ABOUT CENTRALIZED CLIENT SERVICES: The Centralized Client Services team includes approximately 12 team members focused on providing direct, holistic, trauma-informed services to young people. Key services areas include homelessness prevention; Anti-Human Trafficking and Sexual Assault/Abuse; Justice Involved Youth & Young Adult Community Partnership Work; School & Community Education, Outreach & Support Services. The Centralized Client Services division within YouthCare's Programs Department is responsible for connecting youth and young adults to services that provide continuity and stabilization; connecting youth, young adults and families to critical resources and services to prevent homelessness; outreaching into the community to educate the public and share the work YouthCare is doing to end youth homelessness; and working internally to foster an integrative service delivery approach that elevates team member competencies and success in providing culturally relevant services. This program is a part of a multi-disciplinary, Centralized Client Services team providing support to the Youth that we serve. YouthCare provides direct support to unhoused young people in the Seattle and King County region, and the Client Services Navigator team is a central program in meeting young people where they are and walking with them towards greater stability. POSITION SUMMARY: Under the supervision of the Senior Program Manager, the Client Services Navigator is responsible for providing direct, trauma-informed services to a caseload of no fewer than twenty (20) and no more than twenty-five (25) clients at a time. This position will work in a multidisciplinary environment, collaborating with staff across programs to support the diverse and intersecting needs of young people engaged with YouthCare. The role includes conducting housing assessments, Coordinated Entry for All (CEA) nominations, diversion, prevention, and housing navigation to appropriate housing and community resources. The Client Services Navigator will work across funding streams and be able to use varied modalities to support clients towards stability and housing. The Client Services Navigator is expected to work closely with all Orion Center staff and related programs, providing broader programmatic support and ensure coordinated service delivery and holistic client support. This role requires flexibility, collaboration, and strong organizational skills to support young people and young adults in crisis as they work toward stability and independence. ESSENTIAL RESPONSIBILITIES: Client Empowerment & Outcomes Leadership: Manage a caseload of 20-25 clients at a time, maintaining a minimum of 20 active cases. Conduct comprehensive housing assessments, create individualized service plans, and support clients through the CEA nomination and referral process. Support clients in developing self-advocacy and life skills necessary for long-term independence. Understand and utilize diverse funding streams, and the supported practices, to meet young people engaging with YouthCare. Participate in the support and activities of the Orion Resource Center, driving continuity of services and integration of programs. Support the program needs including meeting basic needs, coordinating meal preparation, and maintaining the safety and cleanliness of the center in accordance with established cleaning protocols. Documentation & Accountability: Maintain accurate and timely documentation in YouthCare's Caseworthy system, including service plans, progress notes, discharge summaries, and more. Team Leadership & Collaboration: Provide on-site, in-person support to young people while maintaining professional boundaries. The ORCA Center, our brick-and-mortar location, provides post-overdose subacute stabilization services and provides walk-in access to medications for opioid use disorder (MOUD). The ORCA Patient Outreach Division (POD) delivers field-based MOUD care - meeting people where they literally are in the community. ORCA Center The ORCA Center is open 24 hours per day, 7 days per week, including holidays. It is staffed by a multidisciplinary team, including providers, nurses, medical assistants, peers, and milieu specialists. The ORCA Center offers four overlapping types of services: Care to individuals in stable condition who have experienced an opioid overdose, brought to the ORCA Center by first responders or transferred from local emergency departments. Rapid initiation of medications for opioid use disorder (MOUD), including buprenorphine (sublingual and long-acting injectable) and methadone (under the “72-hour rule”, or CFR Title 21 §1306.07 (b)), for people with OUD, regardless of whether an opioid overdose has recently occurred. Harm reduction counseling and OUD-related physical and behavioral health services. Follow-up care for individuals who have started MOUD to ensure support in MOUD continuation. ORCA POD ORCA POD is ORCA's field-based care team, bringing MOUD induction, stabilization, and maintenance services as well as education and other supports directly to patients in the community. ORCA POD operates during business hours (Monday through Friday, 9 a.m. to 5 p.m.). The POD Provider may also support the DESC ORCA Center, which operates 24/7 offering rapid MOUD access and post-overdose care. This is a clinical, non-supervisory position. MAJOR DUTIES AND RESPONSIBILITIES: Field-Based Clinical Care and Community Engagement: Provide evidence-based MOUD treatment (buprenorphine, including LAI formulations; methadone under the 72-hour rule) in a variety of community settings. Provide telemedicine care to patients and remote consultation for nurses in the field. Deliver low-barrier clinical assessments, MOUD initiations, acute follow-up, and ongoing management for individuals engaged in outreach. Conduct harm reduction counseling, including safer-use strategies and overdose prevention planning. Manage common conditions such as skin and soft tissue infections and connect individuals to primary care or mental health services as needed. Participate in Seattle Fire Department Mobile Integrated Health joint outreach operations, pop-up MOUD access