Position Summary: The Engagement Specialist is primarily responsible for coordinating support services and case management to youth and their families. The position provides for the coordination of services, oversight, and leadership along with acting as the liaison of service options for youth and their families.
Position Qualifications:
* A Bachelor's degree in Mental Health, Psychology, Education, Counseling, Child Development/Child Care or Social Work with a minimum of two years' experience in related services or 5 years' relevant work experience.

Primary Responsibilities:

Assistant Program Manager Responsibilities:

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Ensures completion of new hiring orientation and training including mandatory in-services and ICD code training when applicable. Ensures documentation completed for annual in service training, and policy and procedure in service updates.
Responsible for all patient care employees receiving appropriate training according to FMS policy, including training to ensure ongoing compliance with all FMS risk management initiatives.
Provides opportunities for professional growth, and training to ensure clinical competence and the ability for licensed staff to assume Team Leader responsibilities
Responsible for overseeing performance of all licensed personnel, direct patient care staff, reporting indirect patient care personnel as assigned, and when necessary, technical staff. Provides employee education and guidance, and feedback related to performance.

In collaboration with the interdisciplinary team, informs, educates and supports staff in understanding the emotional, psychological and behavioral impact of Chronic Kidney Disease on the patient and family to ensure comprehensive quality care of our patients. Patient Assessment / Care Planning / Counseling

Patient Education

With other members of the interdisciplinary team, provides appropriate information about all treatment modalities.
Facilitates the transplant referral process and collaborates with interdisciplinary team on transplant waitlist management.
Provides ongoing education to patient/family regarding psychosocial issues related to End Stage Renal Disease (ESRD) and all support services that are available.
Reviews patient rights and responsibilities, grievance information (company and network) and other facilities policies with patient and/or the patients' representative to ensure patients' understanding of the rights and expectations of them.
Collaborates with the team on appropriate QAI activities.
Patient Admission and Continuity of Care
Reviews Patient Rights and Responsibilities, Grievance Procedure & Important Numbers Handout, FKC Non-discrimination policy, DNR Statement (if applicable) and address any immediate needs/concerns.
Understands the referral and admission process and supports the clinic in regard to the patient needs for scheduling to maximize adherence and adjustment.

Staff Related

Medication Administration:
Provides the highest degree of quality care and services by administering medication and treatments in a safe organized manner.
Reviews, reads, notates, and initializes Daily Log to document and learn about pertinent information about residents.
Receives medication updates from Resident Care Director (RCD) or Wellness Nurse.
Administers, assists with, and observes medications and treatments for each resident using the medication administration record and the Six Rights of Medication Pass ("Right" resident, medication, dosage, time, route, right to refuse).
Ensures that medications are passed according to times.
Documents and initials as medications are given and ensures that appropriate documentation is completed for refusal or missed doses.
Maintains confidentiality of all resident information including resident medication among other residents.
Reports all resident concerns made while administering the medication to the RCD or Wellness Nurse.
Restocks medication cart after all medication passes.
Assists in checking medication regardless of packaging system.
Assesses the residents to determine need for "as needed medication" and appropriately documents and reports to supervisor.

Maintains and cleans the Medication Room, medication carts, and treatment carts for neatness, cleanliness, availability of medications, and expired medications.
Follows re-fill process for medications.

Ensures any cords, carts, equipment, and other hazards are always kept out of the way, not blocking exits and in compliance with fire codes.

