Advocate for clients with social services, healthcare providers, and other agencies and connect them to essential resources like therapy, housing and medical care. Educate clients on coping mechanisms and self-care strategies. Maintain regular contact with clients to assess progress and adjust care plans as needed. Facilitate support groups or workshops on trauma recovery and resilience. Maintain accurate and timely case notes, client records, and reports. Ensure compliance with agency policies, confidentiality laws, and funding requirements. Track client progress and report outcomes to supervisors or funding agencies as required.
