General Physician, P.C. is recruiting for a PA/NP for our Hospitalist Program at Brooks TLC Hospital located in Dunkirk, NY. The successful PA/NP will provide clinical support to inpatient adult patients on the PM shift with the ongoing readily available support of the Supervising/ Collaborating, Emergency Medicine and Anesthesia providers. This is a full-time Hospitalist position, scheduled as 12-hour shifts PM Shift.
The position hours: 7P-7A, 7 on 7 off
Key accountabilities:
· Hospitalist shall provide prompt, responsive and quality medical services to Brooks-TLC inpatients in a manner that promotes the effective and efficient operation of the Brooks-TLC Hospitalist Service Program
· Admit, diagnose and thereafter assume and provide or arrange for all reasonably necessary inpatient medical care services and treatment to each patient referred to the Program by a referring physician.
· Promptly review patient records upon admission and coordinate treatment with the referring physician.
· Ensure that Hospitalist's own services contribute to favorable health care outcomes by monitoring and evaluating the necessity, appropriateness, efficiency, effectiveness, and safety of medical services ordered and provided to prevent unnecessary utilization of medical services and tests.
· Ensure the proper documentation and coding of medical records through self-education and work with administrative personnel to improve the accuracy of documentation and coding of medical records for billing and reimbursement purposes.

Position Summary

Correctional Health Services
The New York City jail system is the second largest in the nation. A significant number of individuals in the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, clinics in each of the facilities, substance use treatment, medication assisted treatment (MAT), integrated re-entry services that includes alternatives to incarceration, and relationships with multiple training institutions throughout the city. Furthermore, CHS has been engaged in an effort to better identify people with neurodevelopmental/neurocognitive disorders and to utilize psychological testing to connect people to services in the community (e.g., with OPWDD).
Responsibilities:
Under the supervision of a Senior/Supervising Psychiatrist in the Mental Health Service of CHS, this position requires the provision of high quality, team-based psychiatric services including psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. Each of the facilities in the jail system has unique patient population and environmental characteristics; each psychiatric Nurse Practitioner will be matched with the population that most closely meets their clinical expertise, desired work location and schedule, and the clinical needs of the service. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical.
Responsibilities Include:
Provide psychiatric care, including admission evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs
Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information.
Evaluate patients for the need for a higher level of care, including hospitalization, and escalate care when clinically appropriate. If a patient is hospitalized, maintain contact with the hospital psychiatric staff to ensure continuity of care.
Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare services. Document all activities in the electronic medical record in a timely, concise and clear manner.
Provide consultation to non-prescribing mental health staff and medical staff about diagnosis, treatment and/or the need for psychiatric medication.
Counsel patients using advanced psychotherapeutic techniques, including in group settings if needed.
Participate in team meetings with the treatment staff, on a daily basis if working on a mental observation unit and on a weekly basis if working in one of the clinics.

Position Overview
Under the supervision of the Site Medical Director, Nurse Practitioners will deliver comprehensive, compassionate, and thoughtful care to patients with complex chronic diseases within the New York City jail system. This role involves collaboration with physician leadership to provide general primary care, which includes conducting detailed histories and physical exams, diagnosing and managing both acute and chronic illnesses, and assessing the necessity for consultative services. In addition, the Nurse Practitioner will engage as part of an interdisciplinary team, focusing on patients with chronic medical conditions or physical disabilities who do not require acute inpatient admission. This approach ensures a holistic view of patient care and leverages the expertise of diverse healthcare professionals to optimize treatment outcomes.
Responsibilities include: (Detailed Task)
Diagnose and treat acute and chronic illnesses. Evaluate the need for consult services and submit the prioritized consult when indicated.
Complete comprehensive histories and physicals on all new admissions including documentation of problem list, diagnosis, orders (e.g. labs, imaging and referrals) and ordering appropriate medications where applicable.
Evaluate patients requesting sick call, schedule follow-ups and update medication orders. Update problem lists and reconcile patient orders at all visits.
Implement plans for patient care utilizing protocols approved by the Medical Director and/or treatment plans reflecting the current standard of care.
Request radiology exams, lab tests, EKGs when clinically indicated and interpret these results based on clinical findings and in consultation with supervisors where appropriate.
Review all specialty consults and hospital returns to ensure that the standard of care is met and recommendations of the consultant are implemented.
Perform chart reviews and summaries for patients transferring facilities including updating problem lists, rewriting medication orders, and reconciling orders and consults as needed.
Generate special needs referrals and documentation as needed (for patients with (disabilities, dietary restrictions, heat sensitivity, or other relevant flags).
Teach patients about their medical conditions and treatments; counsel on risks and benefits of different treatment decisions; witness, sign, and document patient refusals of care.
Ensure that all progress notes and orders are signed before the end of the shift.
Respond to emergencies in a timely and professional manner.
Notify the staff MD and Urgicare about 3 hour runs and EMS activation.
Complete special housing rounds when assigned.
Be familiar with quality of care and population health indicators. Take appropriate action to meet or exceed standards.
Maintain clinical competency by participating in all CME and CHS training and in-service requirements.
Maintain your schedule as directed with particular attention to punctuality and timely notification of absences.

