What You'll Do
As an Admissions Advisor, you'll play a key role in guiding new patients through the beginning of their Forum Health journey. After patients receive their lab results and a video explanation from one of our team members, you'll step in to bring the next steps to life. With a blend of empathy and clarity, you'll ensure they understand their care plan and feel supported as they decide how to move forward.
Connect with prospective patients virtually to review a recorded lab reading video of their test results
Lead, coach, and mentor chronically ill patients into a life-changing clinical model
Explain the recommended action plan clearly and confidently
Present payment options, including third-party credit or in-house payment plans to help patients make informed decisions
What You Bring
Associates degree required, Bachelors degree preferred
Strong communication skills and a genuine passion for helping others
Ability to explain payment options in simple, encouraging terms
Background in sales, case management in a healthcare setting, new patient enrollment, or customer service is a plus

POSITION SUMMARY

The Quality & Licensing Manager is a leadership position serving a key role in facilitating activities related to quality of care, licensing and accreditation, service delivery and contracting within all Fora Health locations. The position collaborates closely with Quality Management staff, the Executive Team, operational leaders, and other staff to create and support a service environment that empowers patients to recover. Quality & Licensing Manager oversees the licensing and accreditation process in a manner which secures continuous certification of program services and endorsement from Commission on Accreditation of Rehabilitation Facilities (CARF), and leads the development of appropriate metrics for success.

ESSENTIAL JOB FUNCTIONS
Provides direct administrative supervision and oversight to the Quality Specialist team.
Works to ensure the utilization of clinical data to support the identification of quality issues and improvement opportunities, drive prioritization and decision making and monitor improvement activities. Such data may include:

Develops annual quality improvement plan in collaboration with the Director of Operations & Compliance and in accordance with the organization's strategic plans, CARF, and the Oregon Administrative Rules standards.

Oversees patients' rights and grievance procedure.
Oversees incident reports and serves as a member of Health & Safety Committee.

Leads data collection, analysis and reporting for established quality/patient safety practices.
Creates, modifies, or edits policy and procedures in collaboration with the Director of Operations & Compliance.
Plans and advocates for the implementation of Continuous Quality Improvement principles in the operation of the organization.
In collaboration with the Chief Clinical Officer, develops quality indicators and other meaningful outcome measures; develops procedures for related data collection through planned program reviews and reporting based on state/county requirements, contract requirements and CARF standards if applicable.
Collaborates with appropriate staff to ensure that identified opportunities for enhancement are implemented, monitored, completed and evaluated.
Collects data to evaluate the quality of support and services, including the areas of patient records, utilization management, and medication services. Reports pertinent information to appropriate individual(s) and/or committee(s). Recommends areas for development and, when appropriate, evaluates the results of those areas with the Chief Executive Officer.
Conducts Quality of Care reviews in each clinical department to assure that all departments are delivering services according to P&P, philosophy, treatment model and practices as well as the values of the organization.
Annually and ongoing as needed, evaluates the operation of Fora Health's Quality Management Program as it relates to policies and procedures, new implementations and state regulations in collaboration with the Director of Operations & Compliance and the Executive Team.
Develop Annual QI Report focusing on improvement of “high risk, high volume, high cost” areas of concern.
As requested/needed, communicates Quality Management activities and findings to the CEO and Board of Directors.
Will be required to travel to multiple locations and participate in community-based quality management meetings.
Perform other related duties as required or assigned.
Oversee licensing and accreditation process in a manner which secures continuous certification of program services and endorsement from Commission on Accreditation of Rehabilitation Facilities (CARF).
Acts as point of contact and liaison for licenses, accreditations, and panels.
Prepares for and hosts site reviews, ensuring agency receives positive results and minimal findings.
Assures that CARF, State and County MH and Addiction Services corrective action plans are completed, implemented and that regulatory agencies are notified within prescribed time frames.
SUPERVISORY FUNCTIONS
Provides administrative supervision and oversight to the Quality Specialist team.
QUALIFICATIONS
EDUCATION AND EXPERIENCE REQUIRED or PREFERRED
Minimum four (4) years of experience working with multi-disciplinary work groups, including clinical staff, focused on service delivery and/or quality improvement activities in a hospital or health care organization.
At least two years of this experience must include responsibilities as a leader of a team, work group or program.
3 years supervisory or management experience required.
Possession of a Bachelor's Degree from an accredited college or university, preferably in a health or human service field such as hospital administration, public health, allied health or related area.
Possession of a Master's Degree from an accredited college or university, preferably in a health or human service field, may be substituted for two years of experience.
Experience gathering, organizing, analyzing, and interpreting aggregate data and using the data to identify problems and make recommendations.
Experience authoring written documents or reports in areas related to service delivery and/or quality assurance activities.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED
Process improvement skills. Strong knowledge and understanding of the concepts of Continuous Quality Improvement (e.g., Six Sigma; Lean) and related regulatory requirements.
Strong analytical and problem-solving skills; knowledge of regulatory and licensure standards, risk management and safety desirable. Project management experience with process measurement, clinical data collection, process change and working knowledge of regulatory and value-based performance requirements strongly preferred.
Ability to facilitate training sessions. Experience is preferred.
Knowledge, experience and proficiency with personal computer software including word processing, spreadsheets, statistical analysis, graphics and Outlook.
Schedule may include extended hours including evenings and weekends on occasion.
Valid driver's license, current auto insurance and clean driving record for past 3 years