JOB TITLE: Mental Health Therapist
SUPERVISOR: Program Director
SUMMARY
This is a direct care clinical position that is primarily focused on the provision of counseling services to individuals with a primary mental health diagnosis. A Mental Health Therapist is responsible for the provision of a variety of services including, but not necessarily limited to; counseling, intake, referral, case management, and other duties related to the provision of professional counseling, as well as the appropriate maintenance of a client record. Assignments and other duties may be expanded based on the therapist's ability and scope of professional expertise. Mental Health Therapists are directly supervised by the Program Director of the location where they work. Contact with individuals and groups, family members, managed care and insurance companies, and public welfare offices are a major aspect of this position. Ultimately, the Mental Health Therapist is responsible for the professional delivery of therapy services to those clients on his or her caseload, as well as the appropriate maintenance of all client records under his or her care. This includes assurance that all documentation in a client record is completed in a thorough and timely fashion, and complies at all times with all regulatory obligations (i.e., OMHSAS guidelines).
QUALIFICATIONS
* A licensed mental health clinician (psychiatrist, psychologist, licensed professional counselor, licensed marriage and family therapist, or a licensed clinical social worker)
OR
* A minimum of a master's degree from an accredited university in a clinical mental health discipline, such as: psychology, counseling, clinical social work, psychiatric nursing, or marriage and family therapy. In order for a degree to count as a mental health degree, the degree must contain at least twelve (12) credit hours in clinical coursework in the foundations of behavioral health and the provision of mental health services (i.e., individual, group, and family therapy, abnormal psychology, psychopathology, cognitive behavioral and psychodynamic therapy, crisis counseling, assessment, child development, personality theory, psychotherapy lab, clinical practicum.) Other courses may count as determined by a review of the transcript/syllabi. No more than three (3) of these twelve (12) credit hours can be derived from a supervised clinical mental health practicum experience (teaching/education/other non-clinical practicum experiences will not count). Courses such as statistics, program and policy development, and program administration will not be counted as behavioral health courses because they do not include instruction in the direct provision of mental health services. If coursework is part of a medical degree, there must be clear evidence that the course was directly related to the diagnosis and treatment of mental health disorders rather than medical conditions. Appropriate medical degrees/training will include substantial training in mental health disorders; medical degrees that are behavioral health-related usually include a residency in psychiatry.

* A minimum of two (2) years verified paid experience providing mental health treatment. In special circumstances, individuals with less than two (2) years' experience may be hires. These individuals must receive intensive supervision on a weekly basis until two (2) years' experience is acquired.

Position Details
BP6 is a 28 bed Medical-Surgical Remote Telemetry unit that includes a diversity of services including General Surgery, Bariatric, ENT, Urology, Gynecology, Plastic Surgery, and Trauma Surgery. We have many resources on our unit including a pharmacist, care manager/social worker, and scheduler to support unit based needs. In addition, we have a wound ostomy team and an IV team available throughout the hospital for added support. Team members have the benefit of a shared schedule in which we partner together to complete a final schedule based on unit needs.

Neuroscience ICU is an 18 bed critical care unit with primary focus being on the neurosurgery and neurology patient populations. The NSICU will offer cutting edge neurosurgery technologies and innovations in a fast-paced environment. * Supervises the implementation of service plans that address all life domains by monitoring case activity, documentation and treatment planning.
* Supervises staff and evaluates performance and training needs by direct observation, consumer feedback and the monitor of case activity, documentation and treatment planning.
* Assists the Program Manager in the development and implementation of policies and procedures regarding all service components.
* Acts in role of and supervises staff to act in role of advocate, liaison and case manager in all service components to support services to clients and the mission of the agency.
* Supervises staff to provide and directly provides therapeutic interventions according to individual service plans, court orders, state regulations and statutes and contractual requirements.
* Anticipates and/or intervenes directly in crisis situations, using approved behavior management and crisis intervention techniques, including but not limited to de-escalation; conflict resolution; individual and/or group strategies; and, restrictive procedures, including passive manual restraints only when there is an imminent danger to self and others and when other less restrictive means of managing behaviors are unsuccessful.
* Provides leadership and support in the management of crises.
* Provides and/or directs transportation for external clinical appointments, family contacts and court hearings.
* Provides on-call duties as assigned.
* Ensure compliance with health and safety procedures and instruct staff accordingly.
* Provide all documentation and reports as requested along with responsibility for the completion of timely, accurate, and professional written and verbal documentation of all service, staff and program operations.
* Attend all trainings, supervisions and meetings as assigned.