points, and targeted “hot spot” engagement based on real-time overdose and EMS data. Care Coordination & Multidisciplinary Collaboration Work closely with POD nurses and peers to deliver holistic, relationship-centered care. Coordinate care and warm handoffs with the ORCA Center, DESC outpatient clinics, community MOUD programs, hospitals, and outreach partners. Establish and maintain strong partnerships with SFD MIH, PSH and shelter staff, and other community organizations to expand access to low-barrier addiction care. Ensure continuity for individuals transitioning between field engagement, ORCA Center care, and community providers. Documentation & Clinical Practice Standards Complete timely documentation meeting DESC, regulatory, and payer requirements. Utilize electronic health records effectively and maintain accurate clinical and outreach notes. The ORCA Center, our brick-and-mortar location, provides post-overdose subacute stabilization services and provides walk-in access to medications for opioid use disorder (MOUD). The ORCA Patient Outreach Division (POD) delivers field-based MOUD care - meeting people where they literally are in the community. ORCA Center The ORCA Center is open 24 hours per day, 7 days per week, including holidays. It is staffed by a multidisciplinary team, including providers, nurses, medical assistants, peers, and milieu specialists. This position has administrative duties associated with planning, hiring, quality assurance/improvement, and collaboration with other health system partners. Additional activities include engagement on behalf of DESC in related healthcare and public health system policy work, and leadership support and/or direct involvement in DESC evaluation and other research efforts. This position will interface closely with ongoing efforts at DESC to care for individuals with opioid use disorder or at risk of opioid poisonings, through a variety of harm mitigation and treatment efforts. MAJOR DUTIES AND RESPONSIBILITIES: Administration, Planning, and Supervision In coordination with the Agency Medical Director, Agency Nursing Director, ORCA Senior Medical Lead, and other members of the ORCA leadership, the ORCA Provider Supervisor will support the operation and progress improvement of the services provided by the ORCA Center and POD. Provide and document clinical supervision of the 5-6 providers in the ORCA Program. Collaborate closely with the ORCA Center Operations Manager and ORCA Center Nurse Supervisors in the strategic structuring, recruitment, hiring, orientation, training, and supervision of a float pool of providers to work as needed to fill gaps in ORCA Center staffing. Supervise and evaluate assigned staff, encompassing clinical performance, work efficiency and output, and team collaboration. Incorporate an equity framework for all aspects of work across the ORCA Program in collaboration with other leaders. Participate in DESC medical staff leadership meetings, as schedule allows. Collaborate with other DESC leadership staff and teams whose work interacts with the ORCA Program for enhanced client care coordination and improved client experience as they seek services from different DESC programs. Support ORCA Program staff meetings, including discussion of complex cases, clinical performance standards, literature reviews, and general information sharing. Develop and maintain collaborative relationships with internal and external parties, including partner agencies, funders and relevant other entities. Clinical Provide evidence-based MOUD treatment (buprenorphine, including LAI formulations; methadone under the 72-hour rule) in a variety of community settings. Provide telemedicine care to patients and remote consultation for nurses in the field. Deliver low-barrier clinical assessments, MOUD initiations, acute follow-up, and ongoing management for individuals engaged in outreach. Conduct harm reduction counseling, including safer-use strategies and overdose prevention planning. Provide screening, testing, and treatment related to infectious diseases commonly affecting people who use drugs (HCV, HIV, STIs). Manage common conditions such as skin and soft tissue infections and connect individuals to primary care or mental health services as needed. Participate in Seattle Fire Department Mobile Integrated Health joint outreach operations, pop-up MOUD access points, and targeted “hot spot” engagement based on real-time overdose and EMS data. Support field-based follow-up after overdose, SFD encounters, or ORCA Center visits to ensure MOUD continuation and reduce treatment drop-off. Other Provide in-service education to ORCA Center staff at all levels. Work with quality assurance and others to verify compliance with administrative rules such as and not limited to patient and records privacy. Participate in research and program evaluation activities related to the ORCA Program as capacity allows. At YouthCare's YouthBuild, located at South Seattle College's Georgetown Campus, young people learn construction skills through building affordable housing for homeless and low-income people in their neighborhoods while they complete their GED or prepare for post-secondary education or apprenticeships in the building trades. POSITION SUMMARY: Under the supervision of the Senior Program Manager - Engagement and Workforce Services, the Program Manager - YouthBuild will provide leadership and support to the YouthBuild program team. The Manager will work on the sector focused/industry training elements in collaboration with other department and agency leaders, including program design and implementation. The Manager will support the development and management of various strategic partnerships pertaining to workforce programming, including growing employer engagement relations and communication. This role will be instrumental in creating and sustaining external relationships with partners, employers, funders, and other stakeholders as it pertains to programs under their supervision. The role provides direct supervision to 4-8 staff working across the team. ESSENTIAL RESPONSIBILITIES: Program Planning and Development: Design and develop