The Optometrist is responsible for conducting comprehensive eye exams, diagnosing and managing ocular diseases, prescribing corrective lenses, and collaborating with the broader healthcare team to ensure continuity of care. This role combines clinical excellence with community engagement-bringing essential vision services to schools, rural areas, and underserved populations. Essential Duties and Responsibilities Clinical Care: Perform comprehensive eye examinations and visual assessments for patients of all ages. Diagnose and manage ocular diseases such as glaucoma, diabetic retinopathy, macular degeneration, and infections. Prescribe eyeglasses, contact lenses, and other optical aids as appropriate. Provide patient education regarding eye health, preventive care, and treatment options. Refer patients for advanced ophthalmologic or medical care as clinically indicated. Maintain accurate, thorough, and timely documentation in the Electronic Health Record (EHR). Mobile Clinic and Outreach Operations: Provide care aboard the Mobile Eye Clinic and at designated community sites, schools, and partner organizations. Work collaboratively with the Driver/PSR and Optician to ensure efficient clinic setup, patient flow, and safety. Participate in scheduling coordination, equipment setup, and breakdown as needed. Ensure all mobile clinic operations comply with regulatory, infection control, and safety standards. Support community vision screening and outreach events that promote access to care. Team Collaboration and Leadership: Collaborate with primary care providers and other disciplines to support integrated patient care within the FQHC model. Mentor optometry students during clinical rotations, providing guidance on patient care, examination techniques, and professional best practices. Serve as a clinical resource to optical and support staff, fostering a culture of teamwork and continuous learning. Participate in clinical quality improvement initiatives, training, and compliance activities. Provide feedback to administration regarding mobile operations, equipment needs, and community engagement opportunities. Qualifications Education and Experience: Doctor of Optometry (OD) degree from an accredited optometry program . Current, unrestricted license to practice Optometry in the State of Missouri required (or ability to obtain prior to start date). Minimum of one (1) year of experience in a clinical setting preferred. Experience providing care in a community health, FQHC, or mobile clinic environment preferred. Knowledge, Skills, and Abilities: Strong diagnostic and clinical skills with commitment to evidence-based care. Excellent interpersonal and communication skills, with a patient-centered approach. Performs in-depth studies of the quality and appropriateness of tests performed and reports findings of current issues to the quality improvement committee. Documents diagnosis or unsuitable findings and initiates appropriate deferral/referral process. Using the health team approach makes appropriate referrals to the Medical Director or other medical specialists. A detailed explanation of phlebotomy/apheresis concepts, risks and a demonstration or overview of the procedure is given before the informed consent is obtained. Performs full range of plateletpheresis and plasmapheresis procedures. Prepares collected units for processing, processes the units into red cells and plasma, labels all manufactured products, prepares products for storage/shipment, and distributes products as required. Inputs blood donor demographics as well as blood product information and test results into EBMS. Uses EBMS for tracking and management of blood products in all blood area systems. Performs in accordance with protocols and assists in maintaining conformity of staff with protocols. Performs follow-up care for donors displaying reactions. Contributes to blood drive set-up, teardown, canteen maintenance, quality control and record keeping. Continually monitors and manages complications associated with whole blood and apheresis phlebotomies to include venous access problems, bleeding, circulatory changes (vasovagal responses), mechanical hemolysis by apheresis machine, respiratory distress, allergic reactions, citrate toxicity (parasthesias), tetany, and other side effects that may progress to a medical emergency.     Job Description Clinical Quality Manager Department: Administration Immediate Supervisor: Chief Operating Officer (COO) Job Supervisory Responsibilities: Under the supervision of the Chief of Operations, the Clinical Quality Manager is responsible for the clinical quality delivery of patient care services through management of clinical quality indicators, quality improvement, and quality assurance. This role supports development and maintenance of procedures and workflows and collaborates daily with clinic leadership to support quality activities aligned with the health center's mission, vision, and values. Essential Functions/Responsibilities · Ensure ongoing compliance with applicable state and federal laws, licensing requirements, and external quality/recognition standards and network/payer quality program expectations. · Lead and manage day-to-day clinical quality measures, performance monitoring, and improvement workflows across medical, dental and behavioral health services, ensuring consistent implementation at all sites. · Maintain continuous readiness for external standards and renewal requirements (including PCMH/PCHH-related expectations as applicable) through workflow standardization, documentation oversight, internal review, and ongoing sustainment. · Ensure clinical quality performance measures and program outcomes are achieved; when goals are not met, document contributing factors and provide recommendations for program or workflow changes. · Drive effectiveness of organizational services through measurement and assessment of population health outcomes and clinical performance trends. · Lead clinical improvement efforts related to patient experience by using standardized survey results and qualitative feedback to identify, implement, and monitor improvements. · Partner with the Risk Manager to support clinical components of: Annual Risk Management Plan implementation and monitoring, FTCA-related clinical readiness activities, and incident reporting processes including review, trending, corrective actions, and follow-through. · Lead and coordinate the clinical education component tied to quality and safety programs; monitor progress and adjust training approaches based on clinic needs and performance trends in partnership with medical and dental training coordinators. · Manage the annual review and updating of the organization's Quality Improvement Plan and related clinical quality/patient safety workplans. · Maintain active participation and/or representation in external quality improvement activities/committees as assigned (e.g., MPCA QI Committee) in coordination with clinical leadership. · Support UDS performance measure reporting requirements and ensure accuracy and readiness for leadership review/approval. · Extract, validate, and analyze data from the EHR and other sources to produce accurate reports, presentations, and actionable recommendations; translate findings into clinic-level workflow improvements. · Conduct research, collect data, and make recommendations for future clinical quality and patient safety initiatives with a focus on sustainability. · Build strong working knowledge of dental quality/safety standards and workflows (infection prevention, documentation, patient safety events, and satisfaction drivers) to ensure quality oversight is balanced across medical and dental services. · Perform other duties as assigned.
