Essential Job Functions and Responsibilities SAFETY Provide supervision when children are playing and in activities. Provide personal care to the children and reports health and safety hazards. Follow Fraser Child Care's emergency procedures for fire, accident, tornado, and winter storms. PROGRAM Assist Child Care Supervisor in implementation of age-appropriate classroom activities and lesson plans which meet National Academy for the Education of Young Children standards; utilize the Stimulating Maturity through Accelerated Readiness Training (S.M.A.R.T.)/Boost-Up, Creative Curriculum and other curriculum plans as determined by the Special Needs Coordinator(s), Child Care Supervisor, Coordinator, and Director of Program Operations. Provide guidance and appropriate behavior management to children, as directed. Lead children in structured schedule of activities; accompany children on outings in the community. Complete objectives from special needs programs, as necessary. Participate in planning and providing a books, art displays, flannel board stories and dramatic play props. Use tact to address parents by demonstrating acceptance of varying parenting styles and reporting problems to supervisors. Assist Child Care Supervisor in daily S.M.A.R.T./Boost-Up activities and documentation such activities, as directed. CLASSROOM Support Child Care Supervisor, Coordinator, and Director in organizing and maintaining center areas by tidying and completing the daily and weekly cleaning schedules and stocking and inventorying supplies. Work cooperatively with staff; reports staff conflicts in a timely manner. Assist Child Care Supervisor in providing a multi-cultural and special needs awareness in the classroom environment and lesson plans. Attend staff meetings and in-service training as required. Education and/or Experience Desire and training to work with all children. Ability to relate to management, peers and people supported. Established time management skills and the ability to prioritize and manage workload. Computer Skills To perform this job successfully, an individual should have working knowledge of basic computer programs. Physical Demands While performing the duties of this Job, the employee spends a lot of time walking and moving about. The employee must have sufficient strength to turn, lift, and assist people with movement. Actively participate in the S.M.A.R.T./Boost-Up curriculum by demonstrating and participating with children in the essential components of Boost-Up. This includes, but is not limited to: crawling, rolling on the floor, jumping on rebounder, going across monkey bars and other physical activities. Certificates, Licenses, Registrations CPR and First Aid Certification within 30 days, CPR renewed annually, First Aid renewed as required. Current State of Michigan Registered Nurse (RN) license. Minimum of three (3) years of health care management experience required; five (5) years preferred. A minimum of three (3) years of hospice/home health experience required; five (5) to seven (7) years' experience and clinical expertise in hospice preferred. Certification in hospice nursing (CHPN) preferred. Expertise in regulatory requirements and compliance as it pertains to hospice required. Must be able to travel extensively through region, including overnight stays if needed. Frequently works variable hours/days; activities and workload may require extended days. Excellent interpersonal skills with the ability to comfortably interact at all social levels and with colleagues from many different background, ethic origins and skill levels required. This includes but is not limited to meeting all deadlines for reporting or recertification for Veteran's Administration Benefits, Social Security and Supplemental Security, County Benefits, Tribal Monies, Medical Assistance, SNAP, Pre-paid Burials, etc. and the meeting the reporting requirements for these services. It is the responsibility of this position to assure that the people supported have access to the services that they are entitled to. Prepares financial profiles for annual OSP meetings. PERSONNEL Works with the Independent Living Services Administrator (ILSA) to interview and hire Direct Support Professionals. Supervises Direct Support Professionals and Residential Coordinators and evaluates performance annually, implementing administrative recommendations. Completes Direct Support Professionals scheduling and hours as assigned and authorizes weekly employee time sheets. Completes census of people supported and reports individuals' days absent from facility. Models expertise to the Direct Support Professionals and Residential Coordinators regarding methods, strategies, and techniques relative to the OSP implementation. PROGRAM Manages assigned caseload to ensure coordination and completion of QIDP/QDDP responsibilities as they relate to housing, financials, health, and the day-to-day wants and needs of the people supported. Maintains open communication with people supported, guardians and family, Case Manager, Direct Support Professionals, and other team members. Uses their working knowledge of licensing, certification and accreditation standards when observing service delivery to assure that compliance with the standards is being met. Completes and updates General Events Reports, including team member notifications, environmental assessments, and any additional required follow-up. Initiates and documents monthly Guardian and family contacts. Responds to requests from people supported/family/Guardians/vocational and day support providers pertaining to the residence and day to day needs. Assists Fraser, Ltd.'s medical services department, as requested. Assures the consistent and accurate entry of program related data by Direct Support Professionals. Participates in internal and external investigations, as requested. Adheres to on-call policy and procedure and participates in the on-call rotation as scheduled. TRAINING Coordinates staff meetings and ensures Direct Support Professionals receive both in-service and program specific training. Provides in-home expertise and ongoing modeling for Direct Service Professionals regarding methods, strategies, and techniques relative to OSP implementation. Observes program delivery on an on-going basis to identify additional training opportunities for Direct Support Professionals. Assists with any additional training needs which may arise, as requested. Acquires and maintains CPR & First Aid Instructor Certification, instructing training classes as requested.