strategic plans for the YouthBuild Pre-Apprenticeship Program that aligns with organizational goals and community needs. Establish program objectives, milestones, and timelines. Create and maintain program documentation, including manuals, policies, and procedures. Oversee implementation of Department of Labor requirements. Ensure training content is current and aligned with industry standards and employer expectations. Partnership Development and Engagement: Establish and maintain relationships with local employers, apprenticeship programs, educational institutions, and community organizations. Develop partnerships that provide opportunities for internships, job placements, and advanced training for participants. Represent the program at community meetings, events, and with potential partners to promote engagement and support. Monitoring and Evaluation: Develop and implement systems for tracking participant progress, including attendance, performance, and outcomes. Conduct regular program evaluations to assess effectiveness and identify areas for improvement. Collect and analyze data to inform program decisions and report on outcomes to stakeholders. Budget and Resource Management: Manage the program budget, ensure funds are allocated effectively and expenditures are monitored. Identify and secure resources necessary for program operation, including materials, facilities, and staff. Seek additional funding opportunities through grants and partnerships. Contracts and Reporting: Ensure the program complies with all relevant laws, regulations, and guidelines. Maintain accurate and up-to-date records and documentation for all program activities. Prepare and submit required reports to funders, partners, and organizational leadership. Team Leadership and Supervision: Recruit, train, and supervise program staff, including educators, trainers, and support personnel. Foster a collaborative and positive team environment. Implement strategies to manage crises and ensure the safety and well-being of participants and staff. Provide support and resources to participants facing personal or professional challenges. Bachelor's degree in social work or relevant field of study and 1+ years of relevant work experience, OR an equivalent combination of education, relevant work experience, and/or lived experience (3+) serving young people experiencing homelessness, housing instability, poverty, and systemic oppression Experience and credentials must comply with program contractual requirements, including but not limited to the Washington State Department of Children, Youth, and Families (DCYF) background clearance process and must be at least 21 years of age. DCYF Clearance must be maintained for entirety of employment when working with under-18 clients. PREFERRED REQUIREMENTS: Master's degree in social work, Public Administration, Education or related field. Demonstrated ability in youth development, employment training and/or familiarity with YouthBuild model. JOB DESCRIPTION: The MRRCT Day Shift Project Manager is responsible for overseeing a highly motivated and effective crisis response outreach team, representing the roles of Clinical Supervisors, Mental Health Professional Outreach Specialists, Certified Peer Outreach Specialists, and Behavioral Health Outreach Specialists. This team will respond to the crisis needs of community members throughout the central region of King County, as dispatched by Crisis Connections/988, and will provide follow up care or recommendations as deemed clinically appropriate. In conjunction with the project manager, team leader, Nursing Supervisor, psychiatrist and Medical Director, this position has overall clinical responsibility for monitoring participant treatment and staff delivery of clinical services. All members of the team are expected to work with participants in a variety of shared functions. This team is designed to work with individuals who have experienced long durations or multiple episodes in psychiatric hospitalizations and/or residential mental health treatment. Individuals served experience chronic or multiple episodes of severe mental illness, co-occurring substance use disorders and possibly homelessness. These individuals have either been unable or unwilling to participate and succeed in traditional treatment settings. Therefore, it is critical that the members of this team work together in a collaborative manner that eliminates the traditional treatment borders and barriers. MAJOR DUTIES AND RESPONSIBILITIES: Coordinate client care with a multi-disciplinary team of social workers/case managers, chemical dependency professionals, psychiatric nurses, peer counselors, employment specialists and other mental health staff within the PACT team. Perform full psychiatric evaluations and provide treatment and recommendations as appropriate, at DESC sites and in the community, as needed by program and individual need. Diagnose the nature and extent of mental and/or substance-related disorders by reviewing the client's clinical record, requesting and reviewing known medical/psychiatric documentation, coordination with other providers, and evaluating the client's current physical and psychological condition. Coordinate all psychiatric services provided with Program management, Medical Director, nursing and other management staff. PACT always has a provider on-call who is available for consultation with other staff as well as Emergency Departments, first-responders, and other medical providers. Maintain compliance with all related licensing, DEA, and other regulations. Conduct all work within principles of the Recovery model. Evaluate clients and advise course of treatment. Respond to emergency situations as needed. Refer and advocate for clients to consult with a primary care physician on a regular basis. When appropriate, refer clients to other specialists. For all clients receiving medication treatment, conduct a face to face evaluation at least every 90 days or other schedule as related regulations or contract terms change. Document all client contacts and pertinent information according to agency, County, and State requirements within stated time lines.
