This role will work collaboratively with all providers to provide complete, comprehensive clinical care for patients in an inpatient setting. Clinical Care will include diagnosis, treatment, and coordination of care for preventative and healthcare maintenance to patients. Physician will be joining an established group with an instant referral base and will perform the full spectrum of gastroenterology tests and procedures.
Schedule:
This position will be scheduled to work 7 days on and 7 days off.
The provider will work Friday through Thursday 7:00am-5:00pm.
The provider will take night call 5:00pm-7:00am Friday, Saturday and Sunday.
Call includes coverage for Northeast Georgia Medical Center Gainesville & Braselton.
Advanced Practice Provider assistance for night call and day inpatient consults.

Working Hours:
The set clinic hours for the practice are Monday-Thursday 8:00am-5:00pm and Friday 8:00am-12:00pm. While these are the set clinic hours during which the physician will be scheduled to see patients, the physician may have to come in earlier or work later to get the work completed.
In addition to clinic hours, this position will include a rotating schedule for procedure blocks at the Ambulatory Surgical Centers and local outpatient hospital blocks. This position is also part of a rotating schedule for night call, weekend, and holiday coverage for Northeast Georgia Medical Centers in Braselton and Gainesville. Coverage will be assigned on a rotating basis with the other physicians in the practice.

Speech Language Pathologist (SLP)
A Speech Language Pathologist (SLP) administers speech therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Nursing Supervisor. Speech therapy services are furnished only by or under the supervision of a qualified speech pathologist or audiologist.
QUALIFICATIONS:
1. A person who has a master's or doctoral degree in Speech Language Pathology, and is licensed as a Speech Language Pathologist by the state where they furnish services, or
2. A person who has successfully completed 350 clock hours of supervised clinical practicum (or be in the process of completing), at least nine months of supervised full-time SLP experience, and has successfully completed a national approved examination.
3. Currently licensed in the state(s) in which practicing.
4. Two (2) years' experience, preferred.
RESPONSIBILITIES:
1. Improves or maximizes the communication of the patient.
2. Returns the individual to optimum and productive living within the patient's capabilities.
3. Periodically participates with all other home care personnel in patient care planning.
4. Provides full range Speech Language Pathology Services as ordered by physician.
5. Directs and supervises personnel, as assigned.
6. Takes initial history and makes initial evaluation.
7. Performs all skilled procedures.
8. Consults with physicians regarding change of treatment.
9. Writes reports to physicians regarding patient's progress.
10. Instructs patients and family members in home programs.
11. Periodically presents an inservice to the Agency's staff members.
12. An initial evaluation, including plan of treatment and goals, must be completed and submitted to the physician for approval. A recertification by physician every 60 days is necessary if further treatment is to be continued. A progress note is written each visit. A reevaluation is written when expected duration of treatment is reached. A summary is written upon patient's discharge.
13. A supervisory visit is made every fourteen days on each patient seen by a home health aide as assigned.
14. Assists the physician in evaluating level of function.
15. Helps develop the plan of care and revises as necessary.
16. Prepares clinical and progress notes.
17. Advises and consults with the family and other Agency personnel.
18. Participates in inservice programs.
19. Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.
20. Appropriately utilizes ICD-10 codes.
21. Participates in QAPI activities as assigned.
WORKING ENVIRONMENT:
Works indoors in Agency office and patient homes and travels to/from patient homes.

ESSENTIAL DUTIES • Mix biologics and other medications, along with pre-medications with aseptic technique according to medication guides. • Review health history, allergies, and current medications with patients or patients' parent upon arrival. • Monitor vital signs before, during, and after infusions. • Start intravenous lines with aseptic technique. • Draw bloodwork as ordered from IV or through venipuncture. • Chart detailed nurse's notes and charges in Epic electronic charting. • Document detailed nurse's notes on adult patients on paper infusion flowsheets. • Educate patients at discharge about signs and symptoms to call in to the office. • Demonstrate sound clinical judgement while following guidelines and medication protocol for allergic reactions. • Notify physicians of any changes in patient status since last infusion. • Fax updates to adult or referring physicians. • Record verbal and phone orders from physicians and enter into Epic accordingly. • Schedule patient appointments. • Administer PPD skin tests and subcutaneous and intramuscular injections to patients and occasionally to staff. • Participate as infusion nurse or pharmacist in research studies • Attend and participate in staff meetings as scheduled. • Attend periodic training sessions and meetings related to position. • Maintain excellent communication and effective working relationships with patients, providers, as well as all staff, administration and supervisor colleagues. • Establish remote Epic access on home computer. • Potential to be cross-trained across other departments. • Potential to infuse patients at satellite locations. • Maintain RN licensure and CPR certification. • Performs other duties as assigned by management.
