This position supports personal care, daily living activities, and agreed-upon domestic tasks while promoting safety, independence, and comfort for each client. Educates residents/family/staff about the importance of advance directives, and facilitates completion and management of such documents when needed. Coordinates the Social Work function from admission through discharge, to include room changes, and transition planning when appropriate, in collaboration with staff. Collaborates with Mental Health Providers and provides referrals as needed.
