GENERAL STATEMENT OF DUTIES: Under the direction of the Chief Medical Officer or designee, performs evaluations, diagnoses, ongoing assessments, and treatment of individuals with severe and persistent mental illness, serious emotional disturbance, and/or developmental disabilities. Identifies client needs, makes treatment plans and referrals, and provides medication management and psycho-educational support in collaboration with existing medical staff and program; performs related work as required. MINIMUM REQUIREMENTS: Educational Requirements License Requirements Master's degree. Graduate of an accredited Physician Assistant Program Fully licensed in the State of Michigan as a Physician Assistant, has Michigan Controlled Substance License, Current MCCPA certification. ADDITIONAL REQUIREMENTS: Must have a valid Michigan driver's license, current automobile insurance, and the use of a vehicle during work hours. PREFERENCES: Three (3) years' experience working as a Certified Physician Assistant AND three (3) years clinical experience working with individuals with severe and persistent mental illness and/or developmental disabilities, including assessment, medication management, and crisis intervention in the outpatient or inpatient setting. General Statement: Under the direction of the Manager of Behavioral Health or designee the Health Coach II conducts a set of non-clinical activities that requires the use of independent judgment to engage, educate, and offer support to individuals and their families to successfully connect them to relevant health services, including prevention, diagnosis, timely treatment, recovery management, education, transportation, and follow up. They contribute to the efficient coordination of patient care through collaborative practice within the health center team, as well as the provider network.
