    The Housing Supports and Service Specialist assists with administering the Housing Supports and Services Program providing In Lieu of Services (ILOS) for Nevada Medicaid. Essential Functions: Coordinate specialized care management for eligible members, to include intake and screening, member assessment, and development of individualized service plans. Develop and implement screening tools to identify member preferences and barriers related to successful tenancy and options and resources to secure housing. Search available housing and present members with housing options. Ensure service coordination utilizing data systems for housing (e.g., Homeless Management Information Systems). Assist members with securing housing, to include assistance with housing applications, securing required documentation, and accessing other benefits in support of housing stability, to include SSI, SNAP, and/or housing vouchers. Assist member with receiving maximum allowable and available benefits under community social services Housing Supports Program to subsidize the cost of rent and utilize ILOS covered benefits for expenses that support securing housing. Help members with requests for reasonable accommodations. Communicate and advocate on behalf of members with landlords; facilitate landlord education and engagement; provide coaching to members on developing and maintaining relationships with landlords and/or property managers. Familiarity with Continuum of Care (CoC) standards and guidelines for member's region. Collaborate with CoC workgroups and housing service organizations to mitigate tenancy risks. Possess knowledge of Public Housing Agencies and guideline/eligibility for vouchers programs like Housing Choice, Section 202 Section 811, etc. Coordinate in-home health care services and supports as needed to ensure care is available to members in support of successful tenancy and home living. Identify, coordinate, secure and/or assist with funding non-emergency, non-medical transportation to assist members' mobility. Create housing sustainment plans for members, create monthly budgets, and provide identification and early intervention for behaviors that may jeopardize housing (e.g. late rental payment, hoarding, substance use, or lease violations). Educate members on rights and responsibilities of tenants and landlords under Fair Housing Act. Connect members with community resources to prevent eviction when housing is, or may potentially become, jeopardized. Provide independent living and life skills education and training, to include budgeting, financial literacy, employment supports and connection to community resources and services to support housing stability. Perform any other job-related instructions, as requested. We are presently looking for Full Time Behavioral Health Associates who will be responsible for provision of routine mental health/psychiatric nursing care for inpatients and partial hospitalization patients in the Adolescent, Adult Psychiatric, Adult Chemical Dependency and Geriatric Programs under the supervision of the Registered Nurse.