Correctional Health Services
The New York City jail system is the second largest in the nation. A significant number of individuals in the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, clinics in each of the facilities, substance use treatment, medication assisted treatment (MAT), integrated re-entry services that includes alternatives to incarceration, and relationships with multiple training institutions throughout the city. Furthermore, CHS has been engaged in an effort to better identify people with neurodevelopmental/neurocognitive disorders and to utilize psychological testing to connect people to services in the community (e.g., with OPWDD).
Minimum Qualifications
A valid New York State license and current registration to practice as a Registered Professional Nurse and,
Current certification as a Nurse Practitioner issued by the New York State Department of Education and,
A current certificate issued by the State Education Department to prescribe medication, and,
Two years satisfactory experience as a Nurse Practitioner, or two years satisfactory experience as a Registered Professional Nurse, eighteen months of which must be as a Nurse Practitioner
At least 3 years of hospital experience both as RN & NP in Behavioral Health
An equivalent and satisfactory combination of education and experience may be considered

Demonstrate advanced knowledge of psychopathology in assessing patients in need of mental health treatment, including mental status examination and diagnosis.
Demonstrate advanced knowledge of psychopharmacology as it pertains to the patient population being treated and writing prescriptions.
Responsibilities:
Under the supervision of a Senior/Supervising Psychiatrist in the Mental Health Service of CHS, this position requires the provision of high quality, team-based psychiatric services including psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. The mental health service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units (“mental observation units”) for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, outpatient-level clinics in each of the facilities, substance use treatment, integrated re-entry services, and relationships with multiple training institutions throughout the city.
Each of the facilities in the jail system has unique patient population and environmental characteristics; each psychiatric Nurse Practitioner will be matched with the population that most closely meets their clinical expertise and desired work location and schedule. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical.
Responsibilities Include:
Provide psychiatric care, including admission evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs
Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information.
Evaluate patients for the need for a higher level of care, including hospitalization and escalate care as appropriate. If a patient is hospitalized, maintain contact with the hospital psychiatric staff to ensure continuity of care
Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare services. Document all activities in the electronic medical record in a timely, concise and clear manner
Integrate nursing and medical concepts in planning care for medically complicated psychiatric patients.
Provide consultation to non-prescribing mental health staff and medical staff about diagnosis, treatment and/or the need for psychiatric medication
Counsel patients using advanced psychotherapeutic techniques, including in group settings if needed
Participate in team meetings with the treatment staff, on a daily basis if working on a mental observation unit and on a weekly basis if working in one of the clinics.
Engage in clinical supervision, as indicated, with the Supervising or Senior psychiatrist
Provide clinical oversight for psychiatric nurse practitioner trainees or other trainees, as needed, including appropriate direct supervision and documentation of evaluation and feedback
Other duties as assigned by the Senior Psychiatrist or Mental Health Manager for the facility, Medical Director or Chief of Service for Psychiatry, or their respective designees

Assesses and records a patient and family physical history
Completes a physical assessment as needed

Demonstrates competence in collaborating with members of the health care team in the revision of the plan of care and documents the same
Demonstrates safety and accuracy in performing treatments and procedures according to the nursing standard of care and practice by:
Consistently demonstrating competency in infection control, utilizing universal precautions and following standards of practice in caring for the infectious patient.
Demonstrates safety by being observant of the environment and reporting or eliminating any potential hazards.
Consistently transcribes medication orders accurately
Consistently administers medications safely
Consistently documents administration of and all information related to Medications
Ensure compliance with the Vaccine for Children Program for annual and ongoing vaccines management. Ensuring compliance with program requirements.
Provide education specific to the patient's relevant health care needs, in ways understandable to the patient and/or family such as:
The safe & effective use of medication, if any
The effective use of medical equipment, if any
The instruction of potential drug-food interactions
Counseling on modified diets, as appropriate
When and how to obtain further treatment, if needed
Qualifications
