POSITION SUMMARY:
Provide excellent patient and customer service in the selection, ordering and dispensing of eyewear. Complete pre-screening activities prior to patient exams. Prepare work orders for Optical Lab. Process medical billing information. Maintain frame inventory.
DUTIES AND RESPONSIBILITIES:
CUSTOMER SERVICE
· Provide excellent customer and patient service according to company and Optical Center standards
· Analyze patient eyewear needs
· Recommend specific lenses, lens coatings and frames to satisfy patient needs
· Assist patients in selecting frames according to style and color. Ensure frames are coordinated with facial and eye measurements and optical prescriptions
· Order and dispense eyewear accurately and professionally
· Assist patients with proper frame repairs and adjustments
· Show patients how to insert, remove and care for their contact lenses
· Assist Optometrist with case histories and screenings
BUSINESS AND OPERATIONAL STANDARDS
· Maintain a complete knowledge of third-party plans and claims
· Explain promotions and special lens offerings to patients and customers
· Balance accounts
· Prepare work orders containing specific instructions for Optical Lab
· Process medical billing information
· Ensure proper merchandising of frame inventory
MINIMUM KNOWLEDGE, EXPERIENCE & SKILLS REQUIREMENTS:
· High School Graduate or equivalent

JOB SUMMARY
Provide supportive services to the community using the Pathway of Hope strengths-based case management approach to individuals and families with a desire to take action to break the cycle of crisis and vulnerability in their lives and change the trajectory of their lives.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Conduct outreach and engagement services in the community to identify and support potential participants of Pathway of Hope.
Conduct client intake interviews to determine eligibility for programs or agency referrals.
Provide case management services to clients to include, but not limited to: reviewing intake material for accuracy, assessing client needs, provision of direct assistance, case plan development and compliance, act as liaison to community resources, provide referrals to appropriate agencies, Act as an advocate for clients when needed
Maintain an appropriate caseload (at least 10 unduplicated families per fiscal year and carry at least 5-7 active families at any given time). Provide appropriate referrals and services to support individuals and families of the local community.
Document and maintain written and electronic case files ethically and confidentially, to include progress, referrals, types of assistance provided, and other information as required.
Record, track and submit statistical information as scheduled to maintain compliance with The Salvation Army and grants funders as required.
Engage and collaborate by participating in monthly supervision, divisional case manager cluster call-ins, and local Pathway of Hope team meetings driven by the POH Regional Coordinator to ensure best practices.
Maintain working relationships with community agencies to provide comprehensive services for participants and to stay informed of current trends and resources available.
Collaborate with local leadership to best support clients and their designated goals.
In administering programs, uphold the policies, goals and mission statement of The Salvation Army.
Maintain positive work atmosphere through appropriate conflict management, courteous demeanor, and professional and appropriate communication practices.
Maintain compliance in The Salvation Army's “Safe from Harm” Certification.
Assist with answering the phone and doors/guests of the Corps for community needs.

Education:

Associate Campus Pastor | Freeport Campus
The Freeport Associate Campus Pastor will have two core ministry areas along with providing support to the Freeport Campus Pastor. The two core ministry areas are Director of The Freeport Dream Center (outreach ministries) and Pastor of Care.
The Freeport Dream Center (FDC) role will be one of shepherding our volunteers and stewarding our ministry focuses. The primary focus of the role will be working with and raising up volunteers in our church/community to grow these ministries. Currently FDC provides a furniture ministry to those in need, a food pantry, and on occasion community clean up days and residential clean up projects. The goal of FDC will be as a community lighthouse serving our community. Future vision includes (but isn't limited to) classes/services for Addiction Recovery, Parenting, Financial Training, Work Training; community restoration through works projects in blighted portions of Freeport, weekly ministry to serve Kids and families, serving our Hispanic neighbors, and launch of urban style church service.
The Pastor of Care role will focus on the growth of our care ministry. This care ministry is focused primarily on visitation of current members who may be home bound or at the hospital. They will be responsible for raising up lay-leaders for the ministry of care in a visitation format. We also desire to serve members in our church who are aging more effectively as they enter care facilities or face challenges that require them to be home bound. As such the Pastor of Care will help us launch ministry opportunities for those in nursing homes or extended care facilities with the long-term goal of launching satellite church campuses in these facilities. Primarily this role will focus on the management and training of individuals and couples who have a call to serve in pastoral care. The Pastor of Care will also help support the Campus Pastor in meeting with members of the church for very light counseling and premarital counseling.
While this role starts as a full-time split role, it is our intent to have each of these roles become independent full-time positions at Crossroads.

