Per Diem MDS Coordinator: As part of a high-functioning care team, conduct and coordinate the development and completion of the resident assessment in accordance with the rules and regulations of State and federal government and the policies and goals of the facility. Essential Job Functions:

* Instruct all disciplines in proper procedures, completion of MDS items, and CAAs, Care Mix payment and Quality Indicators.
MDS Coordinator Minimum Qualifications:
* MDS and ICD-10 coding experience preferred & RAC-CT certification preferred
* Current, active, Maine RN licensure or current active, Maine Licensed Practical Nurse (LPN)
* Strong nursing clinical assessment, care planning, and delivery of care skills, with a long term care population
Skills and Abilities:
* Working knowledge of geriatric health care needs
* Clear and effective communication skills
* Understanding of Federal and State regulations pertaining to long term care and the resident assessment process
* Medicare and MaineCare regulations, as well as the application of the State Operations Manual, and Principles of Reimbursement

Overview
Wound Care Nurse will report to the Director of Nurses and Administrator. The Wound Care Nurse must possess and maintain a current RN License in the State of Maine and must possess demonstrated knowledge and experience sufficient to perform all directed duties with excellence. The Charge Nurse will also work closely with supervising physicians to assess and monitor resident's needs, ensure treatment and care are conducted according to the facility's procedures, administer medications, and perform other related tasks.
Essential Job Functions
* Ensure all nursing staff understand and follow their respective job descriptions.
* Ensure that nursing practice complies with regulatory and legislative requirements.
* Ensure sufficient numbers of licensed and certified staff are available each shift to meet regulatory requirements and provide appropriate care to residents.
* Complete routine clinical rounds each day to ensure all staff are performing work assignments in accordance with acceptable standards.
* Promote, establish, and maintain customer relationships with residents, families, staff, community, and other professionals.
* Review complaints and concerns made by families, residents and staff, communicate these concerns including follow up with the managers, or Director of Nursing, or Administrator as appropriate.
* Ensure policies and procedures regarding communicable diseases, infection control, and isolation procedures as well as other systems pertinent to nursing are followed and maintained.

* Assess deep wounds, skin tears, ostomies, severe burns, pressure injuries or bedsores, and diabetic foot wounds. Communicate with physician and create a plan of care that staff nurses follow.
* Implements care plans to help prevent wounds from worsening.
* Educating staff nurses on wound care protocols.
* Creating, updating, and managing care plans.
* Performing specialized wound care procedures.
* Working with a multidisciplinary team to plan skin care interventions.
* Performing diabetic foot care.
* Educating patients and their caregivers on wound care procedures and preventative measures.
* Charting clinical information for insurance reimbursement.
* Writing nursing orders that promote wound healing and/or prevent skin breakdown.
* Staying up to date with current wound treatments.

We offer a range of services including: long-term care, skilled care, with physical, occupational, and speech therapy services. RN Job Summary
Registered nurses (RNs) work closely with supervising physicians to assess and monitor residents needs, ensure treatment and care are conducted according to the facility's procedures, administer medications, and perform other related tasks.
Responsibilities and duties include but not limited to:
* Cares for residents
* Exhibits strong clinical skills
* Demonstrates excellent bedside manner
* Records a resident's medical history, symptoms, and vitals.
* Updates resident charts and electronic medical records (EMR).
* Administers medication and other doctor-recommended treatment plans.
* Complies with federal, state, and local nursing regulations, standards, and policies.
* Collaborates with nurses, doctors, and administrators.
Requirements:
* AS or BS degree in nursing
* Must be currently registered/licensed in Maine.
* Must have CPR certification upon hire or obtained during orientation. CPR certification must remain current during employment.
* Knowledge and use of Electronic Medical Records and/or solid computer skills
* Strong communication skills, both in writing and verbally
* Effective leadership skills and/or ability to invest in growing this skill
* Read, write, speak, and understand the English language
