The Home Health Physical Therapist Assistant (PTA) performs treatment and provides exercise instruction and patient education aimed at improving/enhancing the patient's well-being while following the established plan of care.
Under the supervision of a Physical Therapist, is at all times responsible to follow all state specific laws governing the provision of physical therapy in home care, to follow the treatment set only as defined by the Physical Therapist; to perform only those procedures that he/she is qualified and trained to perform and to make all recommendations for changes in the plan of care to the supervising Physical Therapist.
Following the plan of care, instructs and aids patients in evidence based treatment within the scope of the physical therapy assistant and according to the state laws governing physical therapy assistants.
Observes, records, and reports to the supervising PT, the nurse supervisor, and/or the physician the patient's response to treatment and changes in the patient's condition. Coordinates care with the other members of the healthcare team as appropriate.
Instructs patient, family, caregiver, and other members of the health care team in the areas of therapy within the scope of the physical therapy assistant.
Visits patients within the entire service area of their respective agency and other nearby LHC agencies as needed
According to Plan of Treatment; complete a progress note for each visit and submits progress notes per LHC policy.
Schedule flexibility is needed in this role to ensure we provide care to the patient as needed. Requires the ability to participate in a weekend rotation and potential availability on observed holidays to ensure we are meeting the demands of our patients.
Participates in staff conferences and committees as requested by the agency.
Adheres to LHC Therapy Standards of Practice in provision of services to all patients.

License Requirements
• Current licensed, registered, or certified to assist in the practice of physical therapy in state of practice.

35% Create customized downloads from on-line Medicaid claims data warehouse as requested by investigators, attorneys, and multistate investigative and litigation teams.

20% Assists and supports Medicaid fraud investigators and attorneys in the compilation and analysis of investigative information and the development of damages models for use in Florida-specific and multistate health care fraud investigations and litigation. JOB DUTIES:
This is an environmental professional position that will serve as a point of contact for Binding Determinations (BDs) and program complaints and provide support to program initiatives within OAWP. * Assist the environmental administrator with technical, scientific and professional interpretation of information on groundwater modeling, surface-water modeling, or crop modeling that quantify the nutrient use efficiency and water conservation benefits achieved through the implementation of FDACS BMPs and cost-shared technologies.
* Provide technical support for producers and Agricultural Water Field Services staff regarding BMP and cost share implementation. The Division of Water Restoration Assistance is seeking an Environmental Consultant for the Clean Water State Revolving Fund Program. This position will be involved with implementing the State Revolving Fund Loan Program authorized under Title VI of the Clean Water Act, Section 1452 of the Safe Drinking Water Act, Chapter 403.1835, F.S., Chapter 403.8532, F.S., and the Small Community Wastewater Construction Grants program authorized under Chapter 403.1838, F.S. This position will primarily provide technical review and approval of asset management plans, facilities plans, plans & specifications, and construction oversight. The incumbent will be required to work 40-hour work weeks, Monday through Friday, and be present for work between the core hours of 9AM - 3PM on site at your workstation.
The advanced environmental position involves project management work and providing support for the Clean Water State Revolving Fund project managers to assure compliance with Title VI of the Clean Water Act, Section 1452 of the Safe Drinking Water Act, Chapter 403.1835, F.S., Chapter 403.8532 F.S., and the Small Community Wastewater Grants program authorized under Chapter 403.1835 F.S. This is advanced-level work where the employee performs and is responsible for the completion of clean water project management tasks under very limited supervision. The selected candidate will join a multidisciplinary team that implements Department policy, programmatic, technical, and regulatory responsibilities under several Everglades restoration efforts, including the Comprehensive Everglades Restoration Plan (CERP), Everglades Forever Act (EFA), and the Northern Everglades and Estuaries Protection Program (NEEPP). This position provides technical support to Everglades planning, permitting and compliance activities by evaluating, interpreting, and reporting water quality data specific to the Everglades ecosystem. This position requires frequent participation in intra- and interagency teams and working groups and is also involved in the implementation of the Innovative Technology Grants that identify methods to prevent, combat, and clean-up harmful algal blooms. This position reports to the Everglades Water Quality Technical Manager and has no supervisory responsibilities. The incumbent will be expected to work 30 to 40-hour work weeks, Monday through Friday, and be present for work between the core hours of 9AM - 3PM on site at your workstation.
This position provides technical support for planning, permitting and compliance activities, including but not limited to generating the daily South Florida system updates, reviewing permit application materials and water quality monitoring plans, reviewing regulatory reports, and statistically analyzing and illustrating water quality, biological and hydrological data for the greater Everglades ecosystem. This position will assist in writing Chapter 3 of the annual South Florida Environmental Report. Other duties include participation in interagency meetings and working groups with Federal and State partners and local stakeholders, including the Restoration Coordination & Verification team (RECOVER) and Project Delivery Teams (PDT) for Everglades restoration projects. The position will also provide technical support for the Office of Water Policy and Ecosystems Restoration on the Everglades Technical Oversight Committee. Other responsibilities include supporting the program's quality assurance (QA) system and conducting technical reviews of Innovative Technology Grant deliverables, including Quality Assurance Project Plans (QAPPs).