POSITION SUMMARY
The role of the PSA/PIC is to coordinate all aspects of patient fiscal responsibility to Fora Health Treatment and Recovery while providing high level customer service and support. The PSA/PIC treats each patient that walks into our facility with the same respect while seeking equitable solutions for patient insurance coverage. The PSA/PIC determines coverage and coverage needs by assisting with financial resources and other support to ensure our patients receive the quality care they need.
ESSENTIAL JOB FUNCTIONS
Patient Support Associate
Greets patients and all other visitors directly, routes visitors to correct location, checks patients in for appointments, notifies other staff and urinalysis vendor of patient arrival.
Scan paperwork from community referrals and track reporting deadlines in spreadsheet and prepare report for site manager.
Controls visitor passes, confidentiality procedures and, when appropriate, serves as the gatekeeper to limit access to the building, patients or staff.
Answers phones, returns calls, schedules appointments, takes messages, when necessary, and handles printing, copying, scanning and faxing documents.
Collects payments for services.
Monitors lobby and lobby restroom key to ensure safety and tidiness for patients, visitors and staff.
Supports intake process for outpatient and residential services by confirming the nature of the intake, explaining intake process, and working cooperatively with the patient intake team and admissions team to complete enrollment with patients.
Prepares paperwork in EHR and assists patients in completing necessary documentation.

Patient Intake Coordinator
Maintain the integrity and accuracy of the day to day patient information.

Work with billing to assist patients and their families to resolve financial obligations and changes in eligibility status.
Be the primary contact between clinical staff and Billing.
Work closely with management and staff to develop and maintain a working relationship that enhances the two primary goals of revenue expansion and quality control.
Tasks are to be completed with little follow-up from supervisor. Support is provided in the form of teamwork and weekly team meetings.
Meet with all patients to determine their eligibility for program services either prior to admission or within 24 hours following admission.
Ensure all patients provide necessary documentation regarding eligibility for benefits.

Coordinate financial arrangements between patients and insurance company for patients who have benefits other than OHP and for any patient who elects to pay for services and/or who has an insurance plan that requires co-pay or other financial responsibilities on the part of the patient.

POSITION SUMMARY
The Director of Quality Management is a leadership position responsible for facilitating activities related to quality of care, licensing and accreditation, service delivery, and contracting within all Fora Health locations.

