Attends and participates in staff, education, and agency committee meetings as appropriate 15. Assists in all therapy and skilled nursing referrals and coordination 16. Responsible for updating HCBS client spreadsheet 20. Responsible for data entry of HCBS referrals and scanning appropriate documents into client charts 21.
