclusion in work processes and in job functions. Responsible for promoting a feeling of welcoming, belonging, and acceptance to all clients and staff.

Job Summary:
Under general supervision, provides child life services for ambulatory and inpatient children and their families. Administers programs and provides services that reduce distress, increase adaptive coping and protect and enhance developmental integrity by offering opportunities to play, learn and interact with others in an emotionally and physically safe environment.
1. Partners with a trained Facility Dog to provide comprehensive child life services and therapeutic interventions in a variety of clinical settings.
2. Maintains all requirements for the Facility Dogs as set by Canine Assistants regarding dog's certification, hours worked, weight management, veterinary care, home environment, grooming, training and reporting.
3. Continually assesses and prioritizes health and safety of the Facility Dog, ensuring it receives adequate rest, play and toileting breaks throughout the work shift.
4. Plans and implements a therapeutic play program to help hospitalized and ambulatory patients to adjust to the health care experience. Utilizes therapeutic play techniques, assessments and observations as diagnostic and treatment tools. Documents treatment plans, goals and observations.*
5. Assists in preparing and orienting children and families for diagnostic and treatment procedures using developmentally appropriate methods. Provides support and helps facilitate coping through stress reduction techniques.

7. Participates as a member of the interdisciplinary treatment team through program planning and team meetings in order to provide services in a holistic manner. Consults with other team members regarding the developmental status and individual needs of children to promote adaptation to treatment.
8. Presents educational programs to health care professionals and to the community regarding child health and development, the impact of illness, injury and hospitalization on children and families and parent education.
9. Collaborates with other professionals regarding the psychosocial, emotional and environmental needs of the child to promote adaptation to hospitalization and treatment. Offers counsel specific to the impact of health care experiences.
10. Provides guidance and training to child life students and volunteers. Assists with maintaining oversight of the Pet Therapy volunteer program.
Qualifications:
1. Bachelor's degree in Child Life, Child Growth and Development, Recreational Therapy or related field.
2. Eligible for certification as a Child Life Specialist by the Child Life Certification Commission.
3. 600 hour internship experience including six weeks training in pediatrics preferred.
4. Requires minimum of 2 years' experience working as a Certified Child Life Specialist in a clinical setting.
5. Requires application acceptance, Facility Dog placement and official team training by Canine Assistants.

We have a market-based compensation structure. The estimated salary for this position will vary based on a candidate's transferable work experience and qualifications. This is a salary grade 105. Hourly rates start at $23.35.
As a Child and Family Advocate in the Kids- TALK Program, you will provide direct services to children and families seen at the Kids-TALK Children's Advocacy Center. This position contributes to the mission of the Children's Advocacy Center by providing advocacy, crisis support and referral services to child victims of abuse and neglect and their non-offending families and caretakers. The Advocate is required to ensure all aspects of the Kids-TALK program meet the program's mission while staying in compliance with required reporting agencies. The Advocate monitors that referrals are handled in a timely manner and that all required data is submitted accurately and timely.
Requirements
To qualify for this position, you will need the following:
Bachelor's Degree in Human Services related field required.
Two years of experience working with abused/ neglected children and their families is required.
A working knowledge of the dynamics of child maltreatment, trauma, and child development is required.
Knowledge of the criminal justice system, Children's Protective Services/ Foster Care Services and/or the courts is required.
The ability to engage children 17 and under and their families from diverse ethnic, racial, and socio-economic backgrounds is essential.
The preferred candidate will also have training and experience in working with multidisciplinary teams consisting of law enforcement, human service professionals and the court system.
Candidates should be able to demonstrate excellent interpersonal communication skills, a strong commitment to customer service, the ability to work in a cooperative manner with diverse professional groups, and the ability to develop and maintain a strong resource network.
Must have MDHHS Central Registry and Police clearance. Valid driver's license and favorable driving record is required.
Additional requirements include: a nonjudgmental approach that reflects cultural competency and sensitivity to the unique needs and diverse experiences of children and their families, the ability to develop collaborative relationships with other providers, the ability to assess and intervene within complex family and community systems, and a willingness to be flexible, patient, and creative.
