The Behavioral Health Case Manager will provide telephonic and in-person support for both direct referrals and data identified referrals. This requires clinical expertise and the ability to negotiate the complexities involved with special needs conditions such as substance use, suicidality/homicide, major depression, ADHD, eating disorders, and severe mental illness. This position may require minimal field work to meet with members at local facilities in the future.
Primary Responsibilities:
Facilitate member education and involvement of caregiver in the delivery of interventions
Provide advocacy and support to member and family members, including caregiver support & appropriate referral to applicable / needed resources
Ensure that members understand treatment options and are effectively linked to treatment resources
Promote health, wellness and optimal psychosocial functioning for member (identify caregiver gaps, facilitate education and respite support)
Consider the member's needs holistically to identify gaps in care requiring intervention
Exhibit excellent customer service in engaging providers in collaborative planning
Create and maintain appropriate clinical records
Participate as directed in clinical rounds with other members of the team and other external health care management organizations / vendors, as applicable. Also participate in advancing the Quality Improvement Program
Conduct condition specific research to meet member needs
Maintain success stories which can be utilized to promote program
Provide case management support for individuals who meet diagnostic requirements including engagement of member and/ or family making available support throughout the entire continuum of treatment

Required Qualifications:
Master's degree in Psychology, Social Work, Counseling, or Marriage and Family Counseling or Licensed Ph.D.
Active, unrestricted clinical license to practice independently without supervision in the state of Missouri
2+ years of post-licensure experience in a related mental health environment
1+ years of case management experience
Proven intermediate level computer skills including proficiency with MS Office Suite

About the Remote Licensed Therapist (1099 Contractor) role at Headspace:
Headspace is currently seeking part-time, independently licensed contractor therapists to provide direct, virtual care as part of a multidisciplinary team. We are currently accepting applications from providers licensed in MISSOURI.
Contractor therapists are asked to offer a minimum of 5 sessions/week, with the flexibility to offer additional sessions when needed. More is always welcome depending on your schedule. Please note that due to care demand and member scheduling fluctuations, it could take a number of weeks to achieve a consistent schedule.
The pay rate ranges vary by state and currently starts at $69 - $74 per session for the state of MISSOURI. Pay rates by state will be confirmed by recruiter during the interview process. About the Role
The Licensed Therapist will be responsible for working closely and effectively with a multidisciplinary care team (including nutrition, medical, psychiatry, and mentorship) and utilize evidenced based care modalities (such as FBT and CBT-E). The Licensed Therapist will be responsible for evidence based treatment delivery, quality documentation, and ongoing assessment of patients' and families' needs.
The Licensed Therapist role involves emotionally rewarding but inherently difficult work. Working Hours:
Equip's working hours are Monday - Friday, between 8am-8pm.
* Ability to provide 30 hours a week of availability for direct patient care, including new patients and follow-up sessions.

* Maintain a caseload of 30-40; with the expectation being 20-26 hours of direct care and remaining time dedicated to administrative tasks.
* Participate in initial and ongoing trainings.
* Assess and diagnose mental health conditions.
* Collaborate with a multidisciplinary treatment team of physicians, dietitians, patient mentors, and family mentors, along with outside providers.
* Utilize between-session messaging to support patients and communicate with the treatment team through Equip's EMR in accordance with Equip's policies and procedures.
* Engage and collaborate in Treatment Team meetings, supervision, and department meetings.
* Maintain licensure in home state and obtain licensure in other states as requested by Equip (licensure costs will be covered by Equip).

Qualifications
Must Haves
* Active, unrestricted, and independent license to practice psychotherapy from at least one US state in this posting's time zone.
