The Family Advocate is responsible for implementing the core family partnership, parent involvement, and community partnership functions for the center-based option of an Early Head Start program. The Family Advocate recruits and enrolls children for the program and then uses a family development model to support and assist a caseload of up to 40 families as they identify and meet goals, they establish in Family Partnership Agreements. This position also assists with the completion of the child health requirements and other required screenings and assessments. The Family Advocate supports the larger system of social services coordinated by Early Head Start, thereby supporting the goals of the program. This position may involve reassignment of caseload and/or center location as deemed necessary for program needs and operations.
RESPONSIBILITIES/DUTIES (including but not limited to):
Recruit underserved populations and determine the eligibility of children.
Maintain funded enrollment levels and fill vacancies.
Assign infants and toddlers to program options that best fit their family's needs.
Analyze causes of absenteeism.
Assist with child health screenings and other required screenings.
Facilitate parent orientations to the program.
Engage in collaborative partnership building with up to 40 families.
Identify family goals, strengths, and necessary services and support.
Identify and access services and resources for families.
Ensure parents are involved in program services.
Arrange for education and other appropriate intervention for families.
Establish community partnerships and promote access to community services.
Maintain contact with teaching staff and infants/toddlers in a classroom setting.

The Arlene Cooper Community Health Center is a leader in providing comprehensive, high-quality HIV testing, STI screenings, and treatment to people in need, with compassion, dignity, and respect. Under the supervision of the Director of Clinical and HIV Services, the HIV Advocate plays a vital role in helping reduce the transmission of HIV and STIs in our community. Our program follows the Harm Reduction model, which calls for the non-judgmental, non-coercive provision of services and resources to people who use drugs and the communities in which they live in order to assist them in reducing attendant harm. They will conduct HIV tests, STI screenings, and counseling. The HIV Advocate will ensure newly diagnosed HIV+ individuals are referred and linked to medical services. Other duties include maintaining confidentiality of participants, reviewing and completing data collection forms, compiling data for monthly reports, and helping co-workers with any other duties assigned. With a focus on the reduction of stigma, the HIV Advocate interfaces with the community to educate them on HIV/AIDS through individual and/or group sessions, Center-related programs, services, and events. The HIV Advocate provides non-medical case management and case coordination to HIV positive patients to assist them in navigating services. _______________________________________________________________________________________________________________________________________________________________________________________________________ KEY RESPONSIBILITIES:
Conduct rapid HIV testing, rapid HCV testing, and pregnancy testing.
Identify at-risk groups to prevent, treat, and thereby reduce the incidence of communicable diseases and risk to others.
Provide counseling concerning personal behaviors and activities that contribute to disease transmission.
Act as patient advocate; counsel clients regarding medical and social services available and intervene on their behalf for access to services or resolution of conflict.
Counsel and educate patients before testing, patients receiving test results, and post-test results. Answer disease-related questions.
Draw blood samples from individuals in the clinic.
Conduct community outreach activities to increase awareness of HIV Counseling and Testing services within the Men Who Have Sex with Men (MSM) community through distribution of flyers, e-mail blasts, events, and testing sites at bars, clubs, and similar establishments.
Work as part of an interdisciplinary team with the Director of Clinical and HIV Services to provide clients with client-centered care, systems navigation, referrals, and follow-ups.
Conduct outreach in a planned and coordinated manner with local prevention programs to avoid duplication of services and provide statistical data to the Director of Clinic and HIV Services.
Target populations known to be disproportionately at-risk for HIV infection, as well as establishments frequented by individuals exhibiting high-risk behaviors throughout the community.
Maintain up-to-date confidential client records, including progress notes, support group documentation, Arlene Cooper Community Health Center utilization, and all other required paperwork, and electronic submission of client data into the electronic health record.
Perform tests at off-site locations; educate the public to provide risk reduction training; give speeches and informative talks to various small groups.
Work as part of an interdisciplinary team with the Director of Clinical and HIV Services and other team members to increase awareness on what resources are available for the HIV positive community and update the resource card to distribute via safer sex kits when HIV positive individuals collect their medication.
Assist with recruitment and retention of clients, particularly those who are disconnected from care and services.
Work closely with the Director of Clinical and HIV Services to provide education/training on harm reduction, safer sex messages, and cultural sensitivity related to the HIV-positive and the LGBTQ communities with HIV/AIDS, who are not in care, to connect them to services and support groups.
Ensure the clinic environment is clean, and ensure that testing rooms and provider rooms are maintained and kept in an orderly manner.
Answer phone calls, navigate patient questions with guidance from medical providers, review and return patient voicemail and calls in a timely manner.
Complete tasks as outlined on the daily task sheet and as assigned.
Draw up medication and deliver injections to patients.
Take and record patient vital signs, medical histories, and prepare patients for medical tests and examinations.
Complete morbidity reports for reportable diseases. Keep current with local, national, and international disease trends and outbreaks.