POSITION SUMMARY
The NOC Shift Supervisor is responsible for ensuring effective overnight operations for the 3.7/Withdrawal Management Unit. This leadership role also mentors & supports overnight nursing/tech staff in providing Fora's radically kind, compassionate, high quality medical care on a 24 hour basis and helps the night team feel an integral part of the larger organizational mission. The NOC Shift Supervisor works under the direction of the Nurse Manager and collaborates closely with a dynamic interdisciplinary team, including 3.7 Residential nurses and case coordinators, WM and MAT case coordinators, Peer Recovery Mentors, Licensed Medical Providers and IT staff. The NOC Shift Supervisor leads the night nursing team's seamless integration with daytime operations, forging a crucial link in Fora's 24/7 Medical Services Model.
ESSENTIAL JOB DUTIES
Provides initial training & onboarding to new NOC nurses, including technology setup support, education on Fora medication protocols, Fora operational work flows, EMR documentation, and basic addiction medicine nursing assessment & treatment skills.

Clinical Duties
Assist nurses with care of patients withdrawing from drugs and alcohol, and may include nighttime shifts.
Assist the medical provider with the triage, admission, and discharge process of patients from withdrawal management services.
Assist the medical provider, patient and residential staff with the transition of withdrawal management patients to other programs.
Tracks potential withdrawal management patients and ensures consistent, quick screening and documentation.
Performs thorough searches of patients' belongings and inventories belongings accurately.

We currently have an opening for a full-time Dual Diagnosis Counselor. This position provides professional, compassionate treatment for adults with mental health and co-occurring substance use disorders. Duties and responsibilities include: treatment planning for dual diagnosis, providing mental health assessments and diagnoses and individual and group counseling.
The Dual Diagnosis Counselor works closely with other programs in transitioning patients to our outpatient program including residential and medication supported treatment/recovery. Helps to foster development of staff's knowledge of mental health conditions. Counselors have clear and manageable caseloads with clear expectations of service requirements. Additionally, candidates must be proficient and timely with required productivity and clinical documentation standards.
ESSENTIAL JOB FUNCTIONS
1. Clinical Evaluation, Treatment Planning, and Documentation
Gather data systematically from the client and other available collateral sources. Obtain and interpret all relevant assessment information.
Determine the client's appropriate level of care and stage of change.
Ability to utilize the DSM-5 to assess and diagnose substance use and mental health disorder at time of enrollment.
Formulate with the client mutually agreed upon and measurable treatment outcome statements for each need. Identify appropriate strategies for each outcome.
Inform client of confidentiality rights, program procedures that safeguard them, and the exceptions imposed by regulations. Protect client rights to privacy and confidentiality in the communications of client information.
Assess treatment and recovery progress and, in consultation with the client and significant others, make appropriate changes to the treatment plan to ensure progress toward treatment goals at regular intervals and/or when indicated by changing circumstances.
Describe and document treatment process, progress, competencies, and outcome relative to treatment goals and objectives. Apply placement, continued stay, and discharge criteria.
Conduct and document continuing care, relapse prevention, and discharge planning with the client and involved significant others.
Work collaboratively with all service areas within the agency to provide appropriate client referrals and communicate relevant client information as needed.
Communicate and collaborate with referral agencies regarding scheduled screenings, status of client, and treatment and discharge planning.
Contact referents within 24 hours of admit, at least once per month during the client's treatment, and before and during the client's discharge. Document these contacts in progress notes.
Progress notes are documented in client charts within 24 hours for individual treatment.
ASAM treatment plan reviews are completed and reviewed within 30 days of admit and every 30 days thereafter. Submit treatment plans to supervisor as required and posted in a timely fashion in order to obtain continuing authorization for clients.
Progress toward goals are updated bi-monthly and communicated during with both internal and external treatment providers.
A Discharge Plan is developed with the client 30 days prior to a planned discharge, and continuing care plan is completed with client in final session. When required by contract, County exit surveys are completed within the last 5 days prior to the planned discharge.
A Discharge Summary is completed within 5 days following discharge. When required, insurance is notified within 24 hours of discharge.
2. Counseling and Education
Establish a helping relationship with the client characterized by warmth, respect, genuineness, concreteness, and empathy.
Facilitate the client's engagement in the treatment and recovery process. Role models and coordinates with residentials counselors on effective milieu therapy interventions.
Provide individual and group counseling as a part of the therapeutic process to achieve treatment goals. Supports family therapy by reinforcing goals in individual and group sessions.
Sensitize others to issues of cultural identity, ethnic background, age, and sexual orientation in treatment and recovery.
Describe the health and behavior problems related to substance use, including transmission and prevention of HIV/AIDS, TB, STDs, and other infectious diseases.
Teach life skills, including but not limited to, stress management, relaxation, communication, and refusal skills.
Provide therapeutic interventions to address treatment-interfering behaviors as needed.
Meet individually with clients on caseload with the frequency determined on treatment plan.
3. Referral
Arrange or assist clients in following through with referrals to other professionals, agencies, community programs, or other appropriate resources to meet client needs.
Explain in clear language to the client the necessity for and process of referral.
Exchange relevant information with the agency or professional to whom the referral is being made. Facilitate the client's transition to continuing care resources prior to his/her discharge from treatment.

