The Community Based Resource Coordinator (CBRC) will serve as a community liaison for partnering agencies.
The CBRC can explain the nature, goals and policies of Gwens Girls services and programs. Will be able to assess which program will best fit the callers needs and provide them with the necessary resources. In addition, will work closely with community providers in the Diversion community to serve as a resource of the best services to meet the needs of referring families.
The CRBC is responsible for maintaining a robust partner network, with a particular focus on cultivating relationships with direct service providers.

POSITION SUMMARY
During the COVID-19 pandemic these duties will require virtual and some in-person engagement with social distancing. The community connector works closely with the collaborative network to collectively support young people, families, and community partners, and works to:
Cultivate relationships across diverse communities and connect young people and families to resources and opportunities around housing, education, employment, financial capability, health/mental health, social capital, and permanence that are responsive to their cultures and identities.

WORK ENVIRONMENT

What you will be doing to make a difference:
· Be part of a dedicated and committed team including Clinician's, Family Partners, and other behavioral health providers
· Work in tandem with the Clinician to collect information to complete comprehensive assessments
· Meet independently and with your team members with the youth and/or families to provide support, education, and resources.
· Support the Treatment Plan Goals
· Identify and connect caregiver(s) to formal and informal community resources
· Assist with referrals and resources

Requirements
QUALIFICATIONS (Education and Experience)
Bachelor's Degree or Associates degree in Human Services from an accredited university OR
intern enrolled in a master's degree program in the human services field
Trained to provide family members with therapeutic support for behavioral health needs
Experience with care coordination/targeted case management
Skills in client advocacy and conflict mediation
Excellent communication and organizational skills
Valid MA driver's license acceptable driving record, reliable transportation, and proof of insurance
Acceptable background record check as required by program

The Mental Health Counselor - Outreach Services will provide trauma-informed, wellness-focused support to individuals and families experiencing homelessness. Provides early intervention and care management services. Coordinates activities and events to promote community engagement and support homeless individuals and families to rebuild sustainable independence. This position will provide direct services to people of all ages, according to OCO's service delivery standards in diverse locations, including street-based, OCO offices, other agency offices, program sites, public places and the homes of consumers. Independently provides early intervention and care management services to homeless individuals. Carries out duties in accordance with Agency and Program policies and procedures.
Key Responsibilities
Provide crisis counseling, advocacy, and solution-focused support to individuals in crisis or with severe mental illness.
Assist consumers in accessing community services and monitor service provision.
Develop assessments and service plans using a strengths-based approach.
Facilitate life skills coaching, education, and support groups.
Promote positive relationships among participants, families, and communities.
Collaborate with community partners to implement joint protocols and ensure compliance.
Organize and participate in community-building and empowerment events.
Maintain accurate and timely documentation of services and outcomes.
Provide training to staff and partners on specialized topics.
Support direct service staff through casework guidance and problem-solving.
Represent the agency in community settings and lead program-specific initiatives.
Participate in on-call crisis intervention and staff support as needed.
JOB REQUIREMENTS
Must be able to communicate effectively orally and in writing.
Must be comfortable providing direct services on the street to homeless individuals, including counseling supports
Must be able to work independently and be self-motivated, resourceful, organized and adaptable in carrying out job duties.
Must have the demonstrated ability to work with others in a warm, non-judgmental manner and be a positive role model
Must be able to cooperatively plan with others and be creative in planning.
Must exhibit professionalism, good judgment and flexibility.
Must possess valid NYS Driver's license and have access to registered, reliable vehicle for travel and transporting clients.
Must have acceptable physical and mental health to carry out responsibilities of the position.
MINIMUM QUALIFICATIONS
LCSW, LMSW, LMHC, RN, or Licensed Psychologist (active NYS license) and one year experience working with homeless individuals.
