Must be willing to obtain and maintain QMAP certification within six months of hire, and demonstrate ability to follow QMAP procedure as well as CPR/First Aide Certification within 6 months of hire. Food Handlers Certification within 6 months of hire.

Essential Functions
1.
Reliable and punctual attendance is essential; expected to be at job as scheduled each scheduled day.

Communicate necessary information to others as appropriate.

Promotes resident independence in accordance with service plan
4.
Become familiar with resident's daily needs, likes, and dislikes to provide personalized care.
5.
Support and respect each resident's unique life experiences, including spiritual and cultural beliefs.
6.
Maintain a clean and safe physical environment, redirecting and assisting disoriented or confused residents, applying and following house rules and honoring resident rights.
7.
Assist with daily personal care tasks, including bathing, grooming, toileting, oral hygiene, and more.
8.
Providing cleaning of the resident's room, common areas, and work areas. (To include but not limited to the following: sweeping, moping, and dusting all common shared spaces and disinfecting all high-touched surfaces areas.
9.
Assist with transfers or helping residents into wheelchairs or with walkers. This may involve operating a mechanical lift.
10.
Assist with turning or reposition a resident in bed to prevent bedsores or other discomforts.
11.
Perform palliative and post-mortem care.
12.
Assist residents with incontinence care, catheter, and colostomy care.
13.
Complete documentation in an accurate, complete, and timely manner per organizational standards including the use of electronic medical record.
14.
Provide laundry services to resident as assigned, ensuring all laundry is picked up and returned with complete.
15.
Assist with scheduled activities in the absence of Activity Specialist.
16.
Assist with reminders, escorts, and record attendance to activity events, sending, delivering, and reading mail upon request, and assisting with visitation systems.
17.
Respond to resident pages in an appropriate timeframe and manage and prioritize tasks.
18.
Respond to any safety or medical issue, and perform minor first aid.
19.
Assist residents with remembering and being ready for appointments.
20.
Assist dietary staff with setup and cleanup, remind or escort residents to the dining room, assist with menu selections. Deliver meal trays, serve beverages, cut meat, butter bread, and adheres to safe food practices.
21.
Communicate any problems, concerns, or issues to the resident's family members as needed.
22.
Observe, monitor, and record resident's physical and emotional well-being, and promptly report any changes to nurse, lead, supervisor, director or administrator.
23.
Review and record observation notes, initiate incident reports as soon as the event has been discovered after taking appropriate immediate action to ensure the safety of the resident, employee or visitor.
24.
Review and respond to emails and calendar invites. Attends, participates and contributes to staff meetings addressing resident and departmental needs.
25.
Restocks medication carts, drawers and/or storage areas with miscellaneous supplies only (i.e. gloves, sanitizer cups, etc.)
26.
Reports suspicion of abuse, neglect or misappropriating or resources as required by regulation.
27.
Completes work orders to communicate necessary replacements or repairs.
28.
Maintains open communication with supervisor and/or peers to promote awareness of resident issues.
29.

Position Summary
The Day Habilitation Specialist (Day Hab Specialist) will assist individuals with developmental disabilities to maximize their personal and vocational potential in their center and community. The Day Hab Specialist provides direct care support and assists in the implementation and documentation of each individual's program plan. Day Habilitation Specialists for designated programs may be expected to support other programs based on staffing and attendance. Floating Day Hab Specialists will be assigned based on need. Salary Range:
Day Hab Specialists I (drivers)- $17.90 to $19.90 per hour
Day Hab Specialists II (drivers)- 18.41- 20.41 per hour

Position Standards:
Delivers and documents the services, supports, safeguards, behavioral documentation and activities identified by ISP/CFA and the Day Habilitation plan. Completes the monthly note and ensure it covers the individual's response to service within the seven days of the following month for assigned individuals.
Performs individual specific personal care as required, including but not limited to assistance with toileting, transferring, changing needs, handwashing and oral care. Documents any finding on appropriate forms including but not limited skin integrity, bowel and repositioning charts.
Ensures active engagement and appropriate communication with staff and individuals.
Assumes responsibility for medication administration and appropriate documentation in an accurate and timely manner.
Develops a trusting and friendly rapport with individuals served while maintaining professional boundaries.
Fulfills mandated reporter role by interrupting and reporting suspected abuse to administration immediately.
Ensures that the center is kept safe, clean and decorated in an age appropriate and comfortable manner and that is kept in good repair inside and out.
Demonstrates proficiency in all goals and skillsets outlined in the NYS DSP Core Competencies, on which performance will be evaluated.
Responsible for providing transportation to individuals receiving services driving agency vehicles with a capacity up to 14 passengers.
Ensures all required trainings and certifications are current, and seeks further support and training where needed.

She is a HUGE Syracuse Orange fan who has a knack for creative writing, enjoys adult coloring books, using her IPAD, going to the Casino, watching sports and the food channel, surfing the internet, listening to the radio, researching new recipes to try and baking cupcakes.
She and her staff will provide new hires with training. New hires will have training shifts on transfers using an electric sit-to-stand/track system. New hires must have a driver's license, as you will be driving her specialized modified minivan.
Evening shifts are pretty quiet, plenty of down time as she goes to bed around 7pm... DSP can watch TV, read, bring computer etc...
