What You'll Do
* Travel daily to provide in-home, community-based services to families and youth in caseload.
* Complete referral reviews and intake assignments. Communicate service availability to all assigned clients.
* Provide instruction, intervention, and assistance as deemed necessary through the exercise of independent judgment and knowledge or as part of the treatment team to preserve the family unit.
* Coordinate with county agency as appropriate and attend family sessions to promote successful clinical interventions with foster youth.
* Build and maintain relationships with families and external casemanagers.
* Coordinate and monitor service provisions for youth and family in the community and with school officials, courts and other appropriate parties involved with the client. This may include but is not limited to mental health, medical, educational, psychological, vocational and social services.
* Develop and ensure completion of appropriate assessments including but not limited to Individualized Treatment Plans, Psychosocial Diagnostic Assessments (PSDs), and Mental Health Services Plans according to established time frames involving all appropriate parties as required by policy and regulations of the state and/or contract.
* Timely document outcome of home visits, support sessions, safety assessments, interaction with parties involved in the youth's care and incident reports as required by policy, regulations, and contract requirements.
* Ensure foster parents, youth and casemanager needs are met during periods of staff transition.
* May require rotating on-call schedule.
* Participate in performance quality improvement process and execute any assigned tasks.
* Successful completion of all required training pertaining to job.
* Establish and maintain a positive working relationship, sensitive and responsive to the cultural differences with all levels of SAFY staff, clients, families and vendors.
* Ability to work nontraditional hours daily/weekly to fulfill position duties; may include nights and/or weekends.

* May complete Rules Investigations of all foster homes assigned by Lead LS or TD.
* Completion of the Large Family Assessment (the state required assessment if a Foster to Adopt home has 5 or more youth).
* Collect all documentation pertaining to licensing of applicants & Foster Parents.
* Other duties or special projects as assigned.

Qualifications

The Financial CaseManager's primary responsibility is to provide financial counseling to patients and families, including insurance benefit education, responding to financial inquiries, and collecting and processing co-pays and deductibles in accordance with ARS policies and procedures. In addition, the role supports CaseManagement functions by ensuring compliance with State and Federal guidelines, participating in discharge planning, completing discharge needs assessments, coordinating aftercare services and appointments, and collaborating with the treatment team, payors, and facility leadership to support continuity of care.
Works effectively with the facility leadership team to ensure success of the facility by completing the following:
Core Job Duties:
Serve as the primary financial counselor for patients, providing education on insurance benefits, financial responsibility, billing policies, and payment options.
Verify and validate patient insurance benefits and financial responsibility by first reviewing the UR Daily Census column to assess the daily status of insurance coverage, followed by checks in approved payer portals (e.g., InstaMed, NaviNet, Availity, or other designated systems), and conducting live payor calls as needed for inactive, unclear, or unresolved coverage.
Collect private pay fees, co-pays, and insurance deductibles within 72 hours of admission for inpatient and outpatient clients, in accordance with the “Collection of Patient Responsibility” policy.
Ensure completion and signature of all required financial and admission-related documentation within 72 hours of admission, including but not limited to billing acknowledgments, payment plans, advance repayment agreements, coordination of benefits, authorized claims representative forms, and the initial CaseManagement Discharge Plan (CMDP).
Facilitate payment arrangements and advance repayment agreements when co-pays or deductibles are not immediately collectible and notify leadership as needed.
Collaborate with the Admissions/RCM team to resolve collection barriers and secure (at minimum) agreed-upon payments at the time of admission.
Maintain accurate, timely documentation of all financial transactions, co-pay and deductible activity, and payment arrangements within the electronic medical record.
Work closely with Facility Leadership (Site CEO) and Aftercare Manager to ensure consistent financial processes and patient support.
Maintain open communication with the multidisciplinary treatment team regarding financial considerations that may impact treatment engagement or discharge planning.
Requirements
Bachelors' Degree in health-related field, Finance/Accounting or Medical Management office experience preferred. Minimum high school diploma.
Minimum one- or two-years' experience, preferred experience in the medical, behavioral healthcare or financial field.
Familiar with community resources and proficient in providing, discussing, and resolving financial issues and policies.

* Works with clients on a regular and ongoing basis to develop and maintains an on-going plan of action creating individual goals by assessing client strengths and needs, client responsibilities, and action steps to help client in achieving their goals. This includes provides budgeting and other planning assistance toward goal attainment such as assisting clients in the development of job and life skills as needed, how to maintain their apartment, pay bills on time, apply for benefits, develop a budget and how to keep a job, etc.
* Makes appropriate client referrals and linkages to community resources and support systems as needed and assists clients in securing stable housing and income. Acts as a liaison and link for clients with outside community resources.
* Conducts intake interview to determine program eligibility from walk in and referrals. Maintains waiting list as appropriate and makes referrals to other organizations as appropriate. Completes required paperwork, communicating boundaries, rules, services and limitations of the program.
* Conducts outreach efforts to external partners and organizations providing education about programs and services available to ensure maximum access and utilization by eligible participants. Solicit, collect and organize area donations from the community.
* Stays up to date with community resources available to clients.
* Assists clients as needed with transportation to move into housing, grocery shopping, etc, by accessing Lancaster Transit and other options when possible.
* Establishes and maintains file, ensures timely, accurate, and complete case notes and documentation and statistics as required for the program including detailed documentation of meeting dates, referrals, casemanagement hours, etc.
* Keeps a close eye on direct client assistance to conform to budget. Keeps accurate mileage documentation.
Requirements for a CaseManager- Outreach Program with Lutheran Social Services:
* Requires a minimum of an Associates Degree in Social Work or related field. Bachelor's degree preferred
* 2-4 years of casemanagement and/or crisis intervention experience.

Overview
Under general direction, is responsible for professional correctional counseling work of average difficulty and related work as required. * Verifies that offender sentencing complies with judgment order.

POSITION SUMMARY
Community Health and Wellness Partners (CHWP) is an integrated primary care community health center that utilizes a team-based care model, sharing patient-care responsibilities among members of a team. The Team CareCaseManager provides leadership by maintaining an excellent attitude and utilizing leadership by example. As part of this model, the Team CareCaseManager provides relevant interventions under the guidance and supervision of the Assistant Behavioral Health Medical Officer and Behavioral Health Manager and is the primary interface for patients to access community resource services. Responsible for the coordination of ancillary services, community resources, screening for chronic conditions and resource needs, providing brief intervention, referral to appropriate treatment and services, and documentation of care provided. WORK ENVIRONMENT
The work environment characteristics are those for a normal community health center environment (ambulatory primary care office). Work hours
Full-time, non-exempt hourly position. Generally, a 40-hour work week is required for full-time; part-time as agreed upon by parties; hours are subject to change. Office hours vary by site Monday through Friday. Some Saturdays and after business hours work may be required. Assigned meetings and travel are a part of the position. Remote work /Telecommuting may be available on a limited basis at the discretion of the Integrated Health Coordinator or Manager. Starting wages for Team CareCaseManager will be negotiable based on Community Health & Wellness Partners pay policies and factors such as education and experience, national and state average compensation recommendations based on location and region.