Clinical Coordination:
Cares for patients in the withdrawal management program according to staffing ratios determined by the Oregon Administrative Rules.

Reviews newly admitted patients' medical histories and obtains additional medical information as necessary.

The Withdrawal Management Nurse provides nursing support, assessment and care for patients withdrawing from drugs and alcohol. This position is the first line support for patients, staff, Nurse Practitioners and the Medical Director on service specific clinical issues.

ESSENTIAL JOB FUNCTIONS
Clinical Coordination:
Cares for patients in the withdrawal management program according to staffing ratios determined by the Oregon Administrative Rules.
Collaborates and communicates issues/concerns with the treatment team on clinical and non-clinical issues.
Supports admission & medical staff in screenings for potential admissions for medical appropriateness.
Reviews newly admitted patients' medical histories and obtains additional medical information as necessary.
Reviews risk assessments and provides further evaluation and referral as needed.
Completes comprehensive nursing assessment as medically indicated on newly admitted patients.
Provide on-going, accurate assessment, including: obtaining & monitoring vital signs, drawing blood, urinalysis, performing CIWA-Ar & COWS scales, and performing other physical and social assessments as appropriate and necessary.
Screens ongoing patients' medical symptoms or concerns to determine level of medical intervention needed.
Provides nursing interventions as appropriate with available facilities, following medical policies, procedure & protocols safely & accurately.
Review medication and orders and enter prescription orders safely and accurately.
Administers medications as indicated by LMP ordered protocols and orders in a safe manner.
Evaluate diagnostic tests to identify and assess patient's condition and notify provider of any abnormalities.
Maintain up to date documentation in patient's hard chart.
Maintain accurate controlled medication storage, disposal and count as indicated by policy.
Educates patients on general health guidelines and preventative measures to improve over-all health and decrease risk factors, including continuum of care in A&D treatment and withdrawal symptoms.
Arrange or assist patients in following through with referrals to other appropriate resources to meet medical needs and exchange relevant information with the agency or professional to whom the referral is being made.
Collaborate with outside providers and facilities as needed in order to optimize continuum of care.
Documents assessment, planning, implementation and evaluation in the patient record.
Documentation is thorough, timely and in accordance with industry standards. Documentation reflects objective/subjective data, nursing interventions and patient's response to treatment. Notes provider orders accurately and in a timely manner.
Provides care based on the best evidence available. Interacts and participates in the education and orientation of medical support staff, withdrawal management nurses, patients and staff. Promotes/supports growth of others through precepting and mentoring when appropriate.
Role model healthy styles of communication and behavior, by using non-violent communication, verbal de-escalation, and demonstrating appropriate physical and emotional boundaries. Maintain professional boundaries with the patients at all times.
Regularly attend medical withdrawal management staff meetings.
Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to Fora policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access.

WORKING CONDITIONS
This position generally works in an indoor office environment with periodic travel between sites or to special events.

