    Job Description The Medical Assistant - Regional manages the check-in, rooming, and check-out process for patients at regional outreach locations, ensuring the process runs smooth and efficient for both the patient and the provider. Completes the patient rooming and check-out process, educates patient on care plans and related items, ensures readiness of charts by reviewing and preparing all pertinent patient information, assists with and/or performs procedures, and accurately records all actions in the electronic medical record. *This position rotates between Rhinelander & Woodruff. Periodic travel to Wausau is also required. Essential Job Functions and Responsibilities Completes the patient rooming and check-out processes. Obtains, documents and/or verifies all required information, including but not limited to reason for visit, past medical history and procedures, allergies, medications, social and family history, demographics, pharmacy, Advance Directive, and other required information. After provider visit, schedules patient follow-up appointments and procedures, contacts pharmacies with new and refill prescription orders, assists in referring patient for ancillary services as directed by provider and documents future appointments and procedures. Accurately enter provider's orders using computerized provider order entry (CPOE) for (but not limited to) medication, laboratory, diagnostic imaging orders and procedure orders. Educates patients on care plans, prescribed medications, injections, lab testing and specimen collections, preparation for pre- and post-procedure care, and other related items. Ensures readiness of chart by reviewing and preparing all pertinent patient information prior to patient's office visit with provider, and works with HIM Department to obtain records. Accurately records actions in the electronic medical record to reflect all care given to the patient, observations made about the patient, and the response to treatment or medications by the patient. Assures information required to successfully measure and report care quality measures are properly documented in all encounters and orders. Chaperones and/or assists with procedures, including but not limited to anoscopy, trigger point injections, internal hemorrhoid banding and other procedures within scope of practice. Maintains inventory for exam rooms and stocks with supplies/current magazines/reading material. Cleans exam rooms using appropriate cleaning methods and also maintains neat work/desk area. Performs clerical duties such as photocopying forms, insurance cards, reports, patient information, labels and faxing or requests faxes as needed by the department or the office. Checks fax log to ensure faxed item transmission. Communicates timely and effectively with supervisor, clinical team leads, and administrative functions at the main office. Actively supports both Physician and Advanced Practice Provider clinics as assigned and as needed. Attends and participates regularly in Clinical Forum and other educational forums and, as appropriate, implements the information presented into practice. Performs other duties as assigned. Minimum Qualifications Education High School diploma or equivalent required. Successful completion of an accredited Medical Assistant program is preferred. Licensure/Certification BLS Certification Required Current certification through an approved Medical Assistant credentialing organization or Assessment Based Recognition in Order Entry (ABR-OE) through AAMA is preferred; Certified Medical Assistant (AAMA), Registered Medical Assistant (ARMA), Certified Clinical Medical Assistant (NHA), or other comparable Medical Assistant certification preferred. Experience Minimum of one year of experience as a Medical Assistant highly preferred. Competencies Required Knowledge Working knowledge of medical terminology. Basic understanding of pharmacology. Skills Possesses interpersonal, communication, and listening skills necessary to deal effectively and courteously with patients, physicians, and all staff members. Proficient computer skills working in an Electronic Medical Record (EMR) and Practice Management software. Demonstrates professionalism and respect in all forms of communication and correspondence. Abilities Ability to maintain strict confidentiality of health information. Ability to work in a fast paced, multi-tasking environment and cope with rapidly changing patient care needs. The Medical Scribe at Marin Community Clinics (MCC) is an integral part of MCC's team care model, enhancing the quality and efficiency of our patient care. The medical scribe annotates any dictated or written information for the treatment of patient following all local, state, and federal guidelines for documentation into the electronic health record (EHR). The medical or specialist scribe will be responsible for clearly recording each patient's symptoms, history, physical exam, and documenting the diagnosis as well as treatment plans. The medical scribe is not a direct patient care role. This positions requires working at different clinic locations in Marin County (San Rafael, Larkspur and Greenbrae), clinics are within close driving distance of each other. Work schedule: Monday through Friday 8:00am to 5:00pm, evening shifts of 11:00am to 8:00pm and Saturday shifts with advanced notice. Responsibilities Scribe-Specific Tasks and Duties: The primary role of the scribe is to assist the provider with documentation of each patient's clearly recording each patient's symptoms, history, physical exam, and documenting the diagnosis as well as treatment plans in the medical record during a clinic visit. At the start of the visits scribes checks the “document scribed” checkbox on the intake template at the top center. Accompanies the clinician into the patient examination area, or joins the appointment remotely by telephone, in order to transcribe a history and physical examination as given by the patient and provider. Under the direction of the provider, transcribes and places patient orders, including laboratory tests, radiology tests, referrals, etc. Documents any procedures performed by the provider or nurses in the EHR. Transcribes any consultation or discussions with family and/or the provider. Completes the patient's chart by transcribing results of any labs, x-rays, or other evaluations. In doing so, the scribe continuously checks on the progress of this data in order to get the patient's workup complete so that the provider is able to make his/her decisions regarding that patient. Scribe lists all proper diagnoses as well as any follow up instructions and prescriptions, as dictated by the provider. Scribe does not directly assist with patient care. The scribe documents orders generated by the provider. Scribe assists the provider and support staff with paperwork, telephone calls, faxing, and chart abstraction and may facilitate communication among the care team. Scribe is able to process portal communications and either task to appropriate member of the care team or get information from the provider to respond to the patient in a timely way. The Medical Scribe does not perform the following: Medications: A scribe cannot prescribe medications. Scribes cannot stop, start, or make any manipulation to any medications in the "medication module”. Orders: A scribe cannot order any in-house medications that are injected, ingested, or inhaled into the body including nebulizer treatments. Document Sign Off: The provider should always review the record and attest to its correctness and completeness. At the end of the day or shift, the provider should review the scribed documentation to ensure all events were recorded accurately. The provider generates the document, creating the completed "Master IM." The scribe does not. Patient Care: The scribe is not directly involved with patient care; can't touch the patient; can't handle bodily fluids; can't act independently without direct clinical oversight; conduct other duties while acting as the medical scribe; however, he/she may be asked to give patient paperwork during visit or discuss discharge instructions with a patient. The scribe can't give orders to Registered Nurses. They interact with patients on a limited level. For exceptions to this section, please see the “Optional Trainable Skills” section below. General Documentation: A scribe cannot document any of his/her personal objective findings or be involved in any of the medical decision-making. A scribe cannot act independently; instead, he/she must simply document the provider's dictation and or/activities during the visit. Optional Trainable Skills: With the permission of medical/operational leadership and participation in an in-house training/competency program, a Scribe can be trained to perform vitals, room patients. Other duties as assigned Qualifications Education and Experience: High School Diploma or Equivalent (GED) required. Experience with EPIC Electronic Health Records preferred. Required Skills and Abilities: Must be able to communicate effectively in English (able to read, write and speak). Bilingual in Spanish is required. Ability to multi-task work duties.
