Integrates assessment using information related to cultural clinical ethnic age and other factors into the plan of care in order to implement discharge plans and interventions. Acts as an active team member in the discharge planning process and primary responsibility for the assessment of complicated social and financial factors and barriers to appropriate discharge and transitions to needed placements and resources.