POSITION SUMMARY
Fora Health is seeking a full-time Addictions Counselor (AC) for its Hillsboro-based outpatient substance use disorder treatment program. The AC will be a core member of a multi-disciplinary team including access to medications to treat addiction. The site has a particular focus on improving access to care for the Latino/a/x community. The AC will be expected to work closely with the entire Hillsboro team to provide evidence-based behavioral health interventions to all Hillsboro patients. The AC should also have a robust understanding of trauma-informed care, motivational interviewing, medication-supported recovery, and harm-reduction.
PAY
This is a full-time position (40 hours per week) earning an hourly wage of $27.48-30.36 (plus a 5% pay differential for employees with demonstrated fluency in Spanish/English language). This position is non-exempt and eligible for overtime compensation.

ESSENTIAL JOB DUTIES
Clinical Evaluation, Treatment Planning, Insurance Authorizations, Course of Treatment, Collaboration, Transition, and Documentation
Documentation to be completed within 24 hours of service performed.
Inform patients of confidentiality rights, program procedures that safeguard them, and the exceptions imposed by regulations. Protect patient rights to privacy and confidentially in the communications of patient information.
Gather data systematically from the patient and other available collateral sources. Obtain and interpret all relevant assessment information.
Continually determine the patient's readiness for treatment and change, using the Stage of Change model as a guide for interventions throughout the treatment process.
Continuous treatment planning: formulate mutually agreed upon and measurable treatment outcome statements for each need identified in the assessment. Identify appropriate strategies to reach each outcome desired. Track and document patient progress towards intended outcomes. Initial treatment plans are completed at intake, individualized within 30 days of intake, and updated on a regular basis, indicating patient engagement and progress in treatment.
Assess treatment and recovery progress, and through consultation with the patient, significant others, and medical providers, make appropriate changes to the treatment plan to ensure progress toward treatment goals at regular intervals and/or when indicated by changing circumstances.
Describe and document treatment process, progress, and outcome in relation to the treatment goals in progress notes, ASAM reviews (such as for re-authorizations or referral to residential), screenings related to enrollment in specific programs such as medication as treatment, and transfer summary. Apply placement, continued stay, and completion/transition criteria based on medical necessity and other factors such as patient readiness and engagement in the program.
Conduct and document continuing care, relapse prevention, and completion/transition planning with the patient and involved significant others.
In order to justify continued care and document medical necessity, ASAM assessments, and other screenings are completed for reauthorization and progress in specific programs.
Progress Reports are completed electronically, faxed or emailed to referents and other appropriate entities every 30 days and when requested by referent, specifically within the DUII program within 14 days of a urinalysis positive for a controlled substance. When sent electronically, an additional note is added to the report to document the electronic method of sending.
Communicate and collaborate with referral agencies regarding scheduled screenings, status of patient, and treatment and completion/transition planning. Develop a report with referents. Establish and maintain partnerships in the community that work to provide increased access to culturally specific communities, especially the Latinx community. Engage in attending community events along with the Program Supervisor to increase public awareness of our services.
Provide and document service coordination activities, outreach and re-engagement activities.
At least three re-engagement efforts must be documented over a 1-2-week time frame when a patient is disengaged.
Charts are to be closed prior to or on the 30
th
day of non-engagement in both individual and group services.
Support patient engagement in outside community support membership/activities.
Transfer Summary is sent to referents within 5 days of patient discharge when appropriate.
Counseling and Education
Establish a helping relationship with the patient characterized by warmth, respect, genuineness, concreteness, and compassion. Motivational Interviewing is the foundational approach expected of all outpatient counselors.
Facilitate the patient's engagement in the treatment and recovery process.
Provide individual and group counseling as a part of the therapeutic process to achieve treatment goals. Individualize these services throughout the episode as indicated.
Sensitize others to issues of cultural identity, ethnic background, age, and gender in treatment and recovery with special focus of having an understanding of Latinx values, family roles and community supports. Be able to explain and support abstinence based, medication assisted and/or harm reduction models of treatment with patients. Help to reduce stigma for the individual through increased understanding of both use disorders and the healthcare system in general.
Be able to describe the health and behavior problems related to substance use, including transmission and prevention of HIV/AIDS, TB, STDs, and other infectious diseases. Provide referrals for above as needed.
Be able to teach life skills, including but not limited to, stress management, relaxation, communication, and refusal skills.