QUALIFICATIONS:

RESPONSIBILITIES:

* Maintain an appropriate caseload (at least 10 unduplicated families per fiscal year and carry at least 5-7 active families at any given time). Provide appropriate referrals and services to support individuals and families of the local community.
* Document and maintain written and electronic case files ethically and confidentially, to include progress, referrals, types of assistance provided, and other information as required.
* Record, track and submit statistical information as scheduled to maintain compliance with The Salvation Army and grants funders as required.
* Engage and collaborate by participating in monthly supervision, divisional case manager cluster call-ins, and local Pathway of Hope team meetings driven by the POH Regional Coordinator to ensure best practices.
* Maintain working relationships with community agencies to provide comprehensive services for participants and to stay informed of current trends and resources available.
* Collaborate with local leadership to best support clients and their designated goals.
* In administering programs, uphold the policies, goals and mission statement of The Salvation Army.

POSITION SUMMARY:
Provide excellent patient and customer service in the selection, ordering and dispensing of eyewear. Complete pre-screening activities prior to patient exams. Prepare work orders for Optical Lab. Process medical billing information. Maintain frame inventory.
DUTIES AND RESPONSIBILITIES:
CUSTOMER SERVICE
· Provide excellent customer and patient service according to company and Optical Center standards
· Analyze patient eyewear needs
· Recommend specific lenses, lens coatings and frames to satisfy patient needs
· Assist patients in selecting frames according to style and color. Ensure frames are coordinated with facial and eye measurements and optical prescriptions
· Order and dispense eyewear accurately and professionally
· Assist patients with proper frame repairs and adjustments
· Show patients how to insert, remove and care for their contact lenses
· Assist Optometrist with case histories and screenings
BUSINESS AND OPERATIONAL STANDARDS
· Maintain a complete knowledge of third-party plans and claims
· Explain promotions and special lens offerings to patients and customers
· Balance accounts
· Prepare work orders containing specific instructions for Optical Lab
· Process medical billing information
· Ensure proper merchandising of frame inventory
MINIMUM KNOWLEDGE, EXPERIENCE & SKILLS REQUIREMENTS:
· High School Graduate or equivalent
* Maintain license as required by state
· To become Certified, teammate must complete Shopko OCE Certification requirements and calendar modules
· Previous experience in an office and/or optical dispensing is preferred
· Able to handle multiple customer interactions/phone calls at a time
· Able to work with patient eyewear needs including analyzing prescriptions, repairs and adjustments and lifestyle dispensing
ESSENTIAL FUNCTIONS & WORK REQUIREMENTS:
· Ability to effectively communicate at all levels within the organization through written and two-way verbal communication
· Able to read and write at a high school graduate level
· Able to sit or stand for extended periods of time
· Able to operate various office equipment (e.g., personal computer, telephone, fax machine, copier, etc.)
· Ability to lift 10 to 20 pounds
· Ability to see (Near, Distance, Color, and Depth Perception)
· Manual and finger dexterity, as well as hand/arm steadiness
· Ability to grip and hold items
· Good eye and hand coordination
· Able to operate a cash register, various optical equipment and tools
· Able to work normal and/or extended (evenings, nights, and weekends) office hours to meet established deadlines
· Able to travel independently to support Company objectives and personal development
