The Hospice Journey Counselor will provide information, education, and support to patients, families, and caregivers regarding end-of-life planning and care. Assist them in the transition to Hospice care when the patient is both eligible and appropriate for Hospice services. This position will be in our Butler, PA territory.
Hospice Journey Counselor Job Responsibilities:
• Manage Journey patient census and referrals for the assigned area from the Journey candidate list, the Journey Administrative Assistant, Customer Service, Clinicians, and other sources
• Review all assigned patient charts to determine eligibility and appropriateness for the Hospice benefit and prepare for conversations with the patient, family, and/or caregivers
• Make initial phone calls to all new Journey referrals to schedule home visits, as well as follow-up calls to maintain contact with the patient and assess patient decline
• Conduct patient visits for building a relationship with the patient and/or family, and in-depth discussion of the hospice benefit when appropriate
• Maintain Journey patient charts, documenting all Journey visits and contacts in Journey Visit/Phone call notes or as Coordination notes when applicable
• Build relationships with and maintain effective communication with Clinicians to ensure effective collaboration for addressing patient needs
• Ensure that Journey charts are created and maintained for each Journey patient
• Meet and exceed if possible, expectations for weekly patient visits, weekly patient phone calls, and monthly transitions to hospice care
• Assist clinicians in a more accurate understanding of the philosophy, goals, eligibility, and benefits of Hospice care
• Assist the Hospice and/or Home Health team in efficient transitions to Hospice in whatever capacity may be needed, whether in extra visits, calls, paperwork, or any other task
Hospice Journey Counselor Qualifications/Requirements:
• High school diploma/GED
• Prior home healthcare or relative work experience in the healthcare industry preferred
• Understanding of the hospice philosophy preferred

SUPERVISORY RESPONSIBILITIES
The Clinical Lead Behavior Specialist is responsible for overseeing behavior specialists. Duties include managing and directing staff, conducting meetings, interviewing, and training employees, planning, assigning, and directing work, evaluating performance, addressing complaints, and resolving issues. Additionally, this position guides, informs, supports, and evaluates with the behavior specialists the needs of those we support concerning behavioral health requirements and may provide input on the performance evaluations of direct support professionals, particularly regarding the clinical aspects of their responsibilities.
EDUCATION and/or EXPERIENCE
A behavioral /mental health professional - Licensed psychologist, Licensed Clinical social worker (LCSW), professional counselors (LPC), Licensed Marriage and family therapist (LMFT), Licensed Behavioral specialist (LBS), Licensed Social worker (LSW), CRNP, BCBA, BCABA, Licensed Associate Professional Counselor (LAPC), Licensed Associate Marriage and Family Therapist. Work experience working directly with persons with intellectual and/or developmental disabilities. Highly preferred to have two years of work experience working with people with intellectual and/or developmental disabilities
OTHER REQUIREMENTS
* Extensive local travel.
* Staff hired will be at least twenty-one years of age and will have received Acts 33 and Criminal clearances. All staff will be routinely screened for exclusion from participation in Federal Health Care Programs.
* Valid driver's license and current vehicle insurance, registration and inspection as well as access to a vehicle.
* Maintain training compliance as defined in all pertinent regulations.
* Ability to comprehend, access, and utilize electronic medium and various computer programs.
* Ability to present information effectively, write reports, respond to questions from both internal and external stakeholders, and guide the team of behavior specialists in performing these tasks.
* Ability to comprehend and apply basic mathematical concepts.
* Some non-traditional working hours (i.e., evenings/weekends/early mornings) will be necessary to meet the needs of the people supported.
* Pursuit of state level licensure (as eligible) or Nationally Recognized Certification Program within first year of hire.
* Successful completion of motor vehicle records and background check.

Southwestern Human Services is hiring Part-Time and Full-Time Behavioral Specialist Consultant and Mobile Therapist positions.
A Behavioral Specialist Consultant (BSC) serves as the lead clinician on the child's treatment team. The BSC responsibilities include designing the behavioral treatment plan, modeling interventions for the therapeutic staff support (TSS) and parent/caregivers, and implementing behavioral interventions to stabilize targeted behaviors.
Responsibilities also include: assisting in educating and empowering families, overseeing the implementation of the behavioral treatment plan by the TSS, submitting weekly clinical documentation.
Services are intended to implement specific interventions to stabilize the client, provide support to the family, and promote age-appropriate behavior and are provided in the client's natural environment; in homes, schools, and community programs throughout Allegheny County.
