Job Summary
This position performs Magnetic Resonance Imaging (MRI) procedures to create three dimensional images of tissue, organs and other body parts that are used by radiologists to detect abnormalities, make diagnoses and treat patients.Job Description
Minimum Qualifications
Education: Graduate of an accredited School of Radiologic Technology.
Licenses/Certifications:
Certified in Basic or Advanced Life Support, required.
One (1) of the following required:
Registered MRI Technologist by the American Registry of Radiologic Technologists (ARRT-MR); or
Registered MRI Technologist by the American Registry of Magnetic Resonance Imaging Technologists (ARMRIT).
Experience / Knowledge / Skills:
Ability to lift and/or maneuver patients in conjunction with others in order to perform job functions.
Principal Accountabilities
Performs MRI procedures on patients: prepares patients for MRI exam, explains procedure, administers contrast when necessary, positions patients for the MRI scanner, selects appropriate imaging techniques, and ensures technical quality of the scan.
Enters and monitors patient data.
Performs quality assurance on magnetic resonance equipment and matrix camera. May assist with initial and renewal application for ACR and other accreditation.
Identifies and reports any accidents, complaints or equipment malfunction to department management.
Takes emergency call within the department.
Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.

What We're Looking For - Qualifications and Skills:
A high school diploma or equivalent.
Minimum two years' experience with proven success in a supervisory or leadership role in retail, finance, service, or related industries.

What You'll Do - Essential Duties and Responsibilities:
Manage overall store performance by meeting or exceeding Company performance standards.
Coach, lead, and develop all team members to build new business and increase store growth by demonstrating knowledge of and training on systems, Company standards, account management, recovery (collections), job duties, and performance reports.
Lead the charge for all team members to identify local marketing strategies, use business-to-business partnership opportunities, obtain referrals, host and participate in community and in-store events to steer growth and build revenue.
Examine, evaluate, and process loan/pawn applications and all relevant transactions, and assess risk within established limits.
Enforce adherence to quality standards, procedures, and local and state laws and regulations.
Participate in audits and compliance reviews as directed by the corporate office or District Manager.
Supervise and maintain office security including cash management and loss prevention by verifying and documenting cash overages/shortages, vault, inventory, deposits, and expenses.
Conduct proper opening and closing procedures and train new staff in keyholder duties.
Participate in the selection, review, hiring, and retention of new employees.
Develop work schedules in accordance with budget, workloads, and store needs. Ensure the store is staffed for optimal performance.
Handle complex customer situations that arise with integrity and professionalism.
Monitor and maintain internal and external store appearance and address basic facility needs, including scheduling maintenance services. This includes overseeing the store planogram and ensuring seasonal and/or promotional marketing material are displayed properly.
Perform duties outside of the office, when applicable, including on site vehicle appraisals, store errands, and external marketing.
Work efficiently in a rapidly changing and fast-paced environment and handle multiple challenging tasks with ease to meet individual and team performance standards.
Conduct additional tasks as directed by leadership.
Maintain a full-time work schedule with regular, in-person attendance, including weekends. A full-time work schedule for this position includes, at a minimum, 40 hours per week.*

ROLE SUMMARY:
Our Supply group is responsible for brewing, packaging and shipping the famous Budweiser beer. They also oversee aluminum can manufacturing, barley fields, hop farms and a rice mill. Life in the brewery is non-stop action with high-speed lines, tight production schedules and astonishingly high quality standards. This is a place where we measure efficiency in seconds, not hours, days or weeks. JOB RESPONSIBILITIES:
Maintain machinery, meeting maintenance requirements for 480-volt, 3 phase systems, and minimizing downtime will be necessary.
Support, troubleshoot, improve, and maintain high-speed production and packaging machinery.
Utilize SAP to accurately and thoroughly detail and track all corrective and preventive maintenance performed throughout the course of the day; including searching and issuing parts, documenting equipment issues and entering inspection/measurement points
Prioritize and self-schedule maintenance work around production activities.
Drive plant goals & objectives through individually led projects and continuous improvement activities
Responsible for programmable controllers and computer process controls.
Ensure quality standards are met at all times.
JOB QUALIFICATIONS:
PLC programming and troubleshooting experience required.
3 year industrial electrical/programming experience or two-year technical degree preferred
Outstanding communication and team-orientation skills.
Knowledge of Servos and related controllers including hydraulic and pneumatic, temperature controls.
Ability to troubleshoot problems, identify root causes (e.g. six sigma analysis) and propose and implement remedies

The Community Care team is a multidisciplinary service including Registered Nurse (RN) Community Care nurses, Licensed Practical Nurse (LPN) Community Care nurses, Community Social Workers (CSW) and Community Health Coordinator (CHC) who work with our highest complexity patients and their primary care physicians to meet their medical and social needs with the aims of fully engaging them in our intensive primary care model and maximizing their healthy time at home.
Intensive Community Manager will serve as a clinical lead for a Community Care team. They will coordinate the team's efforts to stabilize our highest risk patients, with special areas of focus including safe transitions of care from facilities back to our primary care teams, stabilization of our highest risk ambulatory patients and outreach to patients who are assigned to us but are not engaged in care.
This person will perform initial assessments and design comprehensive plans of care for many of these patients. This professional will also provide clinical supervision to other team members in delivering the plan of care and in other tasks necessary to meet their needs and engage them in care. Summary:
The Telemetry Registered Nurse is responsible for providing comprehensive patient care in a telemetry unit, focusing on continuous cardiac monitoring and assessment. This role encompasses evaluating patient health, implementing specialized care plans, administering treatments, and collaborating with healthcare teams to ensure high-quality, patient-centered care for individuals with cardiac conditions. Essential Work Functions:
Monitors and interprets cardiac rhythms, vital signs, and other telemetry data to detect abnormalities and respond appropriately
Administers prescribed medications and treatments in accordance with approved nursing techniques, with a focus on cardiac care
Prepares equipment and aids physician during cardiac-related procedures and examinations
Maintains awareness of comfort and safety needs of telemetry patients
Observes patient, records significant conditions and reactions, and notifies supervisor or physician of patient's cardiac status and response to interventions
Responds to life-saving situations based upon nursing standards, policies, procedures, and protocols specific to cardiac emergencies
Documents nursing history and physical assessment for assigned telemetry patients
Initiates patient education plans according to individualized needs, focusing on cardiac health, risk factors, and lifestyle modifications
Collaborates with the interdisciplinary team to ensure comprehensive care for telemetry patients
Maintains confidentiality of patients and client
Performs other duties as assigned within the scope of practice
Adheres to hospital safety protocols, infection control guidelines, and regulatory standards

