S ummary of Program and Position:
Ulupono Family Strengthening Program works to increase the internal resources of families to handle challenges they face with their children. Family support specialists on Oahu and Maui work with parents/caregivers of children ages birth to 17 years old to provide parenting education, promote child health, development and safety, offer emotional support, link families to resources in their community.
The Family Support Specialist provides voluntary, culturally appropriate, and strengths-based services related to child safety concerns to families referred by the Department of Human Services who are low to moderate risk for child abuse and neglect. This position is under the direct supervision of the Program Supervisor or Clinical Supervisor/Trainer.
Essential Duties:
Ensures that the delivery of service is done in a manner consistent with the standards and guidelines established by the funding agencies, state law, and PACT policies and procedures.
Conducts home visits, engaging families into services through persistent, creative outreach.
Assesses each family's strengths and issues, identifies their needs, and develops goals and objectives as part of the Family-Centered Case Plan developed with the family. Completes required assessments (Child Safety Assessment and Comprehensive Strength and Risk Assessment Tool), including the Family Check-up assessments and intervention according to required timelines.
Provides evidence-based social services using a trauma and healing-informed approach and motivational interviewing consisting of parenting skill building, child development education, alternatives to corporal punishment, increasing knowledge and skills in meeting child and family needs, assistance in developing a support network, transition planning, and other areas as needed.
Links families to needed community resources and conducts follow up activities as needed, teaching the family to identify and access resources throughout the process.
Meets monthly and as needed with the Program Supervisor and Clinical Supervisor/Trainer for case consultation to discuss progress in families, accepts support and carries out directives from the Program Supervisor, and notifies their supervisor of progress, challenges, and any significant and critical issues, including incident reports.
Completes clear, concise, and non-judgmental written and electronic documentation, such as initial assessments, monthly updates, and closing reports, and ensuring that all paperwork is completed and submitted in a timely manner.

Family Center provides a wide array of services to individuals and Families living in Kalihi and surrounding areas. The Sex Abuse Treatment Center (SATC) of the Kapi'olani Medical Center for Women & Children is a statewide program established in 1976 in response to the community's concern over the absence of medical, psychological and legal support services for victims and the absence of police reporting. Today it is recognized for its leadership and expertise in providing treatment services for survivors of sexual assault, preventing sexual violence and effecting change through public policy, awareness and education. Our mission is to support the emotional healing process of those sexually assaulted in Hawai'i, to increase community awareness about their needs and to reduce the incidence of all forms of sexual assault. Through research and education we also aim to improve clinical practices. By promoting and engaging in public policy we are changing public perceptions of sexual violence, overcoming barriers to treatment and prevention, and ensuring effective criminal justice practices.
As a Crisis Worker Trainee, you will go through a comprehensive training program made up of both didactic and on the job training to prepare you for the role of Crisis Worker for the Sex Abuse Treatment Center (SATC). Crisis Workers provide compassionate, front-line intervention and support services to adults and youth who have been sexually assaulted. S ummary of Program and Position:
Voluntary Case Management (VCM) program serves families on Oahu who have been referred by Child Welfare Services (CWS) after a report to the Child Abuse and Neglect Hotline has been made and the family has been identified as moderate to moderate high risk. Each family will be assigned a case manager who will address the range of family system issues that place children at risk of child abuse and neglect. They may also be assigned a skill builder who will work with the family to increase skills such as, but not limited to, parenting skills, co-parenting skills, problem solving skills, and stress management skills.
The Social Services Assistant will provide outreach services to families in the Voluntary Case Management Program and provides hands-on, individual or group, culturally appropriate sessions that may include parenting, practical life skills, role modeling, nutrition, and planning. Additionally, they will provide administrative and clerical support to the management and staff of the VCM Program and provide back-up support to the Administrative Assistant. This position is under the direct supervision of the Program Supervisor.
What you will be doing:
Provides home visits or community-based visits to families referred to the program, focusing on parenting, problem solving, and communication skills and assisting the client in reducing risk factors and increasing protective factors.
Perform the usual clerical duties such as word processing including report reporting, correspondence, telephone and in-person reception, filing, data entry, record maintenance, faxing, and timely and accurate data entry into PACT's eHana and/or assigned program's data system.
Maintains confidentiality and tracks family files and family related information such as immunization records and medical home; responds to calls from referred clients explains the scope of service and/or the waitlist process; calls clients to cancel visits when necessary; and documents client contact in client records. Maintains client files from intake to discharge, including the insertion of external documents; providing information to funders as requested; and ensuring completeness of the client file at the end of service.
Responsible for implementing parenting curriculum, knowledge of community resources available and to do warm hand-offs to service provider and understand how substance abuse and domestic violence affect families. Responsible for supporting families in completing goals on the IPP to reach permanency.
Conducts assessments and observation forms in accordance with funder and state guidelines, and PACT's policies and procedures.
Accurately completes clear, concise and non-judgmental written and electronic documentation, such as initial assessments, monthly updates, and closing reports, and ensuring that all paperwork is completed and submitted in a timely manner.
Regularly consults with the VCM Case Manager and/or Program Supervisor.
Minimum Qualifications:
1. Bachelor's degree in Social Work, Psychology, or a related field plus a minimum of one (1) years relevant experience, required, OR
2. High School diploma plus a minimum of two (2) years' relevant experience under the close supervision of personnel who at minimum has a Bachelor's degree in Social Work, Psychology, and/or a related field.
