Key Responsibilities:
Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.
Develop and implement evidence-based treatment plans tailored to the needs of each patient.
Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.
Maintain accurate and timely clinical documentation in accordance with regulatory standards using our clinician-designed electronic medical/health system (EHR).
Monitor patient progress and adjust treatment plans as needed.
Requirements:
Active and unrestricted license to practice as a Licensed Professional Counselor in State applying for.
Effective oral and written communication in English and Spanish
Basic proficiency with technology, including electronic health records (EHR).
Strong organizational and documentation skills, with attention to regulatory compliance.
Ability to provide in-person services at assigned facilities, up to 45 minutes.
Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.
Full Vaccination and Booster Status may be required in some facilities.
Preferred Qualifications:
Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting.
History of treating Anxiety, Depression and Adjustment Disorders.
Psych or Addictions experience is a plus.
Excellent time management and organizational skills.

The FPS program is mandated to provide services to families whose children are at imminent risk of placement, or whose children are being prepared for reunification. The FPS program works to ensure the safety of all family members, prevent the out-of-home placement of children considered to be at imminent risk of removal from their home, improve family functioning, reduce the recurrence of maltreatment, help families utilize community resources more effectively, and assist families with the process of reunification where separation or removal has already occurred.
Key Responsibilities:
Provide short-term, intensive, in-home counseling to 2 families at a time, each receiving a minimum of 5 hours of weekly face-to-face contact.
Teach, coach and model parenting skills and effective parent-child interactions and advocate and link families to concrete services and supports.
Work collaboratively with DCP&P and community service providers
Requirements:
Bachelor's degree in counseling or a related field with 1.5 years professional experience and a valid driver's license and insured vehicle.
Strong engaging, communication and collaboration skills and must demonstrate initiative, reliability and the ability to work well independently

Program Info
Family Preservation Services (FPS) is an in-home program intended to assist the Division of Child Protection and Permanency (DCP&P) in meeting the State's obligation to preserve the sanctity of the family and prevent the placement of children outside the home by strengthening families through the application of intensive family preservation services. The FPS program is mandated to provide services to families whose children are at imminent risk of placement, or whose children are being prepared for reunification. The FPS program works to ensure the safety of all family members, prevent the out-of-home placement of children considered to be at imminent risk of removal from their home, improve family functioning, reduce the recurrence of maltreatment, help families utilize community resources more effectively, and assist families with the process of reunification where separation or removal has already occurred.
Key Responsibilities:
Provide short-term, intensive, in-home counseling to 2 families at a time, each receiving a minimum of 5 hours of weekly face-to-face contact.
Teach, coach and model parenting skills and effective parent-child interactions and advocate and link families to concrete services and supports.
Work collaboratively with DCP&P and community service providers
Requirements:
Bachelor's degree in counseling or a related field with 1.5 years professional experience and a valid driver's license and insured vehicle.
Strong engaging, communication and collaboration skills and must demonstrate initiative, reliability and the ability to work well independently
Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently

it in the medical record, including information about the personal and family
problems related to the resident's illness and care, support network, actions
taken to meet the resident's individual needs and eventual discharge to an
appropriate level of care based on functional capacity. Pertinent social data
shall be made available to the attending physician and other appropriate staff
members;
· Acts as a liaison between residents, families, outside agencies, and the
facility Administrator to ensure that the resident's rights are maintained;
· Functions in a manner that adheres to all policies/procedures of the
facility, as a representative of the Windsor Health Care, LLC's Center
administration;
· Documents and prepares any reports requested by the administration; also
councils and provides assistance when change in financial status occurs for
residents in the Windsor Health Care, LLC's Center;
· Coordinates discharge planning and assists with the discharge planning and coordination of community

JOB SUMMARY:
The Family SupportSpecialist II is responsible for providing comprehensive case management, referrals, and seamless coordination with community and supportive service activities that promote economic self-sufficiency for families, enhance the quality of life for seniors and people with disabilities, and increase opportunities for residents. This role involves collaborating with a team of professionals in various fields to determine suitable approaches and supportfamilies in different neighborhoods under the U.S. Department of Housing and Urban Development.
JOB RESPONSIBLTIES:
Conduct intake, assessments, enroll residents in family support program, and visit various locations for resident information gathering.
Collaborate with families on Development Plans, assist in accessing social services, advocate for service delivery for up to 100 individuals.
Ensure privacy, track resident data in USI's "LEARN" system, attend community and program-related meetings.
Available for occasional nights/weekends, travel up to 25%, perform administrative tasks.
Providing resources and knowledge for clients to access capital, including loans for financial goals like housing and consumer products, to build credit, reduce debt, and support wealth growth through entrepreneurship.
Offering consultation services in financial management and goal-setting.
Hosting in-person and virtual seminars for clients seeking financial literacy management resources and capital access.
Supporting and promoting the development of the USI CDFI client base.
QUALIFICATIONS:
Bachelor's degree in Social Work, Sociology, Psychology, or related field.
3+ years of experience as a case manager or in direct practice with low-income households; health services experience a plus.
Valid driver's license and automobile availability for work use.
Commitment to strengths-based, family-focused service planning and understanding of program philosophies.
Ability to handle confidential information professionally.
Independent and proactive work ethic, with strong team collaboration skills.
Availability for evening and weekend functions as required.

What You Get to Do:
The Behavior SupportSpecialist is responsible for developing Behavioral Support Plans (BSP) for specific individuals as identified by the Area Director. The Behavior SupportSpecialist will assess an individual and develop a complete plan, which outlines support strategies to address specific target behaviors as identified in the assessment process. The Behavior SupportSpecialist will be responsible for training the Direct Support staff, and the identified Program Director (PD) on all aspects of the Behavioral Support Plan. The Behavior SupportSpecialist will also be responsible for providing on-going consultation and assistance to the Interdisciplinary Team (IDT) for the purpose of updating the plan in response to the changing behavioral needs of the person served.
Qualifications