The Associate Clinical Director will directly supervise and train primary care providers (PCPs) in his/her assigned center. The incumbent in this role is accountable for maximizing overall core model execution, including improving clinical quality, efficiency, outcomes, and clinician/patient satisfaction. In addition to being accountable for the overall clinical outcomes of his/her assigned center, they will have a portion of their time allocated to direct patient care as a PCP and/or other clinical duties (amount dependent on number of direct reports). The remainder of their time is allocated to leadership responsibilities, including PCP performance, engagement, and building a strong clinical-operations synergy and culture. The allocation of time is dependent on several factors, including PCP capacity, market needs, size of centers, patient membership, and Market Clinical Director direction.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
Independently provides care for patients with acute and chronic illnesses encountered in older adult patients.
Takes full accountability for patient care and outcomes and appropriately seeks consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient.
Engages with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not).
Responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.
Leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.
For patients that are unable to come to the office-in hospital, SNF, LTC or homebound, engages with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources that may be available depending on the market.
Leads Super Huddle (SH) and Transforming Care Meeting (TCM) weekly, as well as supports Center Manager/Center General Manager center clinical leader and/or market clinical leader is not available, based on guidance from Market Chief Medical Officer. Fills in as needed for center clinical leadership needs, including monitoring daily center census as part of joint center accountability for outcomes.
Plays an active role in the management of their center and helps cover for other providers who may be out for various reasons. It is also expected that each Clinical Director will take an active role as needed in recruiting patients for the center and recruiting and interviewing additional providers for the company.
Managing, mentoring and coaching PCPs in his/her assigned center to deliver outstanding clinical outcomes, including sampling other PCP's daily huddles within their center
Leadership rounding with the PCPs (reduced involvement of market clinical leader)
Partnering with Center Operations Director/Market General Manager to drive continued improvement of center financial performance, and helping increase center membership
Performs other duties as assigned and modified at manager's discretion.
KNOWLEDGE, SKILLS AND ABILITIES:
Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other applications used in the company
Ability and willingness to travel locally as needed in their market, if applicable, nationally for initial training (2 weeks) and then occasionally regionally and nationally for recruiting or training purposes
Fluency in English, verbal and written. There may be jobs in some centers that require fluency in other languages, and this will be made known at the time of application.

EDUCATION AND EXPERIENCE CRITERIA:
MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required
Current, active MD licensure in State of employment is required
A minimum of 1-year clinical experience in geriatric, adult or family practice setting preferred, with Lead PCP ideally being a ChenMed PCP Partner
Completion of Chen Medical training, including Masterful Conversations and meeting facilitation, as part of the individual development plan
Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred, Board Eligibility is required
Once Board certified, PCP will maintain board certification in their terminal specialty by doing necessary MOC, CME and/or retaking board exams as required
Must have a current DEA number for schedule II-V controlled substances
Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment

The Care Coordinator is a highly visible customer service and patient-focused role. The incumbent in this role works directly with our patient population and their families, insurance representatives and outside vendors, physicians, clinicians and other medical personnel to ensure the referral process runs smoothly. He/She operates in a dynamic and professional environment to ensure the highest level of quality healthcare is delivered to our members.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
Coordinates and processes patient referrals to completion with precision, detail and accuracy.
Definition of completion:
Prioritizes HPP patients in Primary Care Physicians panel, stats, expedites and orders over 5 days.
Orders have been approved (when needed).
Schedules patient (Preferred Providers List of Specialist) and notifies them of appointment information, including, date, time, location, etc.
Uses Web IVR to generate authorizations (Availity, Careplus, Healthhelp NIA and any other approved web IVR for authorization processing).
Completes orders with proper documentation on where patient is scheduled and how patient was notified.
Referrals have been sent to specialist office & confirmed receipt.
Prepares and actively participates during physician/clinician daily huddles utilizing RITS Huddle Portal and huddle guide. Effectively communicates the physicians/clinicians needs or outstanding items regarding to patients.
Enters all Inpatient and Outpatient elective procedures in HITS tool.
Ensures patient's external missed appointment are rescheduled and communicated to the physician/clinician.
Participates in Super Huddle and provides updates on high priority patients referrals.
Addresses referral based phone calls for Primary Care Physicians panel.
Completes and addresses phone messages within 24 hours of call.
Checks out patients based on their assigned physician/clinician. (Note: If assigned Care Coordinator is unavailable at the time of check out, a colleague shall assist patient. This process does not apply to Care Specialist)
Retrieves consultation notes from the consult tracking tool.
Follows up on all Home Health and DME orders to ensure patient receives services ordered.
Provide extraordinary customer service to all internal and external customers (including patients and other