Cultivates new and existing relationships with Community-Based Organizations, AIDS Services Organizations, colleges and universities, clinicians and providers, hospitals, etc.
Engage in various activities to increase The Center's capacity to promote reduction of stigma campaigns and the distribution of Safer Sex Kits through various distribution outlets.
Participate in assessment, evaluation, and development of HIV Services programming.
Attend agency meetings, supervision, and training as required.
Perform other duties as requested.
EDUCATIONAL BACKGROUND/MINIMUM QUALIFICATIONS:
Preferred bachelor's degree in public health, Public Administration, Social Work, Psychology, or Sociology. Minimum high school diploma or GED required. Two years of experience may be substituted for a bachelor's degree.
Candidates with lived experience within HIV care and prevention, and or with substance use treatment or harm reduction are encouraged to apply.
Live in and have strong community ties to Clark County, including Las Vegas, Henderson, North Las Vegas, and unincorporated Clark County.
Proficient in Word / Excel / PowerPoint
Possess a valid driver's license, reliable personal transportation, valid car registration, and insurance. Travel in a personal vehicle is required for this position.
The ability to verify identity and provide proof of eligibility to work in the United States, and completion of the required employment eligibility verification document form upon hire.
Ability to pass a background check, fingerprinting, and a drug test, and have a clean driving record.
WORK EXPERIENCE:

Provides intervention services for IHFS
Develops and maintains effective working relationships with families and with community service resources, including medical professionals, public welfare personnel, lawyers, therapists, law enforcement staff, school officials, neighbors, and religious and youth organization representatives utilizing referral sources as necessary.
Provides screening and assessment to determine program eligibility and identifies areas to address in the service plan if applicable.
Provides training and support in the development and utilization of functional skills to families with problems and individuals associated with the families.
Assesses problems and provides care to youth and families; monitors individual and family progress in the attainment of predetermined goals providing regular review and update of service plans.
May provide outreach and prevention services to include increasing community awareness, and offering informational and educational service programs to youth and families as required.
Maintains appropriate level of direct contact with families per program requirements, and remains available to families 24/7, for crisis intervention as needed.
Provides consumers, including school personnel and others working with children served by the program, training in the management of difficult behaviors.
Promotes family self-determination by assisting family in identifying and accessing community services and resources.
Educates families on the needs of good physical health, quality medical attention and preventive health care.
Assists families in building on their spiritual and religious beliefs or practices as a source of strength to reach their goals.
Promotes culture of professionalism through role modeling and respect.
Meets with families and children at various locations; utilizes own vehicle to travel to work destinations and to transport youth as authorized.
Completes administrative functions for IHFS
Prepares and submits reports and recommendations concerning case load status, budget requests, changes in service plans, and any unusual incidents that occur in the operation of the program.
Reports any critical incidents involving family members or staff according to standard reporting guidelines.
Prepares proper documentation and service plans in a timely manner as defined by the program.
Implements feedback from supervisor.
Ensures compliance with all contractual, regulatory, program, and accrediting body standards.
Notifies supervisor of change in professional status or standing that may affect job status or program compliance including driving record, driver's license suspension, vehicle insurance cancellation, and missed mandatory training.
May be trained to become a certified Common Sense Parenting Instructor and as a certified instructor may be asked to conduct Common Sense Parenting (CSP) Courses at designated locations as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES:
Solid understanding of the philosophy and function of public assistance and social services systems. Knowledge of recognized interventions such as risk assessment and crisis intervention in order to address identified service needs.
Knowledge of regulatory requirements pertaining to youth care and the ability to learn, apply, and explain complex regulations and policies governing child welfare processes and services.
Ability to function independently and have flexibility, personal integrity, and the ability to work effectively with youth, staff, and support agencies.
Knowledge of community resources and service providers available in the geographical area; knowledge of the functions and services of the community organizations and related human services.
Ability to communicate with individuals at all levels in the organization and with external business contacts in an articulate, professional manner while maintaining necessary degree of confidentiality.
Computer skills in Microsoft Office.
Knowledge of Boys Town model and fidelity of care.
REQUIRED QUALIFICATIONS:
Bachelor's degree in human services or related field and 1 year of experience working with children and families required.
Minimum 1 year of experience including working with children and families required.
Possess a valid driver's license with a good driving record required.
Pass an annual Motor Vehicle Registration (MVR) check required.
Ability to provide own transportation (mileage reimbursed) to complete travel requirements of the job required.
Meet auto insurance requirements established by Boys Town policy and/or State and Local laws required.
Available rotationally to provide crisis response and must be available to work non-traditional business hours including early mornings, nights, weekends, and holidays required.
Based on business need, some areas may require bilingual skills required.
May be trained to become a State Approved Family Team Decision Meeting and/or Youth Transition Decision Making Facilitator and may be asked to conduct facilitated meetings at designated locations as assigned.