Patient Support Associate

Job Description
The Patient Care Assistant will be an integral member of an expanding, multidisciplinary medical team dedicated to the compassionate care of vulnerable individuals seeking to decrease their use of drugs or alcohol. This position performs all duties relevant to providing direct care for individuals receiving withdrawal management services within the guidelines of Fora Health's mission and policies & procedures, and will provide direct support to nurses as well as licensed independent providers.
ESSENTIAL JOB FUNCTIONS
Clinical Duties
Assist nurses with care of patients withdrawing from drugs and alcohol, and may include nighttime shifts.
Assist the medical provider with the triage, admission, and discharge process of patients from withdrawal management services.
Assist the medical provider, patient and residential staff with the transition of withdrawal management patients to other programs.
Tracks potential withdrawal management patients and ensures consistent, quick screening and documentation.
Performs thorough searches of patients' belongings and inventories belongings accurately.
Provide direct care for all patients to include monitoring for behavioral needs/changes and medical symptoms.
Collaborates and communicates issues/concerns with the treatment team on clinical and non-clinical issues.
Follows standard milieu intervention guidelines, holding patients accountable to the program structure.
Charting and record keeping as required by the policies and procedures.
Assist medical provider and nurses with medication and the medical needs of patients.
Monitoring urine drug screens.
Enters patient over the counter medication as needed medication accurately and efficiently in accordance with the electronic medical record.
Ensure patients have clean clothing and bedding.
Provide patients with access to fluids, snacks and meals.
Assist with inventory, routine maintenance and storage of medical supplies and equipment.

The Withdrawal Management Nurse provides nursing support, assessment and care for the dual diagnosis and withdrawal management programs at Adult and Youth and is the first line support for patients, staff, Nurse Practitioners and the Medical Director on service specific clinical issues.
ESSENTIAL JOB FUNCTIONS
Clinical Coordination:
Cares for patients in the withdrawal management program according to staffing ratios determined by the Oregon Administrative Rules.
Collaborates and communicates issues/concerns with the treatment team on clinical and non-clinical issues.
Supports admission & medical staff in screenings for potential admissions for medical appropriateness.
Reviews newly admitted patients' medical histories and obtains additional medical information as necessary.
Reviews risk assessments and provides further evaluation and referral as needed.
Completes comprehensive nursing assessment as medically indicated on newly admitted patients.
Provide on-going, accurate assessment, including: obtaining & monitoring vital signs, drawing blood, urinalysis, performing CIWA-Ar & COWS scales, and performing other physical and social assessments as appropriate and necessary.
Screens ongoing patients' medical symptoms or concerns to determine level of medical intervention needed.
Provides nursing interventions as appropriate with available facilities, following medical policies, procedure & protocols safely & accurately.
Review medication and orders and enter prescription orders safely and accurately.
Administers medications as indicated by LMP ordered protocols and orders in a safe manner.
Evaluate diagnostic tests to identify and assess patient's condition and notify provider of any abnormalities.
Maintain up to date documentation in patient's hard chart.

Educates patients on general health guidelines and preventative measures to improve over-all health and decrease risk factors, including continuum of care in A&D treatment and withdrawal symptoms.
Arrange or assist patients in following through with referrals to other appropriate resources to meet medical needs and exchange relevant information with the agency or professional to whom the referral is being made.
Collaborate with outside providers and facilities as needed in order to optimize continuum of care.
Documents assessment, planning, implementation and evaluation in the patient record.
Documentation is thorough, timely and in accordance with industry standards. Documentation reflects objective/subjective data, nursing interventions and patient's response to treatment. Notes provider orders accurately and in a timely manner.
Provides care based on the best evidence available. Interacts and participates in the education and orientation of medical support staff, withdrawal management nurses, patients and staff. Promotes/supports growth of others through precepting and mentoring when appropriate.
