ESSENTIAL JOB FUNCTIONS
General housekeeping on a regular basis.
Sanitizing rooms with the use of a bleach solution or currently approved chemical and a steam cleaner when appropriate.
Assisting with laundry duties for both the Detox & Residential teams.
Cleaning the rooms, hallways and other public spaces at all facilities
Responding to accidents and custodial emergencies as needed.
Cleaning and sanitizing bathrooms.
Damp mopping floors, halls and stairwells
Spot cleaning walls, doors and partitions as needed.
Refilling bathroom dispensers as needed.
Periodic cleaning of the outside of the building including the sidewalk and windows with the use of a pressure washer and window cleaning equipment.
ADDITIONAL JOB RESPONSIBILITIES
Helping the maintenance workers with various tasks that may include moving furniture, light repair work to the facility, painting, use of hand tools that could include drills, saws, hammers, screwdrivers, etc.
Additional tasks and duties, as assigned.
SUPERVISORY FUNCTIONS
This position has no supervisory responsibilities.

The Medical Director works closely with the Chief Medical Officer to provide vision for and implementation of medical programs and services across Fora Health's Departments. This team, in turn, collaborates with Fora-wide leadership, representing other programs in the continuum of care to support Fora Health's mission of providing excellent care for those with substance use disorders.
The Medical Director will provide direct patient care in 3.7 level withdrawal management and medically managed residential programs. Additionally, the MD will direct expansion of both outpatient and residential MAT services. The MD will also direct the development of expanded medical services for patients enrolled in Fora Residential and Outpatient SUD programs. The MD will collaborate with Operations & Admissions leadership to develop and strengthen rapid access pathways from local hospitals into appropriate levels of care at Fora.
The Medical Director will join the Chief Medical Officer in nurturing a program culture that centers excellent medical care within a framework of loving kindness, harm reduction, and trauma informed care.
Expected allocation of time is approximately 60% administrative and 40% clinical.
Essential Job Duties
Completing medical intakes on patients admitting to withdrawal management and residential treatment
Formulating treatment plans for those patients and communicating with nurses and case managers as necessary regarding implementation of medical orders and ongoing assessment of patients
Reviewing medical referrals to Medically Managed Residential Program and communicating with referents, including hospitals and outpatient settings.
Continuing expansion of MMR program to include patients on IV antibiotics, with complex wound care needs, and those newly dependent on insulin
Developing curricula & structure for residential medical groups to address PAWS, tobacco cessation, and MAT for AUD
Providing clinical education to nurses
Sharing overnight phone call with Chief Medical Officer and advance practice providers, typically one week per month.
Supervising advanced practice providers
Supervising and mentoring medical learners throughout their rotations at Fora
Providing education to patients about medications and treatment options.
Collaborating with Access Department to develop and expand relationships with community partners and pathways into Fora medical services.
Collaborating with Chief Medical Officer on development and implementation of new programs
Collaborating with Chief Medical Officer & Medical Operations Director on writing and revision of policies & procedures, creating & maintaining EMR order sets, addressing grievances & incident reports.

ESSENTIAL JOB FUNCTIONS
Clinical Evaluation, Treatment Planning, and Documentation
Gather data systematically from the client and other available collateral sources. Obtain and interpret all relevant assessment information.
Determine the client's appropriate level of care and stage of change.
Ability to utilize the DSM-5 to assess and diagnose substance use and mental health disorder at time of enrollment.
Formulate with the client mutually agreed upon and measurable treatment outcome statements for each need. Identify appropriate strategies for each outcome.

ASAM treatment plan reviews are completed and reviewed within 30 days of admit and every 30 days thereafter. Submit treatment plans to supervisor as required and posted in a timely fashion in order to obtain continuing authorization for clients.
Progress toward goals are updated bi-monthly and communicated during with both internal and external treatment providers.
A Discharge Plan is developed with the client 30 days prior to a planned discharge, and continuing care plan is completed with client in final session. When required by contract, County exit surveys are completed within the last 5 days prior to the planned discharge.
A Discharge Summary is completed within 5 days following discharge. When required, insurance is notified within 24 hours of discharge.
Counseling and Education
Establish a helping relationship with the client characterized by warmth, respect, genuineness, concreteness, and empathy.
Facilitate the client's engagement in the treatment and recovery process. Role models and coordinates with residentials counselors on effective milieu therapy interventions.
Provide individual and group counseling as a part of the therapeutic process to achieve treatment goals. Supports family therapy by reinforcing goals in individual and group sessions.
Sensitize others to issues of cultural identity, ethnic background, age, and sexual orientation in treatment and recovery.
Describe the health and behavior problems related to substance use, including transmission and prevention of HIV/AIDS, TB, STDs, and other infectious diseases.
Teach life skills, including but not limited to, stress management, relaxation, communication, and refusal skills.
Provide therapeutic interventions to address treatment-interfering behaviors as needed.
Meet individually with clients on caseload with the frequency determined on treatment plan.
Referral
Arrange or assist clients in following through with referrals to other professionals, agencies, community programs, or other appropriate resources to meet client needs.
Explain in clear language to the client the necessity for and process of referral.
Exchange relevant information with the agency or professional to whom the referral is being made. Facilitate the client's transition to continuing care resources prior to his/her discharge from treatment.

The Medical Director reports to the Chief Medical Officer.
The Medical Director works interdependently as a member of a program clinical team and works collaboratively with other agency staff.

Qualifications
Education and Experience
Must be a licensed MD or DO
Must be board certified in Addiction Medicine; Fellowship training strongly preferred
Experience in prescribing buprenorphine required
At least one year of attending experience in primary care, hospital medicine or urgent care required
Experience in withdrawal management strongly preferred
Supervising experience is a plus
Other Requirements
For employees with a history of substance use disorder, Fora Health's policy requires certification of a minimum of two years of sobriety for this position.
Ability to pass a pre-employment or for cause drug tests.
Position will require some amount of regular connection to work via cell phone to answer questions and respond to internal and external contacts.
