Full-time Description
The Physician's primary job function is to be responsible for provision of clinic based comprehensive vision care for patients of FHCN. Works independently and interdependently with other clinic providers. Job performance is evaluated through direct observation, consultation and written documentation.
ESSENTIAL DUTIES AND RESPONSIBILITIES
(Included but not limited to the following);
Access & Quality
Provide or arrange for comprehensive primary care services in a primary medical care ambulatory clinic setting, including laboratory and diagnostic workup, and specialty referrals.
Works independently using his or her best professional judgment while practicing medicine at FHCN within his or her scope of practice. Works interdependently with other health care providers and health care staff in a team environment.
Perform minor outpatient procedures in accordance with FHCN credentialing/privileging policy.
Provide consultation to other clinic providers either in person or by telephone.
Consult with and coordinate patient care with clinic physicians, covering community primary care physicians, specialists, pharmacists, nurses, or other health care professionals as appropriate.
Assess patient needs and arrange for specialty referrals, diagnostic and in-patient care and make appropriate referrals for service.
Responsible for accurate, timely, and complete documentation in patient records in accordance with FHCN policy.
Provide patients and family members with counseling and education regarding health maintenance, disease prevention, diagnosis, treatment, and need for follow up as appropriate during each patient visit. Verify and document that the patient understands diagnosis, treatment and follow up recommendations.
Renders care to FHCN patients in accordance with quality and standards as are established by FHCN, the standards of care prevailing in the community, applicable law, and according to state and federal grant requirements for non-discrimination regardless of status, race, sex, religion, national origin, handicap, age, or ability to pay.
Provide clinical supervision and appropriate training of ancillary health care staff to assure quality of care for FHCN patients in coordination with Clinic Operations Managers and nursing staff.
Participate in FHCN Quality Improvement activities including Chronic Care Collaborative programs, immunization improvement initiatives, pharmacy formulary development and utilization, and any other medical staff committee activities relating to peer review, risk management or development of clinical policies and procedures.
Participate in audits as required by state and national accreditation entities in order for FHCN to meet and maintain all necessary standards and requirements.
Maintenance of credentials essential for practice, to include licensure, certification and CME.
Attend all required staff meetings, provider meetings, and participate in FHCN committees or task force activities as directed by Medical Director.
Comply with all HRSA Section 330 requirements, including sliding fee discount program policies, scope of project guidelines, and documentation standards.
Adhere to FHCN policies related to corporate compliance, patient privacy (HIPAA), workplace safety, infection control, and emergency preparedness.
Successfully complete all required background checks, OIG/GSA exclusion screenings, credentialing, and re-credentialing processes.
Advocacy
Treat all patients with dignity and respect.
Provide excellent customer service
Conform to HRSA and HIPAA regulations
PHI (Patient Health Information) Compliance
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
Ability to function effectively as a team member and facilitate completion of tasks.
Experience in FQHC (Federally Qualified Health Center) industry preferred
Requires strong organizational skills, problem solving and analytical skills.
Must be computer literate in accounting software excel, word and outlook.
Able to exercise sound judgment and discretion in managing patient and employee issues.
Requirements
EDUCATION AND EXPERIENCE
Graduation from an accredited medical school with a degree of Doctor of Medicine or Doctor of Osteopathy. Must be Board Eligible/Certified.
Minimum of five years of experience in practice of medicine with progressive administrative responsibilities.
Licensure& Privileges: Must possess current, valid unrestricted license to practice medicine in the State of SC.
Must have current, valid unrestricted DEA registration. Must be able to be continuously insurable by FHCN's professional malpractice carriers at reasonable rates. Must be able to participate in and maintain eligibility to receive reimbursement for services from both public and private health care insurers as required by FHCN.

Coordinate the functions of all departments and develop standards and methods of measuring patient care services, including a chronological record of services provided to patients as required by the ESRD Network Coordinating Council and Medicare.

Conduct and/or participate in both formal and informal meetings with the governing body, Regional Director, Medical Director and the staff as delegated. Assure that appropriate staff meetings, in-service education meetings, and team patient care planning meetings are held monthly as delegated. Assure that Quality Assessment & Performance Improvement Program is current at all times as delegated.
Establish, maintain, and submit all required records and reports concerning staff, patients, and the operational affairs of the center if delegated by Administrator.
Continuously monitor to ensure that a safe and sanitary physical environment is maintained throughout the facility; that all equipment is maintained and functions properly; and that adequate and appropriate inventory levels of all supplies are available and used correctly.

· Home dialysis therapy experience (preferred).

Provide radiographic examinations for patients of all ages using diagnostic imaging modalities. This includes assessing patient condition, operating imaging equipment, proficiency in PACS, preparing examination rooms, and maintaining equipment, supplies, and medications.
